The Fargo VA Health Care System is recruiting for a full-time Data Transcribers for the Primary Care and Specialty Medicine Service Lines.

Duties

This position performs duties and responsibilities in various clinics in a medical treatment facility. The incumbent's duties are performed on a regular and recurring basis in clinical and specialty areas, out-patient care areas, and in-patient care areas. Each supported medical center location will consist of identifiable team members who include physicians or other licensed independent practitioners (LIPs), such as nurse practitioner (NP) or physician assistant (PA), nursing staff, and allied health staff. The incumbent is a multi-functional employee with full medical administrative capabilities and responsibilities. The incumbent will interface with applications on the personal computer (PC), including the Veterans Health Information and Technology Architecture (VistA) and the Computerized Patient Record System (CPRS) or Millennium hereafter collectively called the Electronic Health Record (EHR). Duties include, but are not limited to: The incumbent is responsible for accompanying the LIPs during each patient visit and documenting/transcribing information in the EHR that transpires during the LIP's visit at the direction of the LIP. The incumbent does not interfere with the doctor/patient interaction. The incumbent efficiently navigates, ensures regulatory compliance, reviews and gathers information from the health record. He/she accurately and thoroughly documents medical visits and procedures as they are being performed by the physician. The incumbent may also research information requested by the LIP to assist with coordinating referrals. He/she may also prepare operative reports, make phone calls, and conduct other clerical tasks as assigned. The incumbent enters all aspects of the patient encounter into the EHR including the patient history, physical exam findings verbalized by the LIP, procedures, orders, and clinic encounters. He/she facilitates the flow of patients through the clinic by queuing orders including referrals, tests, medications, encounters, and signing and forwarding the documentation to the LIP for review, approval, and signature. The position is fast paced and requires the individual to manage their time appropriately and prioritize workload to ensure work is completed in a timely fashion. During the patient encounter with the LIP, the incumbent documents all aspects of the office visit (handwritten, electronic, or otherwise) in the EHR, organizes information consisting of but not limited to, plan and patient instructions in the EHR, assists the LIP in navigating the EHR, and locates information for review (i.e. previous notes, reports, test results and laboratory results). The incumbent may also record diagnostic results such as radiology interpretations and lab values, enter medication orders, physical therapy orders, home exercises, educational handouts, work restrictions and other orders per LIP instruction. The incumbent must maintain all information in strictest confidentiality, work with LIP staff to ensure quality patient care and services are provided and ensure compliance with clinic standards of quality patient care. The incumbent ensures accuracy, spelling, punctuation and grammar in documentation prior to LIP review and approval. The incumbent is also responsible for the following medical records management duties: Ensures records are prepared for Treatment Team review by providing previously ordered tests if appropriate, adequate forms for documentation by team, recent information profile for LIP to review and appropriate encounter form attached to the medical record. Assembling medical information in correct sequential and chronological order. Ensures record movement control and accountability is completed through record tracking in the EHR. Ensures appropriate forms are completed, accurate and signatures obtained for requests/release of medical information. Initiates action to request records from other facilities, both VA and non-VA, and obtains all medical records as requested by Treatment Team. Assists Treatment Team in ensuring medical documentation complies with Joint Commission and other accreditation body requirements The incumbent participates in quality improvement activities in an ongoing basis and recommends changes to improve programs when indicated. He/she enters appointment management information into the EHR to ensure proper workload credit and statistical information is maintained for the medical center. The incumbent uses his/her familiarity and knowledge of qualitative and quantitative data collection methods to assess and improve upon the EHR. Physical Requirements: The work is sedentary and does not require any special physical effort. Employee may carry light items such as papers, books, and portable computer requirement. The work often requires long hours to meet project deadlines and to devise corrective action and solutions to unexpected technical and/or management crises resulting in sometimes highly stressful work situations.
