This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

Duties include but are not limited to: Establishes and maintains an ongoing education program for Veterans, caregivers, community agencies, students, and staff to facilitate understanding of clinical practice guidelines, mental health issues, readjustment issues and psychosocial issues facing caregivers. Develops and facilitates CSP-program focused direction, orientation, coaching, in-service training, staff development, and continuing education programs for VA facility staff and the community. Develops and implements a plan for outreach to Veterans and Veteran caregivers who may benefit from the CSP program services. This includes outreach to current PGCSS and PCAFC participants to inform of available services. Outreach requirements include but are not limited to event planning for National Family Caregiver Month. Maintains a current network of referral resources to include peer support mentoring groups, caregiver skill building and self-care educational opportunities, mental health support, legal and financial services. Directly supervises the VA HCS Caregiver Support Coordinator(s) (CSC) and, when applicable, Program Support Assistant (PSA) and other interprofessional staff assigned to Caregiver Support Program as outlined in VHA Directive 11152(1), Caregiver Support Program. Coordinates program implementation with multiple clinical services across all campus locations within the VA facility, including but not limited to nursing; physical, occupational, and speech therapists; mental health and primary care providers. Provide clinical interventions to the Veteran and caregiver populations served by the programs. Subject matter expert (SME) to VA Facility leadership and staff for all CSP related issues and serves as a consultant to other VA professionals and administrators on program specific activities. The incumbent serves as a liaison to Veterans Integrated Service Network Program (VISN) CSP Program and the VA Central Office (VACO) Program. Serve as the VA HCS POC for all inquiries and outreach requests regarding eligible PCAFC and PGCSS caregivers. This includes a wide variety of stakeholders. Identifies unmet needs of the caregiver population, their families/significant others and caregivers. The incumbent works collaboratively with facility, VISN, and VA Central Office staff to develop and implement solutions-participating in national, regional, and VISN operational calls; contributing to national leadership council committees; and implementing cultural transformation initiatives. Perform the administrative and personnel management function relative to staff supervised including personnel actions, clinical supervision, consultation, negotiation with other departments, and quality improvement. Works collaboratively with internal staff, VISN and VACO leadership, and external stakeholders to expedite service delivery. Provides consultation and briefings to other staff and internal/external stakeholders on the special needs of caregivers. Responsible for monitoring performance measures at the local, VISN, and Central Office levels; providing responses to congressional inquiries; and briefings to providers, VA leadership, community organizations and other internal and external stakeholders. Develop policies and procedures for program operation, and prepares reports and statistics for facility, VISN, and national use. Manage CSP clinical and business operations, accountability for program effectiveness, and modification of service patterns to comply with the delivery of standardized processes found within Directive 1152(1). Support customer satisfaction, well-being and service delivery. Develop a data-driven, continuous quality improvement program with established goals and outcomes to evaluate and document the program's effectiveness. Certifies staff completion of all required annual and ad hoc trainings through documentation and reporting. Organize and coordinates administrative, operational, clinical and personnel activities for the CSP program. Responsible for all aspects of program staffing, work assignments, budget and treatment services. Responsible for managing and reporting on program specific fund control points, labor mapping, workload, and clinic utilization within the assigned program area. The incumbent manages and controls the use of data related to these programs regarding day-to-day functioning and program evaluation. Provides oversight of facility CSP program fiscal matters of the functions supervised (which would include fund controls, contracts, and equipment expenditures), forecasts resource and equipment needs, and administers the allocated budget. Qualifications

Grade Requirements. This section is only to outline the grade determinations used for initial grade placement upon selection and will not be used for minimum qualifications. These grade determinations can be reviewed in VA Handbook 5005, Chapter II, Appendix G39, Social Worker Qualification Standards. Social Worker (Program Manager) Experience and Education. One year of experience equivalent to the GS- 13 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Individual may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all the following KSAs: Licensure/Certification. Individuals assigned as social worker program manager must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Skill in large scale program coordination and administration across multiple sites, which includes consultation, negotiation, and monitoring. Ability to translate management goals and objectives into well-coordinated and controlled work operations. Ability to analyze organizational and operational problems and to develop and implement solutions that result in sound operation of the program. Ability to collaborate with internal and external partners to further program goals and enhance patient centered care. Ability to provide the full range of supervisory duties which would include responsibility for assignment of work to be performed; performance evaluations; selection of staff, training, and recommendation of awards, advancements, and when appropriate, disciplinary action. Ability to oversee the fiscal matters of the functions supervised (including fund controls, contracts, and equipment expenditures), forecast resource and equipment needs, and administer the allocated budget. Skill in developing policies, procedures, and/or practice guidelines for the program and in participating in national program development. Ability to collaborate with internal VHA partners, locally and on a national level, as well as external community partners to further program goals and enhance patient centered care.
