This is an LPN vacancy in the Domiciliary at the Butler VA Medical Center. The LPN at the Domiciliary is assigned to patients who require the use of highly specialized skills, judgment and observation. The incumbent cares for psychiatric, medical and substance abuse patients in the Domiciliary Care Program.
Duties
Duties include but not limited to the following:
Performs basic nursing procedures requiring technical skill and knowledge to meet the needs of the patient with little or no supervision by the RN.
Keeps well informed of, follows, and promotes all Medical Center policies, guidelines, and regulations.
Recognizes and accurately reports symptoms which indicate subtle physical and/or emotional changes in the patients' conditions promptly. Recognizes variations from normal and quickly takes appropriate action. Calls for professional assistance as needed.
Uses sound clinical judgment demonstrates the ability to vary the order and sequence of assignments based on patients' conditions and needs.
Utilizes knowledge of physical, social and emotional factors when planning patient care and uses this knowledge effectively in day-to-day interactions with patients and families.
Documents observations, treatments and patient care and responses to treatment on appropriate records with minimal monitoring by the RN in an accurate and timely manner.
As needed, prepares and administers medications correctly and understands the action and side effects of all medications administered.
Applies knowledge of MHRRTP and Service Line policies and procedures to his/her daily clinical functions and responsibilities
Applies knowledge of effective verbal interventions and preventative tactics when confronted with an aggressive and potentially assaultive patient.
Co-conducts groups to educate patients on medical aspects of their care while allowing them to share feelings, improve socialization skills and enhance their self-esteem.
Applies knowledge of human behavior and the various degrees of physical and mental impairment, is able to modify patient/family teaching methods.
Assists the professional nurse and allied health professionals in planning implementation of patient educational and recreational activities.
While under the direction of the Nurse Manager, PRRTP or Designee the LPN may be delegated or assume the role of team leader directing MHRRTP
personnel at the same or lesser grade in the completion of patient care assignments.
Acts as a mentor to LPNs of lesser grade, social service assistants and health technicians in developing and maintaining basic medical skills required of their positions.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Basic requirements as set forth in MPS Part1, Chapter 2, Change I Appendix M
dated August 15, 1990.
One (1) year of qualifying experience at the next lower grade or equivalent level.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
Qualifying experience includes nursing care work in a hospital, outpatient clinic, nursing home, or other supervised medical, nursing, or patient care facility that provided a practical knowledge of human body structure and sterile techniques and procedures, performing such duties as:
For GS-4:Successful completion of a program in practical nursing of a least 9 months' duration combined with 1 year of nursing assistant experience; or successful completion of a 2-year degree program in an accredited community college, junior college, or college or university in practical nursing. 6 months of nursing experience is required.
For GS-5 and above: 1 year of nursing experience equivalent to at least the next lower grade level is required.
