The Cincinnati VA Medical Center is recruiting for a full time Professional Licensed Clinical Pharmacy Provider (CPP) in the Emergency Department (ED). The CPP functions at the highest level of clinical practice and works independently under a Scope of Practice to directly care for patients. The employee is the expert in all areas related to drug therapy and functions as a consultant to other health care professionals.
Location: This position is located at the Cincinnati VAMC.
Duties
The Cincinnati VA Medical Center is a two-division campus located in Cincinnati, Ohio and Fort Thomas, Kentucky serving 15 counties in Southwest Ohio, Northern Kentucky, and Southeast Indiana with 6 Community Based Outpatient Clinics located in Bellevue, KY; Florence, KY; Lawrenceburg, IN; Hamilton, OH; Clermont County, OH, and Georgetown, OH. Cincinnati VAMC also supports two off main campus locations in the Cincinnati area via an Eye Center and Mental Health Outreach Community Division.
Duties:
Interviews and evaluates patients upon admission to the ED and before admission to the hospital for appropriateness of pharmacologic therapy and medication reconciliation. Makes a pharmaceutical care plan for those patients who need pharmaceutical care and will document the plan in the pharmacy initial assessment or progress note.
Evaluates drug therapy through direct patient care involvement, with clinical assessment and objective findings relating to patients' responses to drug therapy and communicating and documenting those findings and recommendations to appropriate individuals and in appropriate records (i.e., patient's medical record).
Assists in the development and documentation of therapeutic plans as a member of the treatment team, utilizing effective, safe, and economical medication treatments.
With approved clinical privileges under a Scope of Practice, and under the appropriate supervision of the attending physician, assumes limited authority to direct drug therapy and order, perform, review and/or analyze appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy as a member of the treatment team.
Assists in the management and reporting of adverse drug reactions, and acute and chronic disease states.
Administers medications according to pre-established protocol when requested by the provider.
Identifies and takes specific corrective action for drug-induced problems as a member of the treatment team.
Orders consults (i.e., dietician, social work) as appropriate to maximize positive drug therapy outcomes.
Serves as a liaison between Pharmacy Service and the Emergency Department.
Assists in discharge planning and ensures continuity of care for patients leaving the ED.
Works with patients who have been identified as having compliance/adherence issues with their medications.
Administrative:
As assigned, attends, participates, and contributes as an active member on various Medical Center or VISN committees: Critical Care Committee, Pharmacy and Therapeutics Committee, works groups (such as Root Cause Analysis), task forces, etc.
Participates in various Continuous Quality Improvement (CQI) initiatives, and medication usage evaluations as assigned.
Documents and reports clinical pharmacy interventions, patient outcomes, and cost-effective initiatives.
Ensures optimum utilization of all available resources (personnel, fiscal, equipment, space, etc.)
Serves as the Acting Associate Chief for Inpatient Pharmacy as needed.
May serve as the back-up pharmacist provider for all acute care sections.
Education/Research:
Develops and maintains effective pharmacy education programs, including the ASHP accredited Residency Program and pharmacy student rotations. Participates in interdisciplinary training programs.
Participates in the development and implementation of formal and informal drug education/clinical consultation to medical, surgical, nursing, and pharmacy staff in the form of in-services and continuing education programs.
Acts as clinical preceptor for pharmacy students.
Serves as preceptor for residents in the ASHP accredited Pharmacy Residency program.
Interfaces with other health-care providers and administrators to ensure compliance with established pharmacy and medical center policies and procedures.
Participates in self-development activities to maintain and improve professional competency.
Designs and/or participates in research studies or outcomes research involving drug therapy or clinical pharmacy functions.
Provides oversight to the clinical pharmacy technicians conducting medication reconciliation.
Dispensing and Distribution:
Evaluates and processes providers' orders for patients, clarifying unclear orders with the provider when necessary.
Provides pharmacist staffing assistance in the Inpatient, Pharmacy as scheduled by supervisor.
Provides quality control analysis for dispensing activities of automated dispensing equipment or pharmacy technicians.
Work Schedule: Compressed schedule, including every other weekend and rotating holidays. Hiring Manager will discuss specifics during the interview.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d).
Education:
(1) Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.)(2) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT).
Licensure. Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-13 Clinical Pharmacy Specialist
Experience: In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level GS-11.Pharmacists assigned to this position must demonstrate the following KSAs:
a. Ability to communicate orally and in writing to persuade and influence clinical and management decisions.
b. Expert understanding of regulatory and quality standards for their program area.
c. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise.
d. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy.
e. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters.

Physical Requirements: Work requires moderate lifting, walking, bending, stooping, standing, pushing, and pulling.
