This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Collaborating with the interdisciplinary team of providers in: (1) Assuming primary medical responsibility for assigned G&EC inpatients. (2) Identifying the patients' medical problems. (3) Defining the medical management of these problems. (4) Determining the need for consultation from medical, surgical and/or psychiatric subspecialty clinics (5) Determining the need for, and facilitating, admissions to or discharges from the hospital in collaboration with other providers. b. Serving as collaborator for advanced practice nurses and physician assistants. c. Entering a complete H&P physical note in all long term care veterans, a monthly physician assessment note on all long tenn care residents. do a weekly progress note on all veterans admitted for skilled nursing or short term rehab and do discharge summary within 24 hours of discharge. All admission H&P's need to be completed within 24 hours. d. Actively coordinating with other healthcare providers to ensure Palliative Care consults are entered, completed and answered in a timely manner for inpatients or patients being admitted to the hospital. e. Ensuring proper hand off when transferring patients from one ward to another, by talking to receiving provider prior to transfer. f. Ensuring that the transfer note is completed on the same day. g. Ensuring proper hand off of all transfers to outside facilities {VA or Non VA) by having personal contact with accepting provider (i.e. talking to the accepting provider and documenting such contact in the medical record). Administrative Responsibilities The O&EC Physician is responsible for: a. Collaborating with the entire O&EC team of providers and non-providers by: (1) Providing leadership in the G&EC Service Line aimed at achieving Geriatric &. Extended Care program goals and objectives and the mission and vision of the Carl Vinson VA Medical Center. (2) Assisting in the planning and cooperating in the educational and clinical experience of medical students, residents and fellows assigned to the G&EC Service Line. (3) Assuming a leadership role in the development and implementation of the G&EC perfonnance improvement plan. (4) Advocating for O&EC with VHA leadership and the medical community. b. Providing clinical input and oversight for all patient treatment plans. c. Being readily available to the other team members for collaboration when medical or other problems arise. d. Being able to work effectively in an interdisciplinary team of VA providers. e. Participating in G&EC monthly staff' meetings at the Carl Vinson VA Medical Center in Dublin, as required. f. Sharing with other G&EC team members any medical care advances and practice standards. g. Collaborating with other Medical Directors, VISN leadership. and VA Central Office staff on program development issues. h. Completing, signing and releasing notes promptly and accurately using the Computerized Patient Record System (CPRS). All admission H&P and discharge summaries are to be done within 24 hrs. i. Entering all leave requests ( except for unexpected sick leave) in VISTA and seeking approval &om supervisor or approving official prior to going on leave. j. Participating in quality management activities such as medical records reviews, Drug Utilization Evaluations, Root Cause Analysis (RCA) and Peer Reviews at the Carl Vinson VA Medical Center in Dublin. as required. Work Schedule: Monday-Friday 8:00 am-4:30 pm

Qualifications

To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. The G&EC Physician position is aligned in the Geriatrics and Extended care (G&EC) Service Line, based at Carl Vinson V AMC in Dublin, GA. The selected physician works under the supervision of the G&EC Service Line Manager. The physician attends team meetings as required. The physician refills/renews medications, takes care of inter current medical problems of inpatients, by conducting daily rounds of wards, attending inpatients and ordering medications. The Geriatrics & Extended Care (G&EC) Service Line provides healthcare services in a cost effective institutionalized setting for patients who may be eligible for disease management and coordination of care G&EC targets frail, chronically ill veterans who require care by interdisciplinary health care teams, continuity, coordination of care and the integration of diverse services to cover complex medical, social, rehabilitative and behavioral care needs. These veterans need comprehensive healthcare services on an inpatient setting, as they age, to maximize function, reduce institutionalization and maintain their quality of life. The Geriatrics & Extended Care (G&EC) Physician is responsible for the overall medical care delivered to inpatients at the Cart Vinson V AMC facility. Medical care includes identifying the patients medical problems, diagnosing and treating appropriately, determining the need for consultation from subspecialty care for specialized medical, surgical or psychiatric services and also determining the need for services that may require transferring patients to toher VA or Non VA healthcare facilities, community nursing homes or discharge when appropriate. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: Placement in this position is subject to meeting the physical requirements of this position. Pre-placement medical examination is required. See VA Directive and Handbook 5019.

Education

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
  1. Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
  2. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
  3. For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. § 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
