The incumbent serves as Program Coordinator at the GS-12 grade and completes all aspects administrative and program implementation for the Community Adult Day Health Care (CADHC) Program and the Program for All-Inclusive Care of the Elderly (PACE). The position is located in the Rocky Mountain Regional Medical Center and includes ongoing travel to the Colorado Springs and Pueblo regions as well as other locations as necessary to manage programmatic and clinical functions.
Duties
- Duties are to assist with case finding, providing referral and support to the VA funded PACE and Community Adult Day Healthcare (CADHC) programs, serving as a field liaison to ensure that Veterans enrolled in the program are receiving needed service through the community PACE and CADHC programs.
- Serves as a liaison between VA and PACE/CADHC. Veterans served are primarily geriatric, many with diagnosis of dementia, and in
need of a variety of social services. Many reside in rural areas.
- Provides the following functions independently with ongoing communication and consultation with their supervisor as needed: Administers information and analytical systems to evaluate and enhance the quality of service provided to patients.
- Develops and implements information systems to track service activities including visits, referrals, inpatient
days, patient demographics, equipment utilization, billing, and patient satisfaction for both the CADHC and PACE programs.
- Performs a wide range of services and provides technical assistance to VA and community agencies with specific focus on the PACE and CADHC programs. Technical assistance provided is based on monitoring activities and program reviews, and the development and analysis of information. In addition, the incumbent uses sound judgment in the interpretation of trends and patterns, the application of guidelines, and the recommendation of program enhancements.
- Performs program analysis, development, review, research, design, and contract work as possible with various agencies.
- Is experienced in providing clinical functions that involve specialized treatment of complex physical and/or mental illness.
- Incorporates complex multiple causation in differential diagnosis and treatment of patients, including making psychosocial and psychiatric diagnoses within approved clinical privileges and scope of practice.
- Develops and implements methods for measuring effectiveness of social work practice and services in specialty areas, utilizing outcome evaluations to improved treatment services.
- Demonstrate the ability to design changes based on empirical finding.
- Offers consultation to colleagues and students on the psychosocial treatment of patients treated in specialty areas, rendering professional opinions based on experience and expertise and role modeling effective social work practice skills.
- Demonstrates the ability to teach and mentor staff and students in the special area of practice and provides supervision for licensure or specialty certifications.
- Serves as a liaison between VHA and community stakeholders who are involved in the payment authorization process including, but not limited to Office of Community Care, Payment Operation Management (POM), GEC VISN and National Office points of contact, and PACE and CADHC providers.
- Initiates referrals to community PACE/CADHC for case management services and monitors to ensure that services are provided. They provide crisis intervention services, seeking to address the cause as well as the presenting complaint, coordinates family conferences, and serves as liaison to family members.
- Working knowledge and experience in use of medical and mental health diagnoses, disabilities, and treatment procedures; this includes acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology.
- Maintains knowledge of benefits and services relating to special programs, service-connected compensation, and non-service-connected pension is required. They facilitate referrals based upon the veteran's needs and eligibility.
- Provides both informal and formal education for Geriatrics and Extended Care to VA staff as well as PACE/CADHC staff on any changes in VA policy concerning the PACE/CADHC/VA programs.

Qualifications

Basic Requirements:
Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part)
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to the social worker position GS-0185 series in the VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/ORHM//T38Hybrids.

Grade Determinations:
Social Worker (Program Coordinator) GS-12
Experience/Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, candidates must demonstrate all of the following KSAs:
Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
Ability to supervise multidisciplinary staff assigned to the program.
Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
Ability to provide training, orientation, and guidance within clinical practice.
Assignments: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS-12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.
Preferred Experience: Two years of geriatric experience is preferred.
