This Social Services Assistant position is in(Whole Health) for Connected Care services at the VA Maine Healthcare Systems, located at the Togus, Maine campus. This position is full-time working 40 hour weeks.

Duties

The Social Services Assistant (Whole Health Partner) connects with Veterans and supports their journey through the Whole Health System. Connects Veterans to the best options to support their health and well-being. The position has knowledge of internal & external resources; performs a wide-range of tasks to assist Veterans and peers of all ages. This position functions as an interdisciplinary team member. Major duties include but are not limited to the following: Provides information and advocates for Veterans' personal health goals and plan, and shared goals with clinical care team by partnering with Veterans in completing a Personal Health Inventory and exploring aspects of a Personal Health Plan in group and individual settings. Functions as the steward of how to navigate the whole health system and orient Veterans new to the program about the services, hours, locations, staff, tools, resources, and other pertinent information necessary for the Veteran to understand the program and how to utilize the program. Acts a steward of the community resources available around Whole Health approaches and works to connect Veterans with these programs. Assists the Veterans to explore core elements of personal health planning, exploration of what matters to the Veteran, self-assessment using the components of health and well-being, identification of goals and potential actions in self-care areas, and identification of support and follow-up to implement a plan. Assists Veterans in this exploration through one-to-one or group sessions assisting Veterans in identifying their skills, strengths, supports, and the resources needed to aid them in achieving their health aspirations. Utilizing strong relational and facilitation skills, acts as a Veteran' point of contact to navigate through the Whole Health program (pathway, health and well-being programs, clinical care), assists the veteran in exploring options for participating in individual or group programs to explore the following core elements: Exploration of What Matters; Self-Assessment using Components of Health and Well-being/COH; Set goals/actions; identify support/follow-up. Facilitates group programs aimed at exploring the components of health and well-being, informs Veterans about community and mutual supports and how to use these to support health and well-being goals. Community resources may include, but are not limited to, consumer-run self-help and mutual support services, social security office, Department of Family and Children Services, local YMCA, library, Veterans' service organizations, etc. Performs other duties as assigned. Work Schedule: Monday - Friday, 7a.m. to 4:00p.m. Telework: VA supports the use of telework as a way to help attract and retain talented individuals in public service, increase worker productivity, and better prepare the agency to operate during emergencies. This position may be authorized for telework. Telework eligibility will be discussed during the interview process. Virtual: This is not a virtual position. This position is in the Competitive Service. This position is covered by locality-based comparability pay. Travel and relocation expenses are not authorized. Current and former Federal employees must submit copies of their most recent SF-50, (Notice of Personnel Action). The SF-50 must identify the position title, series, grade, step, tenure and type of service (Competitive or Excepted). In some cases, more than one SF-50 may be required to show a higher grade previously held. Interchange Agreements with Other Merit System: Employees who occupy medical or medical-related positions and were appointed under 38 U.S.C. 4701(1) or (3) [formerly 38 U.S.C. 4104 (1) and (3)] must be serving in a full-time position without time limit and have served continuously for at least one year in this other merit system to be eligible to convert to the Title 5 System. Some exceptions to this rule may apply if you have previously acquired tenure under a Title 5 appointment. This position is authorized for telework but has not been authorized as virtual or remote (i.e., the must be able to work at the location the position is being announced for). Qualifications

To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 12/19/2023. Time-In-Grade Requirement: Applicants who are current Federal employees and have held a GS grade any time in the past 52 weeks must also meet time-in-grade requirements by the closing date of this announcement. For a GS-06 position you must have served 52 weeks at the GS-05. The grade may have been in any occupation, but must have been held in the Federal service. An SF-50 that shows your time-in-grade eligibility must be submitted with your application materials. If the most recent SF-50 has an effective date within the past year, it may not clearly demonstrate you possess one-year time-in-grade, as required by the announcement. In this instance, you must provide an additional SF-50 that clearly demonstrates one-year time-in-grade. Note: Time-In-Grade requirements also apply to former Federal employees applying for reinstatement as well as current employees applying for Veterans Employment Opportunities Act of 1998 (VEOA) appointment. You may qualify based on your experience as described below: Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS-5 in the normal line of progression for the occupation in the organization. The Social Services Assistant has individual occupational requirement experience examples may include, but are not limited to: Group leader in community or residential recreation or training programs. Counselor in a youth summer camp. Instructor in a training program for the development of job skills. Work that demonstrated the ability to be responsible for and to lead and train members of a job crew. Group activities leader in the Armed Services. Unit leader-monitor-proctor for a dormitory group in a boarding school or other residential facility. Aid or assistant to professional or specialist staff in such fields as personnel (private or government), employment guidance in a vocational or academic institution, social service, or manpower development. Aid or assistant to professional or associate staff engaged in work of a social services nature in private, public welfare, government program, or community organizations, or in voluntary activities. Aid or assistant to professional or specialist staff working with individuals who are addicted to or abuse alcohol and/or drugs. OR, Education: Applicants may substitute education for the required experience. To qualify based on education for this grade level you must have successfully completed at least one-half year of graduate level education (9 semester hours above a Bachelor's degree) or an internship in course matter that must include courses directly related to the work of the Social Services Assistant position. This education must have been obtained in an accredited college or university. NOTE: Transcripts (unofficial or official) must be submitted with your application materials. Education cannot be credited without documentation. OR, Combination: Equivalent combinations of successfully completed graduate level education and specialized experience, as described above, which meet the total experience requirements for this grade level. The education portion must include graduate courses directly related to the work of the Social Services Assistant position. Physical Requirements: The work is primarily sedentary. Typically, the employee sits to do the work. There may be some walking, standing, bending, carrying of light items (such as books, papers), and accessing transportation. The work may require occasional use of Department of Veteran's Affairs approved behavioral management techniques with Veterans who present a danger to self or others.
