The position as a member of the Butler VA Heath Care's Quality and Patient Safety (QPS) Department Program is responsible for demonstrating leadership, experience, and creative approaches to the execution of program management responsibilities. The position is responsible for assisting with the ongoing development, coordination, and implementation of Butler VA Health Care's QPS programs such as Infection Control/MDRO prevention, Performance Improvement, Patient Safety and , Risk Management.

Duties

Major duties include but are not limited to: The High Reliability Organization (HRO) Coordinator is a professional nurse who serves as the instigator of strong, safe clinical processes across the healthcare system. Uses Patient Safety, Systems Redesign (Lean) and Quality Improvement skills to foster the development of a HRO at the unit/service/medical center level. Actively goes out to clinical care areas and works with individual staff, teams, and services to improve safety, quality, and efficiency of care. Works with Quality Management, Patient Safety Manager, and Systems Redesign to identify vulnerabilities and develop processes and programs that advance Patient Care across the continuum. Assist with the ongoing development, coordination, and implementation of Butler VA Health Care's Infection Control/MDRO prevention, Performance Improvement, Patient Safety, Risk Management, Performance Improvement programs and activities. Develop and implement a performance improvement plan and related activities. Monitoring, trending and analyzing data, interpreting The Joint Commission standards, interpreting VA handbooks and directives regarding quality management/performance improvement/patient safety issues Collaborates with the facility Patient Safety Officer Monitors progress of reviews and follow-up action items to ensure completion in a timely manner Utilizes a comprehensive, organized, continuous readiness program to review and identify opportunities for improvement in order to achieve excellence in the quality and safety of the care, treatment, and services provided to patients. Assists in developing processes to assure that all required information is procured and acted upon to meet the expectations of the patients, the facility, The Joint Commission, VA Central Office, Office of Inspector General, VISN, or other internal and external organizations. Conducts reviews to evaluate compliance with program regulations, policies, and procedures and assists with the development of facility policies and procedures, as appropriate. Assists in developing approaches for standardizing system related procedures to minimize variation. Supports facility activities in the coordination and development of data collection and analysis techniques, primarily in the areas of specific Performance Measures and Performance Improvement indicators. Knowledgeable of Performance Improvement principles, statistical processes, and tools including trend and comparative analyses and has the expertise to make recommendations based on findings. Facilitates education of personnel on performance improvement both in the facility orientation of employees and in specific training for those involved in the performance improvement activities within services, on committees, and/or within performance improvement teams. Provides clinical expertise and consultation to assist in data driven decision making and performance improvement matters. Participates in the development of innovative approaches to the establishment, enhancement, modification, and/or improvements in a variety of facility programs related to performance improvement/quality management. Reviews service/section/committee meeting minutes for assessment and improvement activities including findings, recommendations, follow-up actions and evaluations. Facilitates and maintains a record tracking system for monitoring a variety of information, potential problem areas and action plans, and coordinates action plans for addressing opportunities for improvement. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) OR, The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR, an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR, a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR, an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR, a BSN with approximately 1-2 years of nursing practice/experience; OR, a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR, ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR, a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR, a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR, a Doctoral degree and approximately 2-3 years of nursing practice/experience. Preferred Experience: 5 years of RN experience. Quality management experience to include high Reliability organization, continuous process improvement, and patient safety. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of walking, standing, computer use and or sitting. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities with short notice based on needs.
