Duties
The Physical Therapy Pelvic Floor Clinical Specialist serves as a consultant, clinician, and subject matter expert for the treatment of patients in both Pelvic Floor Outpatient treatment and Inpatient Physical Therapy at the Robert J. Dole VA Medical.
The special knowledge in Pelvic Floor therapy has been acquired through a minimum of 2-years of clinical experience and specialty training in this area.
Clinical specialist abilities and skills are necessary for treating both male and female patients having complicated medical conditions, including, but not limited to, urinary incontinence, fecal incontinence, dyspareunia, vaginismus, vulvodynia, pelvic organ prolapse, interstitial cystitis, painful bladder syndrome, prostatitis, defecation disorders, management of IBS/IBD, Levator Ani Syndrome, coccygodynia, chronic pelvic pain, pudendal neuralgia, pelvic oncology rehab, breast cancer post treatment rehab, peripartum and post-partum rehab.
Duties include but are not limited to:
Ability to use advanced knowledge and demonstrate expert clinical practice skills to provide assessment and treatment interventions across the continuum of care.
Utilizes advanced knowledge and expertise to modify treatment plans and to identify therapeutic activity interventions based on the changing needs, goals, and performance of patients.
Uses advanced training in specialized technology related to this area of practice. He/she establishes and maintains contact with other health care providers involved in patient care via written, telephone, and personal communication on a regular basis.
Exercises expert professional judgment to establish appropriate discharge planning recommendations to ensure safe discharge from inpatient and/or outpatient rehabilitation programs.
Involves the patient, family/significant other and medical team in treatment planning and goal setting through ongoing communication.
Plans and carries out the full range of complex interventions and treatment modalities across body systems and conditions where therapeutic objectives are complex and procedures involved require the application of advanced knowledge and skill in the area of outpatient primary care physical therapy in a direct access setting (e.g. exercise and wellness recommendations in context of Health Promotion Disease Prevention, psychologically informed approaches based on biopsychosocial models, complex pain interventions, advanced manual therapy techniques including spinal manipulation, dry needling, vestibular rehab interventions).
Adapts and applies treatment procedures with critical concern for patients who are acutely ill, or who have a combination of severe mental/emotional or physical disabilities. These skills and interventions are based on current physical therapy theory, research, and practice.
Demonstrates independent problem-solving skills that allow for frequent modification of treatment plans in response to subtle, but significant changes in the condition of the patient. The employment of innovative treatment techniques is used to obtain the best outcome. The coordination of multiple treatment approaches simultaneously is needed to accomplish routine to treatment objectives. Critical assessment of treatment outcomes is regularly performed.
Assesses for and requests prosthetic/orthotic equipment as needed and is able to make modifications as necessary.
Maintains a flexibility of schedule to allow for unscheduled patients, patient/team consultation and mentoring.
Maintains consistent and timely documentation of patient encounters in the electronic medical record in accordance with accepted PT practice, facility and Rehabilitation Service policies and procedures.
Participates in regular team meetings, verbal and written communication and consultation amongst team members in order to guide and evaluate the work of the multi-disciplinary outpatient programs. Collaboration with the team is necessary to develop a cohesive treatment plan for these varying patient populations.
Assists in the care of patients other than those of the specialized area of outpatient physical therapy as the needs of the department dictate.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education and Experience: The individual must meet at least one of the requirements below.
(1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist.
OR
(2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist.
OR
(3) Doctorate degree in physical therapy.
Licensure: Individuals must hold a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia.
English Language Proficiency: The PT must be proficient in spoken and written English in accordance with VA Handbook 5005.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS 13: You must have 1 year of specialized experience equivalent to the GS 12 Physical Therapist and one of the following:
1. Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years;
OR
2. Board Certification in the specialty area recognized by the ABPTS;
OR
3. Completion of a fellowship program;
OR
4. An additional advanced degree in a related field.
In addition, the candidate must demonstrate the following KSAs.
Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy.
Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy to include but are not limited to, geriatrics, neurology, cardiopulmonary, orthopedics, spinal cord injury, amputee, wound care, chronic pain, pelvic health, polytrauma, brain injury, lymphedema, vestibular, assistive technology, seating and mobility.
Ability to teach and mentor medical center staff in a specialized content area of physical therapy.
Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy.
Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area as outlined in 2 above.

The full performance level of this vacancy is GS 13.
Physical Requirements: Work includes regular and recurring physical exertion in standing, bending, kneeling, reaching, gripping and walking. Moderate to heavy lifting is required to assist patients and operate equipment. Sitting is required in meetings and for conducting administrative work. Above average agility may be desirable in defending oneself (or others) against physical attack/abuse. Work may require good physical health, normal use of arms and legs and good visual and auditory acuity.
