A GS-5 Certified Nursing Assistant (CNA) working Restorative Care/Therapeutic Activity program provides restorative care and therapeutic activities based on the Community Living Centers Restorative Care Program. Restorative Care refers to nursing interventions that promote the residents ability to adapt and adjust to living as independently and safely as is possible.

The Restorative Care Certified Nursing Assistant (CNA) GS-05 has current State certification and shall have completed the required education and experience. Individuals assigned as GS-05 Restorative Care CNA's are considered to be a full performance level. CNA's at this level function as a member of the nursing care team and assist licensed nursing staff in the care of patient's/residents receiving nursing care who are in acute, sub-acute or chronic state of illness. The patient/resident assignment typically involves more complex nursing needs which can vary within range of predictable to unpredictable requirement. CNA's at this level also assume more of a peer education /mentorship role. The Restorative Care CNA is responsible to and directly supervised by a nurse (RN/LPN)
Supports Nursing Care
1. Promotes the incorporation of current resident care by actively contributing to the development of the resident's overall health plan and in carrying out assigned interventions.
2. Prioritizes and organizes workload based on unit needs.
3. Involved in some degree in the development and modification of treatment plans.
4. Makes observations of subtle changes, both physical and emotional, in resident's condition, and accurately and thoroughly documents care delivered and resident's/resident's response to care that could require immediate modification of the care plan.
5. Appropriately seeks assistance and guidance by informing the nurse (RN/LPN) or provider of changes in resident's condition.
6. Recognizes and provides emotional, cultural, spiritual, socio-economic and age related factors in providing resident care. 7. Demonstrates skill in the use of therapeutic communication. 8. Reinforces team's instructions to resident and family by following developed teaching plans and education policies.
Enters Information onto the Resident's Records/Charts
1. Enters specific information into the electronic medical records, beside worksheet, treatment sheets, etc.
2. Enters resident information using commonly used medical terminology such as terms of test, diet changes, or test results.
Resident Care
1. Performs care according the CLC Restorative Care Program: Bowel and Bladder retraining, Bed Mobility, Ambulation and Transfers, ADL, Amputation and Prosthetic Care, ROM, Splint and Brace Care, and Restorative Dining.
2. Carries out the restorative care according to the plan of care for the resident.
3. Using interventions that assist or promote the residents ability to attain his or her maximum functional potential.
4. Performs active and passive range of motion.
5. Splint or Brace Assistance-Provides assistance where care partners provide verbal and physical guidance and direction that teaches the resident how to apply.
6. Identifies and performs proper transfer procedures using appropriate techniques, body mechanics, and equipment.
7. Documents resident's responses to interventions in the medical record as indicated.
8. Communicates resident's status and restorative care requirements.
9. Educates resident on his/her restorative care programs.
10. Develops and conducts small group activities on the unit for residents in order to engage the resident in a meaningful way throughout the day and promote social interaction.
11. Participates in team conferences for planning resident care and health teaching, contributes to the plan of care, and documents information in resident's medical record following stated guidelines in nursing service policies and procedures manual.
12. Understands the resident's condition and medical treatment in order to be able to observe and report significant changes that require action by the nurse or physician. Makes visual observations of changes in resident's physical or emotional condition and keeps the team informed.
Resident Teaching
1. Assists in teaching residents and family members the procedures for proper health care, safety and the necessity for consistency.
2. Assists with general health education, e.g., residents/family/care providers of importance of skin care, position changes, adhering to medication/dietary regime.
Work Schedule: flexible day tour, can be compressed, rotating weekends and holidays; every other weekend

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Certification: A full, active and unrestricted State certification is required.
Proficient in both spoken and written English
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Experience. One year of progressively responsible assignments and experience equivalent to the GS-4 level which demonstrates knowledge, skills, and abilities that are directly related to the specific assignment. In addition, the candidate must demonstrate the professional KSAs in subparagraph (c) below;
OR
Education. Successful completion of a 4-year course of study above high school leading to a bachelor's degree that included 24 semester hours of courses related to health care or possession of a bachelor's degree;
AND
Demonstrated KSAs
1. Ability to assist in the full range of nursing care to patients/residents with physical and/or behavioral problems in a hospital, long term care or outpatient setting under the direction of a Registered Nurse and/or Licensed Vocational Nurse/Licensed Practical Nurse.
2. Ability to communicate orally with patients/residents, families, interdisciplinary team and other personnel. This includes serving as a preceptor to new [NAs] by assisting with the coordination of their orientation and overseeing/assessing their practical experience while in a clinical setting
3. Ability to recognize and react to emergent patient/resident care situations and intervene while waiting for assistance.
For example, recognizing need for basic life support, controlling bleeding and assisting with behavior crisis, etc
Preferred Experience: 2-5 years experience as a CNA in long term or physical rehabilitation setting preferred, certification as a restorative nursing assistant preferred.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don personal protective equipment (PPE) in isolation situations or operative/invasive procedures at the bedside. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders.
