Homeless Program is comprised of Health Care for Homeless Veterans, Housing & Urban Development VA Supported Housing, Grant & Per Diem (GPD) & Veterans Lodge supportive housing programs. GPD provides safe supportive housing & services for homeless Veterans through grants with community-based programs (CBG). Since inception in 1994, the number of CBPs serving Veterans as GPD grantees has increased requiring uniform inspection, oversight procedures, more staff & support from VA medical facilities.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU Duties include, but are not limited to: Providing services to and oversight of the GPD-funded community-based programs as outlined in the Grant Per Diem VHA Handbook 1162.01 and the GPD regulations. Including oversight of the GPD Program, assembling team of experts for inspection of the program site(s) and oversight of participants' care and case management, as necessary. Participating in the development of referral processes for admission to the program and managing extension requests for participants Initiating and monitoring the billing process for per diem payments by ensuring that invoices are accurate and processed within policy timelines Ensuring the clinical documentation, program required documentation and program records are in compliance with national, program, VISN and local VA facility policy and procedures in accordance with GPD provider original grant documenting the services to be provided Verifying Veteran status and eligibility of program participants Verifying admission and discharge dates of program participants for billing purposes Collecting and submitting GPD-funded program participant data as outlined by Homeless Operations Management and Evaluation System (HOMES) procedures, complying with criminal conflict of interest laws and Executive Branch Standards of Conduct to avoid conflicts of interest in carrying out liaison duties and ensuring coordination of care for homeless Veterans involved in GPD funded programs. This coordination of care is shown through assessment, planning, monitoring advocacy, and referral activities by both the GPD Liaison and the GPD funded provider to assist the Veteran in their transition from being homeless to becoming housed. Practices that support coordination of care include, but are not limited to: Coordinated screening and referral processes, Regular service and/or treatment planning which includes the Veteran, the GPD provider, and the GPD Liaison Service and/or treatment plans which clearly delineate the roles of those responsible for the service provision to reduce duplication of services Regular case discussions with the Veteran, GPD Liaison, and GPD provider to discuss progress and plan revisions. Providing oversight of GPD program participants' care and case management provided by the program at sites where organizations are receiving VA-funded per diem payments. The level of intensity of case management or service provision to homeless Veterans is determined by the assessed needs of the Veteran. As case management services exist within GPD-funded programs, there is coordination between the GPD liaison and the GPD provider to facilitate continuity in service provisions and to reduce duplication. Case management is defined as the provision of services by VA clinical staff to homeless Veterans, which includes: Monitoring the care in and assessing the compliance of the program receiving per diem as outlined in the recipient's original grant application or approved Change of Scope Intervening to facilitate compliance or correction of the program when appropriate Arranging and coordinating the care, when necessary. NOTE: The term "when necessary" could mean when determined by the GPD Liaison that the provider does not offer particular services needed by the Veteran, when requested by the Veteran and/or when requested by the GPD provider (on a case-by-case basis); or when determined necessary by the GPD Liaison from admission screening Linking and referring to VA medical facilities, VA Regional Offices, and/or community agencies, when necessary. Intervening and advocating on behalf of the Veteran to fill gaps in the delivery of services, when necessary Maintaining all program records as required by the GPD Program Handbook Successfully completing and maintaining all nationally mandated training Work Schedule: Fulltime, Monday thru Friday 8:00 a.m. to 4:30 p.m. Telework: Ad hoc Virtual: This is not a virtual position. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: Social Worker (Program Coordinator), GS-12 Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Prior experience with Homeless or At-Risk for Homeless populations Prior experience with community outreach Prior experience with coordinating and accessing resources Prior experience with case management and conducting assessments References: VA Handbook 5005/120 Part II Appendix G39 Social Worker Qualification Standard GS-0185 Veterans Health Administration September 10, 2019. The full performance level of this vacancy is GS-12. Physical Requirements: The work is sedentary. Some work may require movement between offices, hospitals, warehouses, and similar areas for meetings and to conduct work. Work may also require walking/standing, in conjunctions with travel to and attendance at meetings and/or conferences away from the work site.
