The Specialty Care Nurse Educator (SNE) is a Registered Professional Nurse who is directly accountable to the unit's Assistant Nurse Manager. While working in Partnership with the Nurse Manager (NM) and Assistant Nurse Manager (ANM), functions as a nurse, educator and coordinator for the Specialty Care Nurse Case Managers and Infusion clinic. Duties

The SNE is a partner, leader, and actively involved in education of unit nursing staff during orientation, ongoing needs, and in sustaining excellence in nursing practice on the unit. The SNE demonstrates superior clinical expertise, leadership and creative approaches to management of complex ethical care. In addition, applies clinical skills necessary to identify, coordinate and /or identify discipline-specific education. The SNE assists NM and ANM in evaluating current practice needs of RN staff through observation, audits, and data collection and develops a plan for educating and maintaining consistent education needs. The SNE will facilitate the performance improvement (PI) process and promote goals related to key clinical outcomes, and the adherence to specialty recommendations of care and standards of practice from facility accrediting bodies, organizations, and directives focused on evidence-based nursing practice. As a registered nurse, the SNE is responsible for and collaborates with the NM and ANM in the professional development of clinically competent staff. Educational materials and forums to be developed by the SNE in collaboration with the NM and ANM. The SNE collaborates with SNEs from other units and the education department to promote nursing excellence throughout the organization. As a Float NCM applicant will be assigned to cover for Specialty Services to ensure ongoing patient care needs are met: Takes responsibility for Outpatient clinic coordination and patient flow management, including appointment management, triage of patients, patient education, monitoring of patient progress toward predetermined goals and timely follow-up. Participates in clinic for setting patient goals of care as part of the interdisciplinary team. Utilizes the nursing process in managing, developing, implementing and evaluating each patient's individual plan in collaboration with the interdisciplinary care team to attain specific clinical outcomes in the outpatient specialty clinic settings. Provides support to providers daily: coordinating dual care with community and other VA facilities; ensuring all needed records, tests and other information is available to provider at time of consultation to allow provider to focus on treatment plan and Veteran's questions in an appropriate and timely manner. Receives and triages consults with the specialty service for one of the following: 1) Provide Scheduling Direction 2) Convert to an E-Consult, providing ordering provider with recommendations and test/images 3) Forward to Community Care for authorization and scheduling Communicates with Veterans their health care options including local VA, other VA Medical Centers and the community based on eligibility criteria for community care per the National Directives (ex. MISSION Act). Works closely with other VA Medical Centers, contracted facilities and community facilities to remove obstacles that would impede the Veteran from utilizing the services of other VA medical centers or community not available at our facility. Participates in activities which meet the local and national performance standards contract for the Medical Center. Participates in the development and implementation of recommended policies and procedures as related to Case Management and/or designated clinical area. Provides for the educational needs of the health care team regarding Case Management and/or topics specific to designated clinical care. Maintains and upgrades own knowledge base and skills regarding Case Management and/or specific clinical areas and applies this knowledge to nursing practice. Participates in clinical research/EBP within designated area and/or related to Case Management. Establishes effective networking relationships with peers and professionals within the community as related to Case Management. Receives and triages new patient consults, assists with ordering and coordinating appropriate tests, and communications with the referring provider, private institutions, and physicians, to expedite patient appointments. Collaborates with clinic staff to ensure patients are given timely appointments and expedite new patient consults. Follows up with patients, monitoring and ensuring adherence to treatments plans, intervening in accordance with prescribed clinical guidelines and providing reports on all cases. Provides clinical care and case management to Veterans in the Specialty Clinics. Triages and applies a collaborative team approach in identifying, analyzing, and resolving patient care problems. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) Current, full, active, and unrestricted registration as a graduate professional nurse AND nurse practitioner in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Possess a master's degree from a program accredited by the NLNAC or CCNE and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. Preferred Experience: 3-5 years nursing experience preferred 3-5 years nurse case management experience preferred VA experience preferred Outpatient experience preferred BSN preferred Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelors degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Masters degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Walking, bending, stooping, and some lifting in the direct care of patients. There may also be extended periods of sitting.
