This position is located in Pharmacy Service, Boise VA Medical Center, Boise, ID. This position provides support to Clinical Pharmacy Services. This position functions an ambulatory care clinical pharmacist specializing in pain management

Duties

Duties include but are not limited to the following: In collaboration with other pain management team members, is responsible for the development of a therapeutic plan which is intended to achieve definite drug-therapy outcomes that improve the patient's quality of life. Interviews patients to determine medical history and reviews history for potential drug interactions, poly-pharmacy, therapeutic duplication, noncompliance and adverse drug reactions. Assess the need for risk reduction related to opiate therapy including dose, signed pain contract, recent UA, and benzodiazepine co-administration. Works with patient and providers to maximize benefit and reduce risk. Monitors and manages medication therapy under an approved scope of practice, with an emphasis in pain management, including initial assessment of new patients (chart review, history taking, medication reconciliation) and follow-up of patients in clinics and in the inpatient and long-term care settings. Management may include opioid medication management. Provides drug information to physicians, nurses, trainees, and other health care professionals. Takes patients' drug histories and counsels them on the appropriate use of medications. Ensures that medications are properly reconciled. Provides patients and family members with information about their medication regimen. Participates in interdisciplinary care conferences related to the Pain Management program. Promotes rational pain management strategies utilizing current VA resources and the development of pain management protocols. Provides care/counseling for patients ranging in age from 18 through the geriatric years taking into consideration knowledge based competencies which include diagnosis, physical, psychological, cultural, moral and religious needs of patients. Identifies and prevents drug-related problems and makes recommendations to providers concerning the management of drug related problems and their prevention. Initiates requests for appropriate laboratory and other diagnostic studies when needed for monitoring drug therapy, with notification of the primary provider. This includes follow up (i.e. makes recommendations for dosage changes as appropriate) when non-therapeutic drug levels are obtained. Monitors and analyzes drug therapy regimens with health care providers (based upon pharmacokinetics, patient disease state status, and medication history) to provide the most effective, least toxic, and most economical drug treatment plan. Evaluates drug literature by analysis of experimental design and evaluation, in order to compare and contrast therapeutic regimens and roles for new drugs as it applies to patient care. Review medication profiles and summarizes significant findings which are documented in the medical record and reported to the primary provider. In conjunction with the associate chief of pharmacy- clinical services, provides in-service educational programs directed to the development of the pharmacy staff and education of other health care disciplines. Supervises and precepts pharmacy residents and students as assigned. Presents lectures and conferences to medical, nursing, and pharmacy staff and students concerning pain medication therapy. Educate staff and trainees. Creates an environment to stimulate, encourage, and support the participation of pharmacy personnel in providing drug information to the medical center staff and other facilities as deemed appropriate. Participates in the development and enforcement of Pharmacy and Health System Policies and procedures Assists in the dissemination and enforcement of guidelines established by the Pharmacy and Therapeutics committee regarding the use of formulary, non-formulary, and restricted drugs. Provides follow up on reported adverse drug events in the Primary/Ambulatory Care areas or other areas within the medical center as appropriate. Serves as a consultant to the Pharmacy and Therapeutics Committee on the selection of pharmaceuticals for formulary listing, and prepares drug monographs for formulary requests for the committee Work Schedule: Monday-Friday, 8:00 am-4:30 pm Compressed/Flexible: Not Authorized Telework: Available on ad-hoc basis only. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: (1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) (2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). GS-13 Grade Determinations: Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level. Knowledge, Skills. and Abilities: Pharmacists assigned to this position must demonstrate the following KSAs: a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions. b. Expert understanding of regulatory and quality standards for their program area. c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: PGY-2 Residency in Pain Management or at least three years experience as a pain pharmacist. References: VA HANDBOOK 5005/55 PART II APPENDIX G15 Physical Requirements: There may be some walking, standing, bending, and carrying of light items. Wearing of a respirator may be required at times in the performance of the duties of this position. As a result, employees assigned to these duties cannot have facial hair or any condition that interferes with the face-to face piece seal or valve function of the respirator.
