The incumbent in this position functions as the Case Manager for Central Alabama Veterans Health Care System (CAVHCS) VAMC's Housing and Urban Development / VA Supportive Housing (HUD-VASH). The HUD-VASH Program falls within the Healthcare for Homeless Veterans (HCHV) Programs of the Behavioral Health & Sciences Care Line. The incumbent is responsible for coordination of case management services for veterans receiving housing provided through HUD Section 8 housing vouchers.

The incumbent is responsible for reviewing agreements between Section 8, landlords, Public Housing Authorities and the VA in establishing appropriate referral sites for HUD-VASH placements. The incumbent performs site visits of HUD-VASH apartments to ensure that veterans reside in safe environments in compliance with local housing codes. The incumbent verifies the veteran's status, eligibility for VA medical care, and eligibility of program participation. The incumbent is responsible for ensuring that homeless veterans are considered as primary participants for program acceptance. The incumbent is responsible for collecting and submitting HUD-VASH Program participant data as outlined by NEPEC evaluation procedures. The incumbent is responsible for providing HUD-VASH case management. Case management is defined as the provision of services by VA clinical staff to homeless veterans and includes all activities to provide appropriate treatment and maintain veterans in HUD approved stable housing. The GS-9 Social Worker is under the direct supervision of the Supervisory Social Worker and/or designee and receives clinical, administrative guidance and mentoring as it relates to Social Work practice. GS-11- While functioning as an independent professional, the individual is under the direct supervision of the Chief of Social Work Service and receives clinical, administrative guidance/mentoring as it relates to Social Work practice. The HUD-VASH Program is part of the Behavioral Health & Sciences Care Line, While it is staffed by social worker who is under the direct supervision of the Chief of Social Work, programmatic authority for HUD-VASH is to be supervised by the Manager of Recovery, Rehabilitation, Reintegration Programs in BH&S, who ultimately reports to ACOS, Behavioral Health & Sciences. a. The incumbent will conduct psychosocial assessments to determine needs of veterans and family members. b. The incumbent is an active participant in the treatment planning process with other disciplines. c. The incumbent will participate in discharge planning with other disciplines and will be responsible for ensuring that discharge plans are executed in a manner that is timely and appropriate. d. The incumbent will provide individual, family and group counseling as appropriate. e. The incumbent will provide consultation/education to veterans and families on community resources, advance directives and VA benefits. f. The incumbent will act as a consultant to other team/staff members. g. The incumbent will act as liaison between VA and community resources, to include marketing and public relation duties as needed. h. The incumbent will provide case management to veterans and families, as needed, throughout the continuum of care. i. The incumbent will provide a full range of social work services within commonly accepted standards of social work practice which includes case work, individual, family and group counseling, discharge planning, case management and other services as appropriate. j. The incumbent will address Advance Directives and Organ Donation in accordance with the standards of their work area. k. The incumbent will provide wellness/prevention education and facilitate Patient/family support groups as appropriate. l. The incumbent will as appropriate, supervise the practice of social work students, or a less experienced social worker. m. The incumbent will coordinate treatment and communicate with the care lines to facilitate continuity of care for veterans. n. The incumbent will participate in committee assignments, as deemed necessary by the service manager. o. The incumbent will maintain data and statistical compilations to comply with JC, CARF, VA and medical center policy and procedures. p. The incumbent will assist patients and their significant others with coping and dealing with the loss and grief experiences in disability, terminal illness and death. q. The incumbent will document social work interventions and activities in patient's clinical record utilizing CPRS and ensure appropriate hand off when transferring of patient care. r. The incumbent will participate in a rotation of on-call social workers, including modification of tour of duty to accommodate patient care needs on weekends. s. The incumbent will maintain familiarity and responsibility for all performance measures relating to the HUD-VASH Program. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: basic requirements for employment as a VHA social worker are prescribed by statute in 38 U.S.C. §7402(b)(9), as amended by section 205 of Public Law 106-419, enacted November 1, 2000. To qualify for appointment as a Social Worker in VHA, all applicants must meet the following: Citizenship: Be a citizen of the United States. English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by38 U.S.C. § 7403 (f).Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Exception. VHA may waive the licensure or certification requirement for persons who are otherwise qualified, pending completion of state prerequisites for licensure/certification examinations. This exception only applies at the GS-9 grade level. Grandfathering Provision:The following is the standard grandfathering policy for all title 38 hybrid qualification standards. Please carefully review the qualification standard to determine the specific education and/or licensure/certification/registration requirements that apply to this occupation.(1) All persons employed in VHA (September 10, 2019) in this occupation on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure or certification that are part of the basic requirements of the occupation. For employees who do not meet all of the basic requirements in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply:(2) Such employees in an occupation that requires a licensure or certification, may be reassigned, promoted up to and including the full performance (journey) level, or changed to lower grade within the occupation, but may not be promoted beyond the journey level or placed in supervisory or managerial positions.(3) Employees who are appointed on a temporary basis prior to the effective date of the qualification standard may not have their temporary appointment extended or be reappointed, on a temporary or permanent basis, until they fully meet the basic requirements of the standard.(4) Employees initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration that meet all of the basic requirements of this qualification standard must maintain the required credentials as a condition of employment in the occupation.(5) If an employee who was retained in an occupation listed in 38 U.S.C. § 7401(3) under this provision leaves that occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. GRADE DETERMINATIONS: In addition to the basic requirements for employment, the following criteria must be met when determining the grade of candidates. Social Worker GS-9: (1) Experience, Education, and Licensure. None beyond the basic requirements. (2) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Ability to work with Veterans and family members from various socioeconomic, cultural, ethnic, educational, and other diversified backgrounds utilizing counseling skills. (b) Ability to assess the psychosocial functioning and needs of Veterans and their family members, and to formulate and implemental treatment plan, identifying the Veterans problems, strengths, weaknesses, coping skills, and assistance needed. (c) Ability to implement treatment modalities in working with individuals, families, and groups to achieve treatment goals. This requires judgment and skill in utilizing supportive, problem solving, or crisis intervention techniques. (d) Ability to establish and maintain effective working relationships and communicate with clients, staff and representatives of community agencies. (e) Fundamental knowledge of medical and mental health diagnoses, disabilities, and treatment procedures. This includes acute, chronic, and traumatic illnesses/injuries; common medications and their effects/side effects; and medical terminology. Social Worker GS-11:(1) Experience and Licensure. Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR (2) Education. In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. (3) Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, candidates must demonstrate all of the following KSAs: (a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. (b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. (c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. (d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. (e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The full performance level of this vacancy is GS-11. The actual grade at which an applicant may be selected for this vacancy is in the range of GS-9 to GS-11. Social Worker Qualification Standard GS-0185 Physical Requirements: (See VA Handbook 5019, Occupational Health): The work is primarily sedentary and non-strenuous and is primarily carried out within the confines of the off ice/work space provided. However, some lifting of equipment and materials, pushing/pulling of patient's wheelchairs, moving of lightweight furniture, bending, walking, and commuting to different locations within the Medical Center, Outpatient Clinics and Community Based facilities is required.
