This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The Registered Nurse (RN) is assigned to Community Nursing Home Program (CNHP) at VA Puget Sound Health Care System with responsibility to provide direct services for Veterans, their families, and caregivers. The incumbent provides oversight and interventions to Veterans served in VA contracted nursing homes as indicated by Local and National policy and in accordance with the VHA Office of Integrated Veteran Care (IVC) Community Care Field Guidebook. Duties include but not limited The incumbent will conduct routine visits at Community Nursing Homes (CNH's) within the VA Puget Sound catchment area to assess Veteran satisfaction, Veteran safety, and quality of care. The incumbent may participate in initial and annual assessments of VA contracted CNH's to ensure that each site meets specific VA requirements to enhance safety and quality of care for Veterans. The incumbent will: assess and evaluate Veterans' ongoing nursing home placement needs; make recommendations for discharge planning and coordination of care; and ensure facility adherence to treatment plan. - The incumbent will provide case management to Veterans and families, as needed, throughout the continuum of care in CNH's. The incumbent will act as a liaison between VA and community clinical staff to facility coordination of care needs to enhance the delivery of co-managed care as needed. The incumbent will coordinate treatment and communicate within appropriate service lines to facilitate continuity of care for Veterans. The incumbent will complete clinical reminders within to include suicide risk screens in accordance with Local policy and National guidelines. The incumbent will systematically evaluate current nursing practice provided in CNH's; will use a variety of cues to obtain data on processes within area of expertise; will propose revision(s) based on evaluation of outcomes. The incumbent will act as a consultant to other team/staff members. The incumbent will review, evaluate, and investigate patient safety related incidents, patient complaints, readmissions, and Veterans deaths to include acting as a Joint Patient Safety Reporting (JPSR) reporter and handler and communicating and documenting these events per programmatic guidelines. The incumbent may support and assist as a JPSR handler for other incidents within the Community Care Section of Social Work Service. The incumbent will report incidents and JPSR safety and severity trends to the CNH Oversight Committee on a quarterly basis. The incumbent will participate in process improvement initiatives, change management initiatives, and/or committee assignments, as deemed necessary by the CNH Program Coordinator. The incumbent will document oversight visits and nursing interventions and activities in patient's clinical record utilizing the electronic health record and ensure appropriate hand off when transferring patient care. The incumbent will provide education and support to staff at CNH's to ensure Veteran needs are met. The incumbent may, as appropriate, supervise the practice of nursing students, or a less experienced nurse. The incumbent participates actively in the emergency management and safety practices as per policy/standard operating procedures for staff practicing on and off-site from the facility. A substantial portion of the workload directed by this position may be carried out at one or more locations that are physically removed from the main unit, under conditions that make direct supervision difficult to administer. Incumbent must, therefore, be able to work autonomously. The incumbent is expected to perform a large part of their work in the community settings and must have an unrestricted state driver's license and be cleared upon hire to drive a government vehicle. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Bachelor of Science in Nursing (BSN). 2-3 years of Registered Nurse experience. Direct Care Nurse/Staff Nurse experience. Experience working in a telework or hybrid telework environment. Ability to work independently. Familiarity with the care provided in Skilled Nursing Facilities and the laws, regulations, and compliance guidelines related to this care. Experience working with community partners. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Physical Requirements: Light lifting (15- lbs), use of fingers, walking (6 hrs), standing (2 hrs), repeated bending (6 hrs), Operation of crane, truck, tractor, or motor vehicle, hearing (aid permitted). Environmental factors: Working with hands in water, working closely with others.
