The RN Care Manager in PACT demonstrates leadership in delivering efficient, comprehensive, and continuous patient-driven holistic care through active collaboration, communication, and coordination of resources. The RN Care Manager works in partnership with the assigned Primary Care Provider and panel of patients for effective disease and chronic care management.

Duties

Major duties include but are not limited to: Demonstrates use of the nursing process in assessing planning, implementing, documenting, and evaluating care for a designated group of patients across the continuum of care, recognizing the age related cognitive, physical, emotional, and chronological maturation needs of the adult and geriatric patient. Triage and apply a collaborative team approach in identifying, analyzing, and resolving patient care problems. Collaborate with patients, family and care providers in any and all settings where care is being provided to evaluate and update changes in the patient-driven life plan and patient management. Review options with patients and families, including costs, alternatives, risks/benefits, and services. Advocate fiscal responsibility through monitoring utilization of resources. Assist with the navigation of health care systems and intervene or refer as needed to facilitate the achievement of patient outcomes. Enhance patient self-management by providing information regarding all options, choices, and resources. Provide care, respecting patient's personal values, cultural, and belief system. Actively participate in care delivery with priority attention to patient preference. Provide and promote patient-driven care in a safe, efficient, and cost-effective manner, using evidence-based principles. Utilize approved Protocols and Guidelines to facilitate autonomy in providing care. Promote cost effective use of resources; effective least-expensive strategies expected in non-face-to-face approaches (including Telehealth) Maintain privacy and confidentiality of all patient information, including electronic, print, and conversations. Serve as a resource for patient and family rights, responsibilities, and decision-making information and processes. Apply skills, such as motivational interviewing to promote patient self-management toward patient-driven holistic care plan for life. Promote health promotion and risk reduction strategies. Guide others in the recognition of and participates in the resolution of ethical issues. Formally share information gained from continuing education experiences within or across units and/or within the medical center. Act as a role model in fulfilling work commitments, seeking out and assuming additional responsibilities and assisting others with career development. - Coach and precept students and novice nursing staff members. Promote wellness by empowering the patient to self-management through education and patient-driven health care delivery. Collaborate with members of both Teamlet and larger Teams in identifying problems, collecting and analyzing data, and problem resolution. Work effectively with the patient/family to respect and support their values, preferences, needs and goals. Collaborate with and develop relationships both internally and externally to better meet the patient's needs. Actively participate in policies, procedures, and standards to promote evidence-based patient-driven care. Complete mandatory reviews, including motivational interviewing, in order to promote patient self-management toward individualized life plan of care. Maintain expertise in chronic care/disease management and Health Promotion/Disease Prevention. The incumbent has responsibility and accountability for all scheduling functions. This includes making, changing, and canceling patient appointments in accordance with clinic profiles and special instructions, as well as all applicable VHA regulations governing the scheduling process. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) OR, The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR, an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR, a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR, an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR, a BSN with approximately 1-2 years of nursing practice/experience; OR, a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR, ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR, a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR, a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR, a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Moderate lifting, 15-44 pounds; Light carrying, under 15 pounds; Pushing 1 hour; Walking 2-4 hours, Standing 2-4 hours; Kneeling 1 hour; Repeated bending 1 hour; Climbing, legs only 1 hour; Both legs required; Ability to distinguish basic colors; Ability to distinguish shades of colors; Hearing (aid permitted); Working closely with others; This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, kneeling, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or during patient procedures and care. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. Use of fingers, both hands, and near and far vision is required.
