This RN Staff Outpatient at the San Juan Primary Care, Women's Health, MOPC, POPC, Arecibo CBOC, Ceiba CBOC, Guayama CBOC, Vieques Rural Clinic, Utuado Rural Clinic and Comerio Rural Clinic serves as Care Coordinator (CC) at the Patient Aligned Care Teams, or PACT.
Duties
This RN Staff Outpatient at the San Juan Primary Care, Women's Health, MOPC, POPC, Arecibo CBOC, Ceiba CBOC, Guayama CBOC, Vieques Rural Clinic, Utuado Rural Clinic and Comerio Rural Clinic serves as Care Coordinator (CC) at the Patient Aligned Care Teams, or PACT, uses the nursing process and evidence-based practice to collaborate with the patient and Primary Care Provider, family/caregiver, internal and community-based services involved in providing care to the patient) in developing the patient driven holistic care plan for life. This RN Staff Outpatient PACT CC is responsible for collaborating with services internal and external to the VA to facilitate care transition to effectively meet the patients' needs. Direct care to patient as well as coordinate care to ensure continuity and appropriateness of care.
The RN PACT CC provides patient and family health education with a focus on self-management, prevention, and wellness, based on the patient's goals. The RN PACT CC demonstrates leadership by serving as an advocate for patients, as she/he continues to enhance his/her own and the team's professional growth, development, and practice. The professional RN PACT CC is knowledgeable in Care Coordination, enhance skills in the ambulatory setting, and attitudes to coordinate patient care across the continuum of care, such as: Transition management coordinating ongoing support of patients and families over time as they navigate care and relationships among more than one provider and/or more than ono health care setting or service. Coordinates with referrals coordination teams, community care and Primary Care Mental Health (PCMH) aligned with ambulatory care with Veterans Integrated Care Optimizing Resilience (VICTOR) in Primary Care, provide and support care for Women Population.
Work Schedule: Rotating Shifts (6am-6:30pm) Weekdays, Saturdays, Holidays.

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Must be proficient in written and spoken English
Preferred Experience:
2 years of RN experience preferred.
Spanish proficiency preferred.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 year's of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.

Physical Requirements: This position involves lifting, moving and transporting patients, assist with transferring patients from wheelchair to to stretcher or radiology suite table and vice versa. Also assist with transferring patients to wheelchairs and/or may need to escort a patient to other areas. This position involves exposure to blood borne pathogens, fine manipulation, standing, walking, bending, crouching and reaching patients. It requires dealing with the acutely ill, with patients who may display disturbed behaviors, and with stressful situations. Moving patients and weighing patients in excess of 50 kg. or more. Wearing of a respirator may be required at times in the performance of the duties of this position. When required to use respirator, employee cannot have facial hair or any condition that interferes with the face to face piece seal or valve function of the respirator.
