As an RN caring for patients with structural heart disease, you will be part of a dynamic team and evolving science. The Structural Heart Nurse Coordinator requires advanced nursing knowledge of patients with cardiovascular and valvular heart disease to ensure patient safety and optimal clinical outcomes. The Structural Heart Nurse Coordinator plans, directs and evaluates the care provided to the patients and their families. This is a front-line RN working in inpatient & outpatient areas.

Duties

Coordinate the delivery of care for patients with Structural Heart Disease, including but not limited to: patient evaluation, triage to appropriate care setting, arranging travel, scheduling cardiologic and non-cardiologic diagnostic and therapeutic procedures, scheduling admissions and procedures, and post procedure follow up. Receive and review all consults to the Structural Heart Clinic, reviewing with providers as needed. Assist with the coordination of inter-facility consults and requests from outside VA facilities. Communicate with referring providers and support staff, the interdisciplinary structural heart team, and other health care professionals to deliver optimal patient care. Serve as the liaison and communication link between patients and their families with the referring providers and cardiac specialists. Provide clinical consultation and education related to the management and care of the SHD patient to staff, physicians, and other disciplines who may have contact or care responsibilities for this patient population. Participate in development and implementation of clinical practice guidelines to streamline cardiology practice related to SHD throughout the Zablocki VA Medical Center. Identify learning needs of patients/families and provide individualized and comprehensive teaching related to the patient's structural heart condition. Develop a therapeutic relationship with the patient and family and maximize the patient's participation and control in their own recovery. Coordinate care amongst multiple services, including but not limited to the operating room, Cardiac Cath lab, Electrophysiology lab, anesthesia, Echocardiography, Cardiac Surgery, and inpatient services. Assist Structural Heart providers in clinic setting as needed. Provide active surveillance of patients waiting intervention. Perform administrative functions that ensure continuity of care and appropriate access to care. Assists with the evaluation of the effectiveness of interventions in the structural heart program and identifies performance improvement opportunities. Monitor patient outcomes of transcatheter valve replacement and repair procedures and submit required data to the Transcatheter Valve Therapy registry. Demonstrate ability to correctly interpret electrocardiograms and cardiac rhythms. EKG Administrator responsibilities Examine facility level processes in the management of EKGs for facility vulnerabilities Coordinate with Biomedical Engineering on the creation and maintenance of an EKG cart location roster. Monitor EKG in MUSE system for critical elements/location Confirm patient demographic errors on EKG tracings sent to MUSE and correct as needed Route EKG studies to Cardiologist physician in-basket in MUSE Manage initial set up of Cardiologist physician user profile in MUSE system, input Cardiologist physician users in MUSE system, and inactivate old users On a weekly basis, verify the workflow configuration on the MUSE system including management of location ranges for user profiles, configuration of physician in-baskets, and oversight of filters applied to the system Monday-Friday, run database reports on the MUSE system to query for unread/unconfirmed EKG tracings, mismatched EKG tracings, or other failures to properly route EKG tracings for physician over-read. Complete clinical documentation in the electronic medical record, as needed Monitor and assist with follow-up of abnormal findings including referral of any incidental findings Monitor adherence to the operational and clinical protocols established for the program Lead the collection and submission of data from a variety of sources Collaborate with CBOC staff on performance of regularly planned reviews of ECG carts to look for errors in transmission of ECG studies to the ECG image management system. Perform all other duties as assigned. The incumbent is clinically and administratively responsible to the Nurse Manager of Cardiology Services & Procedural Labs. Compliance with all healthcare systems, policy rules, regulations, and bylaws as mandatory. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Experience in cardiology strongly preferred Advanced nursing knowledge of patients with cardiovascular and valvular heart disease strongly preferred Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
