This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Clinical Pharmacist rotates through each operational area including Inpatient and Outpatient at Pharmacy Service of the VA Caribbean Healthcare System (VACHS), and serves as a representative of Pharmacy Service in the various sections of the VACHS. The practice of the Clinical Pharmacist covers direct indirect patient care, drug therapy monitoring, quality assurance, staff/patient education Consultation and research and is often in association with or in support of physicians and other health care practitioners. Duties for this position at the GS-12 may include but not limited to: Interprets written or electronic orders utilizing professional knowledge, VA Policies and procedures, for oral and/or parenteral medication including chemotherapeutic or cytotoxic agents, IV admixtures, investigational drugs and controlled substances. Verifies and inputs orders into computer correctly. Evaluates all orders for correct drug for diagnosis according to current standard of care. Evaluates all orders for drug interactions, incompatibilities, misuse, inadequate/inappropriate response, therapeutic misadventure and duplicate therapy. Evaluates orders for appropriate dose, route, interval and length of therapy. Evaluates orders for allergic and adverse drug reactions. Edits orders correctly per policy (therapeutic interchange, half tablet, VISN 8 indicators, etc.) Enter medication management Progress Notes for the record. Monitors and reconciliates patient drug therapy providing their expertise in drug therapy and drug monitoring laboratory levels. Interviews patients, determines medication history for potential drug interactions, therapeutic duplications, efficacy of current medication regimens, compliance patterns and/or drug reactions and acts to resolve identified problems. Documents non-VA medications, OTCs, herbals and others in CPRS. Contacts providers as appropriate when drug-induced problems are identified. Recommends therapeutic alternatives based upon the patient care assessment and desired outcome, drug safety, efficacy, and cost to optimize drug product selection. Orders laboratory tests used in monitoring drug therapy according to VACHS approved policies or protocols. 15. Assists in formulary management including therapeutic substitutions, non-formulary requests approval/disapproval and medication usage evaluations. Documents interventions in CPRS in a timely and professional manner as appropriate. Reviews, evaluates and solves all E-Pharmacy prescription problems rejected by third party payers, CMOP prescription problems, Combat and Copay claims in a diligent and timely manner. Documents and assesses adverse drug events (ADEs) m VISTA and m VA Adverse Drug Events Reporting System (V ADERS). Promotes and monitors compliance for established drug therapy policies. Work with providers to ensure compliance with national, VISN, and local initiatives. Work Schedule: Third Shift Sunday 11:00pm to Monday 7:00am, Monday 11:00pm to Tuesday 7:00am, Tuesday 11:00pm to Wednesday 7:00am, Wednesday 11:00pm to Thursday 7:00am and Thursday 11:00pm to Friday 7:00am. Qualifications

Basic Requirements: Citizenship: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) AND/OR a Doctor of Pharmacy (Pharm.D.) degree. Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Candidates at the GS-12 grade level must hold a full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. NOTE: The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. NOTE: Individuals who have or have had multiple licenses and had any such license revoked for professional misconduct, professional incompetence or substandard care, or who surrendered such license after receiving written notice of potential termination of such license by the State for professional misconduct, professional incompetence, or substandard care, are not eligible for appointment to the position unless such revoked or surrendered license is fully restored (38 U.S.C. § 7402(f)). Effective November 30, 1999, this is a requirement for employment. This requirement does not apply to licensed pharmacists on VA rolls as of that date, provided they maintain continuous appointment and are not disqualified for employment by any subsequent revocations or voluntary surrenders of State license, registration or certification. Language Proficiency: Candidates must be proficient in spoken and written English to work in the advertised location. Candidates must be proficient in spoken and written Spanish to work in the advertised location. Grade Determinations: In addition to the basic requirements above, you must also meet grade requirements for a specific grade level: GS-12 Level, Clinical Pharmacy: Must possess one year of experience equivalent to the GS-11 grade level. ~OR Completion of an ACPE-accredited Pharm.D. program. In addition to the Grade Determinations above, the candidate must demonstrate the following KSAs: Knowledge of professional pharmacy practice. Ability to communicate orally and in writing to both patients and health care staff. Knowledge of laws, regulations, and accreditation standards related to the distribution and control of scheduled and non-scheduled drugs and pharmacy security. Skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Physical Requirements: The physical demands of the work includes: Moderate lifting, 15-44 pounds; light lifting, under 15 pounds; moderate carrying, 15-44 pounds; pushing up to 4 hours; reaching above shoulder; use of fingers, good dexterity (both hands required); walking, up to 4 hours; standing, 6 hours prolonged; kneeling; repeated bending; climbing, use of legs and arms; operation of vehicle; ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in one eye to 20/20; specific visual requirement, ability to read fine print on labels; depth perception; ability to distinguish basic colors; ability to distinguish shades of colors; hearing (aid permitted); clear speech; and emotional stability.
