Duties

The LPN works with autonomy under the clinical supervision of a licensed Registered Nurse (RN), the LPN functions as a member of the multidisciplinary team assisting with managing and coordinating care for a panel of patients with a variety of medical conditions and specialty needs in the outpatient and inpatient settings in Community Care (CC). The LPN coordinates with Non-VA care providers to ensure timeliness of needed services, maintains effective and efficient communication with Veterans, Non-VA providers and staff, VA requesting providers and other services to assure seamless transition to and return from Non-VA care; utilizes several different programs for care coordination; participates in CC care team huddles and staff meetings to improve program effectiveness and efficiency; participates in development of priorities and deadlines, meets deadlines managing workflow in assigned areas according to established priorities; plans and sequences work to meet patient and provider needs; monitors pre-appointment requirements to assure readiness for Non-VA patient visit/procedure; enters appropriate information in the patient record, correctly schedules appointments when indicated. As a member of the CC department, the LPN serves as point of contact from VA to Non- VA network systems of providers. S/he will be responsible to facilitate the administrative needs of patients receiving care and treatment outside the facility for surgical and medical needs by obtaining and reviewing all pertinent clinical information from the community provider. The LPN works collaboratively to ensure seamless communication and transition of veteran care to allow development of treatment plans. S/he will perform at a competent level and work under the supervision of RN. Duties include but are not limited to: LPN will provide care coordination to Veterans in one or more of the following categories of care which may include but are not limited to: physical therapy, occupational therapy, kinesiotherapy, acupuncture, smoking clinic, MOVE clinic, massage therapy, chiropractic care, erectile dysfunction clinic, pulmonary function test, energy medicine healing touch clinic, palliative care, bariatric information session, bariatric surgery support group, pain education, PM&R Clinics: (PM&R Outpatient Craniosacral Therapy, PM&R Outpatient Occupational Therapy, PM&R Outpatient Physical Therapy/ABJ, PM&R Outpatient Physical Therapy/JB, PM&R Outpatient Shoulder Home Exercise Program, PM&R Outpatient Equipment OT Self-Care Aids/JB, PM&R Outpatient Equipment Power Wheelchair/Scooter Request, PM&R Outpatient Equipment PT Mobility Aids/JB), outpatient biofeedback, compensated work therapy, sexual dysfunction clinic, nutrition clinic. LPN will review medical records to ensure they meet the established criteria. LPNs will review consults to determine if consult meets established criteria. LPN will review request for service (RFS) forms to ensure it meets the established criteria, will enter a RFS note and consult placement, holding for signature LPN will confirm and monitor appointments, procedure requests, and other services as needed, documenting appropriately and works with staff to ensure all necessary testing is completed. LPN will assist team to ensure care is facilitated and delivered timely, appropriately, is cost effective and in accordance with established standards, hospital policies and procedures. Demonstrates the ability to review consult for relevancy, accuracy, and completeness prior to submitting to appropriate specialist. LPN is responsible to maintain performance measures as established by department Demonstrates ability to effectively follow-up on specific consult issues until fully dispositional. Ensures all consults are acted on and completed in a timely manner; using various systems such as but not limited to: JVL, REFDOC, VISTA, CPRS, VHIE, HSRM, Consult Toolbox and any new system introduced to enhance CC timeliness. Performs all required audits on consults as stipulated in audit tools Ability to keep current with changes from regulatory agencies regarding standards and conditions. Broadly acquainted with clinical treatment modalities. Presents a positive and professional image of the VA during contacts with Veterans, Non- VA providers (Vendors) and VA staff. Utilizes clinical judgement to determine urgency of the action to care needs/concerns to appropriate detailed information and triage to resource personnel for customers. Work Schedule: 8:00 am - 4:30 pm Monday- Friday Telework: This position regularly teleworks more than 6 days in a pay period. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education: Must be a graduate of practical or vocational nursing approved by the appropriate State accrediting agency and/or the National League for Nursing at the time the program was completed by the applicant. No education exceptions will be applied. License: Must have a full, current, active and unrestricted license as a graduate licensed practical or vocational nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States or District of Columbia. Language: Must be proficient in spoken and written English as required by 38 U.S.C. 7402(d) and 7407(d). May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-06: Completion of at least one-year of qualifying experience at the GS-05 level or equivalent, fully meeting all performance requirements for the GS-05 LPN/LVN. AND You must demonstrate the below knowledge, skills, and abilities. Be technically proficient in initiating, performing and completing assigned duties in providing care to variable patient populations. Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions; to use judgment in selecting the appropriate order and sequence of procedures and treatments; and to accurately recognize, report and record relevant patient information. Completed work should need only a general review by a registered nurse (RN) or physician (MD/DO) for appropriateness and conformity with established policies/procedures. Ability to observe, identify and respond to the patient's needs for care, including medication, equipment-assisted care and patient/family education. In organizing and delivering care, the LPN/LVN recognizes and considers emotional, cultural, spiritual, socio-economic, and age-related factors. Prepare and administer prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for physical and/or emotional changes in patient's condition from prescribed medications/treatments, promptly and accurately documenting noted changes, and reporting any deviations from normal to RN or MD/DO. Knowledge and ability to recognize urgent or emergent patient care situations, seek assistance of the RN and/or MD/DO, and initiate appropriate emergency interventions as directed. Knowledge and understanding of human behavior, patient motivations and reactions to situations, and ability to appropriately utilize this knowledge in working effectively with patients, family members, and other staff. Establish constructive relationships with individual patients and their families to elicit feelings and attitudes, and to promote positive relationships, communication and socialization skills. Fosters an environment of respect for individual patient and family rights to privacy and dignity in all aspects of care delivery. Effectively incorporates knowledge and understanding of established customer service standards in all interactions with patients, family members, and/or other internal/external customers. Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which include preparing the patient, assisting in the diagnostic examination, preparing and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure. Serves as a preceptor in orienting, educating, and training less experienced LPNs/LVNs or NAs/HTs related to support duties for these more complex, specialized tests/procedures. Actively seeks out educational opportunities to enhance nursing knowledge and skills, sharing new knowledge gained with other staff to improve and advance nursing practice. The actual grade at which an applicant may be selected for this vacancy is in GS-06. Physical Requirements: The physical demands described here are representative of those that must be met by an employee to successfully perform the essential functions of this job. The position requires visual acuity, keen hearing, clear distinctive speech, and ability to concisely communicate to a variety of individuals. In some instances, interactions with patients or clinical staff can become very intense, and the employee could be placed in a position of stress. This position requires manual dexterity to communicate efficiently via keyboarding skills. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing, and reaching over head. Daily computer work is required. The work is in an office setting and involves everyday risks or discomfort that require normal safety precautions. Employee must be able to view and read printed material as well as information on the computer screen. Occasionally, employee may be asked to lift and/or move 25 pounds' short distances.
