This pharmacist serves a broad and expansive role throughout the entire SLC VA Health Care System in the coordination and processing of Prior Authorization Drug Requests (PADRs) and formulary management with emphasis on providing the most clinically appropriate, safe and cost-effective treatment to Veterans. Performs the duties of a clinical pharmacist in all phases of assigned areas including outpatient and any assigned clinical activities.

VA Careers - Pharmacy: https://youtube.com/embed/Fn_ickNBEws The Program Manager will work collaboratively with the local Pharmacy and Therapeutics Committee, Pharmacoeconomics staff, and ADPAC as well as establish relationships with professional staff at SLCVAMC to continually review and implement the safe, cost-effective use of medications. This program manager will have knowledge of drugs and drug products, appropriate dosages, routes of administration, physical and chemical properties and incompatibilities. Oversees the local implementation of the VA National Formulary and compliance to VHA Directives, Medical Center Policy, and other regulatory agencies while ensuring timely and uniform access to medications including promoting safety, clinical efficacy, and cost-effective utilization of medications. Coordinates the implementation of formulary management, drug protocols, decision analysis tools and outcome instruments. Designs and develops tools and mechanisms to document and quantify the effects of formulary management changes on clinical outcomes. Updates the local drug file to reflect the updates and changes made by VA national PBM as well as VISN and local Pharmacy and Therapeutics (P&T) decisions as appropriate. Coordinates the communication and dissemination of drug and formulary related updates/alerts to all relevant stakeholders in the facility to facilitate safe, efficient, and standardized drug distribution. Conducts initial training to new staff and ongoing training to current staff regarding the PADR process and all related standard operation procedures. Completes PADRs and provides targeted evidence based clinical education on VA PBM drug monographs, criteria for use, practice guidelines or other high priority targets. Manages and resolves all higher-level review prior authorization/non-formulary drug requests which includes (but not limited to) appeals, new medication and supply requests, high-cost medications, and specialty ordering medications. Develops routine reports on the prior authorization drug request and non-formulary drug request process to ensure quality of review, equity of workload, compliance to nationally mandated metrics and goals, as well as continuous quality improvement. Assists CITC Program Manager and VISN staff with PADR requests from the community. Creates procedures and SOPs as applicable for each REMS drug as well as facilitate routine auditing to ensure compliance to the FDA regulations. Collaborates with procurement, pharmacy staff, and providers to ensure compliance to any specialty drug related regulations, accuracy of documentation, and reconciling of prescriptions in the VA electronic heath record. Maintains professional competency by keeping up with information on new drugs, new drug therapies, innovations in pharmacy practice, and integrating this information into daily practice. Plans, organizes, and coordinates work; solve problems, interpret the extent to which protocols may be modified or adapted, interpret policies and regulations, and initiate pilot work projects to determine new methodology. Assists in the pharmacy specific strategic plan and related outcome measurements to meet patient care goals particularly as it relates to clinical outcomes, adverse drug events, and medication errors. Assists in quality improvement and system redesign activities for Pharmacy service and the Medical Center. Identifies opportunities to reduce costs, eliminate waste, and assure resources are used in the most cost-effective manner. Promotes service philosophy of multi-disciplinary teamwork, cooperation, and customer service, to make patient-centered decisions and monitor the effectiveness of services provided. Inspires and empowers pharmacy employees and pharmacy residents to take initiative needed to achieve patient centered medication goals and to accept individual responsibility for outcomes. Encourages staff participation in developing goals and objectives and establishing policy, procedures, and practice standards needed to assure consistent quality care. Fosters a workplace culture that promotes lifelong learning for pharmacy residents, trainees, pharmacists and pharmacy technicians, that they may become effective members of patient-centered health care teams that strive to continuously improve patient care, safety and health. Provides a framework for continual learning for pharmacy trainees, residents and employees of all levels in order to sustain competence and improve performance within the healthcare system. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Pharmacists must be proficient in spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Verification of approved degree programs may be obtained from the Accreditation Council for Pharmacy Education, 20 North Clark Street, Suite 2500, Chicago, Illinois 60602-5109; phone: (312) 664-3575, or through their Web site at: http://www.acpe-accredit.org/. (NOTE: Prior to 2005 ACPE accredited both baccalaureate and Doctor of Pharmacy terminal degree program. Today the sole degree is Doctor of Pharmacy.) Graduates of foreign pharmacy degree programs meet the educational requirement if the graduate is able to provide proof of achieving the Foreign Pharmacy Graduate Examination Commission (FPGEC) Certification, which includes passing the Foreign Pharmacy Graduate Equivalency Examination (FPGEE) and the Test of English as a Foreign Language Internet-Based Test (TOEFL iBT). Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. A pharmacist who has, or has ever had, any license(s) revoked, suspended, denied, restricted, limited, or issued/placed in a probationary status may be appointed only in accordance with the provisions in VA Handbook 5005, Part II, Chapter 3, section B, paragraph 16. Grade Determinations: GS-13 Facility Program Manager Experience. In addition to the GS-12 requirements, must have 1 year of experience equivalent to the next lower grade level GS-12. Duties may include: manages all aspects of a distinct program to include policy development, quality resources and specialty functions unique to the program to optimize cost effectiveness (budget) and patient outcomes. Coordinates a single program area of a complex pharmacy operation, or multiple program areas within a facility to develop, organize, manage, and control complex pharmacy programs. Develops, organizes, coordinates and manages single or multiple program areas within the medical facility; this may include a single program area at multiple facilities. The Program Manager title is reserved for those individuals who have one or more of the following: supervisory responsibilities for various sections within Pharmacy Service; a complex program requiring coordination of multiple locations such as medical center care facilities, Community Based Outpatient Clinics (CBOCs), rural health, telemedicine, etc; specialized areas of a complex nature such as nuclear pharmacy, quality assurance, pharmacy informatics, clinical applications coordinator, Home Based Primary Care, pharmacoeconomist or liaison pharmacist duties requiring the coordination of processes with other medical facilities or VISN AND Knowledge, Skills and Abilities: Pharmacists assigned to this position must demonstrate the following KSAs: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. Expert understanding of regulatory and quality standards for their program area. Ability to solve problems, coordinate and organize responsibilities to maximize outcomes in their program area or area of clinical expertise. Expert knowledge of a specialized area of clinical pharmacy practice or specialty area of pharmacy. Advanced skill in monitoring and assessing the outcome of drug therapies, including physical assessment and interpretation of laboratory and other diagnostic parameters. Preferred Experience: Experience reviewing prior authorization drug reviews and reviewing clinical evidence for medication treatment. Ability to communicate drug therapy recommendations verbally and in writing. Skill in managing drug shortages including communication alternative treatments with patients and prescribers. References: VA HANDBOOK 5005/55 PART II APPENDIX G15 Physical Requirements: The work involves long periods of moving, standing and pushing carts about the work unit. The work requires regular and recurring bending, lifting, stooping, stretching, lifting. The incumbent must be a mature, sensible individual capable of working effectively in stressful situations, demonstrating cheerfulness, compassion, courtesy and concern for staff and patients. Must possesses math skills in order to convert medications to the correct dosing.
