The Cincinnati VA Medical Center is seeking a Staff Nurse in the Same Day Surgery (SDS)/ Post Anesthesia Care Unit (PACU) unit. The SDS/PACU RN is proficient at admitting and preparing the patient/veteran for surgery as well as caring for him/her post-operatively during Phase I and Phase II of their perioperative experience. The preoperative phase focuses on the physical, psychological, sociocultural, and spiritual preparation for the planned procedure/surgery.

STAFF NURSE SAME DAY SURGERY (SDS) AND POST ANESTHESIA CARE UNIT (PACU) :
The Staff Nurse in the Same Day Surgery (SDS)/ Post Anesthesia Care Unit (PACU) is a registered professional nurse who is accountable to the Nurse Manager (NM) and Assistant Nurse Manager (ANM). The SDS/PACU RN is proficient at admitting and preparing the patient/veteran for surgery as well as caring for him/her post-operatively during Phase I and Phase II of their perioperative experience. The nursing role in the preoperative phase focuses on the physical, psychological, sociocultural, and spiritual preparation for the planned procedure/surgery. During postoperative Phase I, the RN's priority is assessment and performing basic life sustaining needs which require constant vigilance until Phase II has been reached. During Phase II, the RN role is to prepare the patient for care in the home or an extended care environment.
The scope of SDS/PACU nurse includes cultural, developmental, and age-specific assessment, diagnosis, intervention, and evaluation of individuals presenting for a procedure/surgery. Those individuals (the veteran population) will have or had sedation/analgesia and or anesthesia for surgical, diagnostic, or therapeutic procedures. The SDS/PACU RN will utilize critical thinking skills and demonstrate strong effective patient care by following evidence-based practice. The SDS/PACU RN should adhere to ASPAN's Standard of Perianesthesia Nursing Practice and maintain personal accountability for professional competency. In collaboration with the NM or ANM, he/she might consider developing and implementing a personal education/career development plan that may include unit education or advancing their own career through additional formal education.
The SDS/PACU nurse provides Preoperative, Phase I and Phase II care to the veteran presenting for a procedure/surgery. Unit hours of operation are 0600-1900 Monday through Friday. Assignments are made on a daily basis according to staffing by the ANM, NM or the RN acting in a charge role for that day. The staff is divided into assignments for pre-op and post-op care. The pre-op RN may transition to caring for the post-operative patient once their pre-op patient assignment is complete. All SDS/PACU nurses participate in gathering data for performance improvement by making the 24-48 hour post op call back and asking questions surrounding the veteran's perceived care. The SDS/PACU nurses review the surgery schedule for the next day and make note of needs such as consents, 24-hour note, bloodwork etc. Collaboration and excellent communication between the SDS/PACU staff and the Bed Flow Coordinator is vital. This should also be said of the relationship between the staff and the unit/floor NM, ANM or charge nurse, recognizing that patient satisfaction is the goal.
The SDS/PACU nurse follows safety protocols designed to reduce/eliminate risk of injury to self and their patients. The Situation Background Assessment Recommendation (SBAR) technique is utilized by all perioperative staff as a way to ensure a safe patient "hand-off" between the different phases of the perioperative process. This SBAR is started by the RN in the preoperative phase. Patient documentation may be completed in the EHR or PICIS. All SDS/PACU staff is responsible for reading and understanding established hospital policies, SOP's, standards of care and standards of practice. The SDS/PACU nurse performs yearly competencies in addition to pursuing unit specific continuing education offerings. The SDS/PACU nurse verbalizes understanding of the process for advancement utilizing the Nursing Professional Standards Board (NPSB) process and their functional statement.
The surgical specialties include: Orthopedics, General Surgery, Vascular, ENT, Urology, Gynecology, Podiatry and Ophthalmology.

Basic Requirements:

English Language Proficiency: RNs appointed to direct patient care positions must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d).
Graduation from School of Nursing:
Graduate of a school of professional nursing approved by the appropriate State agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant.

4. License:
Condition of Employment. A registered nurse (RN) will have a current, full, active and unrestricted registration as a graduate professional nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the U.S. or in the District of Columbia. The appointing official may waive this registration if the RN is to serve in a country other than the U.S. and the RN has registration in that country (e.g., Philippines). The RN must maintain a current, full, active and unrestricted registration to continue employment with VA.
Nurse Practitioners and Clinical Nurse Specialists. On and after [March 17, 2009], registered nurses appointed or otherwise moving into these assignments must meet and maintain the following additional qualifications. This includes employees appointed before [March 17, 2009], who obtain such qualifications on or after [March 17, 2009.]
Nurse Practitioners. A nurse practitioner must be licensed or otherwise recognized as a nurse practitioner in a State, possess a master's degree from a program accredited by the NLNAC or CCNE, and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. (The certification must be in the specialty to which the individual is being appointed or selected.)
Grade Determinations:
The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I, Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I, Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I, Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree.
Physical Demands:
Per VA Directive and Handbook 5019 - a physical examination will be conducted based on the physical requirements of the position. A physical examination prior to placement is required. This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continues walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs.
BLS/ACLS Requirements:
Current certification in BLS is required for all staff providing direct patient care, and police. Additionally, current ACLS is required for patient care staff that work in specified or high-risk areas (SICU/ICU/CCU/MICU/PACU/ED/and all procedure suites using moderate sedation, etc.). If you are selected for a position which requires BLS/ACLS certification, you will be required to have proof of certification prior to being boarded for appointment with VA. If selected, obtain such certification to avoid any delays in the hiring process.
