Duties

The incumbent is the Personal Care Services Coordinator is assigned to Social Work Service. The incumbent is responsible for the professional and administrative management of the Veteran Directed Care (VDC), Homemaker/Home Heal Aide (H/HHA), Home-Respite and Community Adult Day Health Care (CADHC) programs. Manages and controls the use of data related to all programs regarding day to day functioning and program evaluation Plans, organizes, and directs administrative and oversight activities Planning, establishing, and implementing policies and procedures Developing program goals and objectives Provides administrative, technical, budgetary monitoring and oversight Performs work related to the review, monitoring, analysis, planning and assessment of each program Responsible for key project activities Knowledge of Medicare/Medicaid standards for service delivery of Home Health Aide services Implementing and maintaining a tracking system to ensure appropriate consults Responsible for reporting quality concerns and/or developing and/or disseminating proper procedures to report Work Schedule: Monday - Friday; 7:30am to 4:00pm Telework: Available Virtual: This is not a virtual position. Basic Requirements: Education: Must have a Master's degree in Social Work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. (TRANSCRIPTS REQUIRED) Licensure: Must be licensed or certified by a state at the advanced practice level at the GS-12 grade level. This includes an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (PROVIDE COPY) IN ADDITION TO THE BASIC REQUIREMENTS YOU MUST MEET THE FOLLOWING GS-12 GRADE REQUIREMENTS: EXPERIENCE: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure AND Applicants must also meet the Demonstrated Knowledge, Skills, and Abilities listed below: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice.
