The Program Support Assistant provides specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model.
Duties
Major duties include but are not limited to:
Providing specialized and expert administrative patient support while working collaboratively in an interdisciplinary coordinated care delivery model.
Developing and maintaining effective and efficient communication with the patient, interdisciplinary coordinated care delivery model teams, VA medical centers, and other agencies.
Using a wide range of clinical flow processes relating to access of care across multiple clinics, specialties and/or care in the community resources.
Setting priorities and deadlines while adjusting the flow and sequencing of the work to meet team and patient needs.
Entering appropriate information into the patients electronic record and scanning medical records to the electronic health record with accurate and timely management of assigned workload.
Monitoring pre-appointment information and requirements to assure readiness for patient visit/procedure.
Managing patient systems to verify and validate accuracy and resolve issues.
Evaluating patient information and clinic schedule lists to determine whether the patient requires an immediate appointment.
Participating in team huddles and team meetings to manage, plan, problem solve and follow up with patient care by sharing information and collaborating with the interdisciplinary team.
Assisting the team to reinforce the plan of care and self-help solutions.
Informing team members about shared patients who receive their care at multiple VA centers or those who receive care in the community under multiple specialty areas.
Ability to demonstrate and execute highly developed skills with MS Excel and multiple health care operating systems.
Work Schedule: Monday- Friday 8:00am- 4:30pm

Qualifications
To qualify for this position, all facility applicants must meet all requirements within 30 days of the closing date of this announcement, 01/02/2024.
Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade.

You may qualify based on your experience and/or education as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade (GS-05) in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Experience in a medical clerical or administration position that required knowledge of medical terminology and interaction with a professional staff, management and patients; skill in the use computer systems and applications; ability to conduct quality assurance process improvement checks and studies; skill in compiling readily available data from prescribed sources; knowledge of clinical processes and services to function in and interact with various clinics; ability to apply policies and procedures to operational activities; knowledge of standard office equipment such as typewriters, copiers, fax machines and office automated systems to produce letters, memos, reports ect.

You will receive credit for all qualifying experience, including volunteer experience.
Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
Physical Requirements: The work is mostly sedentary, there can be demands for standing, walking, bending and long periods of sitting at the computer, answering the telephone, and transferring calls frequently. Lifts, bends, pulls, stoops, moves bags, boxes, and charts.
Working Conditions: The work is performed in an office setting with adequate lighting and other facilities with no unusual health hazards. Due to the contact with patients and public, incumbent is exposed to occasional verbal abuse. The incumbent oversees normal safety precautions.
