The Community Nursing Home (CNH)/State Veteran Home (SVH) Coordinator is a Licensed Clinical Social Worker administratively responsible for managing the daily operations of the CNH program as well as direct patient care to those who reside in the contracted nursing home facilities. The CNH/SVH Coordinator manages and controls data regarding day to day functioning and program evaluation of the CNH and SVH programs, prepares reports and statistics for the facility, VISN and national use.
Duties
Social Work Program Coordinators have experience that demonstrates possession of advanced practice skills and judgment. Advanced practice social worker have clinical knowledge in the profession, provide consultation and guidance to colleagues, role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work license or certification.
In conjunction with the Non-Institutional Care Coordinator Supervisory Social Worker, the CNH Coordinator plans, organizes and directs administrative, operational, patient and personnel activities for the program. The incumbent responds to consults for placements, will coordinate and oversee placements, plan and coordinate resident visits and provide quality of care oversight of care and services for the program. The incumbent is responsible for assessing psychosocial needs, planning discharges and level of care or placement changes. The incumbent is customer service oriented, working to ensure a good partnership with the community, our Veterans, their families and VA. This position serves all Veterans who are enrolled and funded under the CNH program.
Duties Include, but are not limited to:
Responsible for the overall function of the CNH/SVH programs.
Is responsible for the recruitment, growth, quality management and quality oversight of new and existing facility partners/care locations to include the following:
the national program field guidebooks and handbooks, development and management of local policies or practice/participation guidelines, management and coordination of nursing homes, quarterly contractor office representative duties (COR), case management, development of policies and care processes, inspections and coordination with various interdisciplinary treatment teams throughout the VA Medical Center.
Establishes and maintains a relationship with the facility team, Veteran, and family members.
Receives and completes consults from interdisciplinary teams and other social workers on complex, difficult cases, using advanced practice skills and expertise.
Provides consultation to referral sources and is responsible for development and maintenance of professional standards of practice and educating others regarding such.
Interprets and explains VA treatment and benefit programs.
Serves as a resource of knowledge of other programs and services from agencies that may enhance the quality of life for Veterans and makes appropriate referrals to said agencies.
Documents independently in the medical records utilizing paper electronic format, including psychosocial assessments, diagnostics, treatment, progress notes, follow up and referrals.
Has extensive knowledge of medical center policies and procedures, VHA and VBA regulations including the national handbooks/field guidebooks for the programs (CNH/SVH).
Ensures the direct supervisor, interdisciplinary team, and facility leadership are kept informed.
May provide outreach and education on the programs and other VA services, which are designed to increase Veteran and general community knowledge of VA services.
Serves as a mentor to other social workers, peers, employees, health care providers and partners.
Provides training to caregivers of the facilities, students, residents, and interns from other disciplines regarding the benefits of a VA services and local community resources.
Can communicate effectively, both orally and in writing, and with people from varied backgrounds.
Must provide oversight and clinical supervision to Social Work Trainees and/or GS-9 Social Workers or Licensure (if State license allows and per SW qualification standards).
Will represent the VA in the community in a positive and professional manner.
The incumbent must have a valid state drivers license and pass a medical exam to drive government vehicles.

Work Schedule: Monday through Friday, 8:00 am to 4:30 pm

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Social Worker Program Coordinator candidates must have the following:
Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Program Coordinator social workers have experience that demonstrates possession of advanced practice skills and judgment. Social workers are experts in their specialized area of practice. They may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Training or additional certs must be submitted with application.
AND
Licensure/Certification: Social workers at this grade level must be licensed or certified by a state at the advanced practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Advanced License must be submitted with application.

Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty. Ability to drive a government car.
