The Battle Creek VA Medical Center Extended Care Service (ECS) Community Living Center (CLC) is looking for a Minimum Data Set (MDS) Staff Nurse for 84-1 Long Term/Respite Care/Rehabilitation Unit.
Duties
The Extended Care Service (ECS) Community Living Center (CLC) Minimum Data Set (MDS) Staff Nurse is a Professional Nurse with broad-based clinical knowledge enabling him/her to effectively implement health promotion, prevention, and management for adult or older adult residents requiring skilled Nursing interventions in a long-term care setting.
The RN provides care for residents with complex, mixed chronic medical and/or psychiatric conditions, residents with cognitive impairments, and residents who are terminally ill.
The Nurse utilizes the Nursing process when assessing, planning, implementing, evaluating, and documenting Nursing care provided. He/she works collaboratively with the interdisciplinary team when planning and providing care to Nursing Home residents and their families.
The Nurse functions independently as a charge Nurse on the neighborhood, organizing and delegating care activities to other RNs, LPNs, and NAs and providing feedback to the Nurse Manager regarding the practice of others to improve care. He/she may either direct the care of other Staff members or provide direct resident care depending on the needs of the neighborhood.
The Nurse participates in Quality Improvement activities in the CLC and/or Medical Center with the objective of improving resident care outcomes. He/she takes an active role in identifying his/her own educational needs and continually updating clinical skills.
The Nurse participates in ECS, Patient Care Services, and Medical Center Committees representing Nursing, the CLC, or ECS as assigned.
In addition to the staff RN role, MDS RN is responsible for completing the MDS and Medicare PPD Assessment Form (MPAF) assessments and Resident Assessment Protocols (RAPs) according to VHA Directives and Medical Center and Extended Care Service guidelines, including adherence to timeframes for assessment completion.
MDS RN assists with resident care plan creation and implementation including developing mechanisms to ensure that all staff is familiar with where resident care plans are located, contents of these care plans, and resident care goals.
RN will ensure that the data entered into the Caribou Software is accurate and is based on supporting documentation in Electronic Health Record flowsheets, the Caribou system, and/or staff member reports regarding the care needs of the unit's residents. He/she will assist in designing and implementing interrater reliability processes regarding MDS and MPAF assessment completion.
MDS RN will educate interdisciplinary personnel with regard to MDS and MPAF assessment completion. He/she will also educate nursing personnel with regard to the use of the Caribou documentation system and will develop or possess expertise with regard the Caribou system.
RN will actively participate in MDS, Restorative Nursing, and other quality and performance improvement processes and will ensure that these programs and process improvements are implemented on the assigned unit.
MDS RN admits residents to the CLC by completing the admission health assessment and is expected to serve as a CLC staff RN regularly to maintain clinical competencies in the area.

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by the appropriate State-accrediting agency and accredited by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience: Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.

Ability to effectively utilize a stethoscope and distinguish breath and heart, and bowel sounds; Emotional and mental stability - ability to effectively perform the essential functions of the position and respond appropriately in emergency situations.
