The CRH RN Nurse supports the VISN 17 CRH under the guidance of the Veterans Health Administration (VHA) which provides health care services to Veteran patients across the continuum of care in both institutional and non-institutional settings. The CRH RN Nurse supports the location to and from which care is delivered, including but not limited to VA clinical locations and provider alternate worksites.

Duties

The incumbent in this position will support the VISN 17 CRH which may provide Telehealth and face-to-face services not only to clinical areas within the VISN 17 CRH but also other VISN CRHs throughout the nation. Services provided by CRH staff are conducted via multiple modalities, including virtual and in-person care. Provides direct care to one or multiple panels of patients. Collaborates with other members of the expanded health care team, facilitates coordination and transition of care between services, and may involve the patient and family in the decision-making process. Minimizes fragmentation of healthcare delivery and optimizes quality of patient and family outcomes in a cost-effective manner. The incumbent works effectively with all levels of staff, to include collaborative efforts with other health professionals, providing ongoing care management/coordination services. Provides coverage for VISN 17 CRH RNs when they are unavailable to perform these duties. Travel may be required for this position. This VISN 17 CRH RN position is organizationally located in the VISN or facility CRH line and attached to VISN 17 facility. The department description and patient population served is defined in applicable Scope of Service. Due to the virtual nature of the work and the provision of care using technology, the CRH RN may telework and/or be within reasonable proximity of a VISN 17 facility. The CRH RN is responsible for managing care across the health care continuum for a select group of clients. Responsible and accountable for all elements of the nursing process when providing direct patient care. Independently provides professional assessment, coordination, telephone triage and planning of multiple health care services; acts on behalf of the Veteran to assure that necessary clinical services are received, and that progress is being made. The incumbent is responsible for providing direct patient care to Veterans and training and support to CRH and Spoke site staff as appropriate. The CRH PACT RN uses the nursing process and evidence-based practice to provide care to the Veteran utilizing the Patient Aligned Care Team (PACT) model. This is done in collaboration with the PACT Team (Patient, Primary Care Provider, Spoke Site RN Care Manager, Clinical Associate and Clerical Associate) and expanded Team (Psychologist, Social Worker, Clinical Pharmacy Specialist, and other clinicians) as well as families/caregivers and internal and community-based services. The CRH PACT RN provides patient and family health education with a focus on chronic disease self-management, prevention, and wellness, based on the patient's goals. The CRH PACT RN is a member of the Primary Care (PC) team. The CRH PACT RN is familiar regarding the PACT Handbook- modeling PACT principles. The CRH PACT RN follows standard processes to be used by the PACT team. Uses data registries to assist in identifying high risk patients for chronic disease management and care coordination. The position may require travel to provide training and support CRH Spoke sites and possible physical deployment to Spoke sites to provide care to a panel of patients for an extended period of time. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Practicing Registered Nurse with at least 5 years of nursing experience, with the past 3 in outpatient mental health nursing and the past 1 year in VHA primary care. One year of Telehealth Virtual Modality experience preferred. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Lifting up to 35·pounds for 8-12 hours, intermittently, moderate carrying (15-35 pounds) for 8 to 12 hours, intermittently, lifting over 35 pounds with assistive devices in accordance with the Safe Patient Handling Policy except in emergent situations requiring prompt patient care, pulling for 8 to 12 hours intermittently, pushing for 8-12 hours, intermittently, reaching above the shoulders for 8-12 hours, intermittently, both hands required, walking 8-12 hours, intermittently, standing 8-12 hours intermittently, kneeling 8 to 12 hours, intermittently, repeated bending for 8-12 hours, intermittently, climbing and use of both legs and arms for 8-12 hours, intermittently, both legs required, near vision at 13 to 16" Jaeger 1 to 4, far vision correctable in one eye to 20/20 and to 20/40 in the other eye, ability to distinguish basic colors, hearing aid permitted, twisting, simple grasp and fine manipulation.

Education

The Associate Degree Nursing (ADN), Bachelor of Science degree in Nursing (BSN), or Master of Science degree in Nursing (MSN) must be fully accredited by the Accreditation Commission for Education in Nursing (ACEN) or the Commission On Collegiate Nursing Education (CCNE).
