The Diabetes Self-Management Education and Support (DSMES) Advanced Practice Registered Dietitian has overall program responsibility, plans, directs, integrates and evaluates clinical elements and results for the DSMES program at the Albany Stratton VA Medical Center and Community Based Outpatient Clinics. Supports the facility's mission, including patient care, education and organization policy.
Duties
Program Planning and Management:
Serves as liaison or representative to national VHA and VISN 2 committees as expert in diabetes, clinical outcomes.
Practices at an advanced level of practice, and provides medical nutrition therapy and DSMES which included insulin pump and continuous glucose monitor (CGM) training and management as reflected as such in their approved scope of practice for facility.
Participates in development of policy regarding standards of care for diabetes patient treated in VHA and VISN 2.
Supports and/or participates in clinical research studies performed in the medical center.
Acts as a clinical reviewer for VHA, VISN and other diabetes or related patient educational materials or professional publications.
Management/Administrative:
Coordinates activities of registered dieticians and other staff, students or trainees working with DEMES Program, its research and related projects.
Provides clinical and technical advice, support and assistance to dieticians, other professionals, students and customers in the Medical Center and Network.
Incorporates and updates standards of care according to: VHA/DOD Diabetes Clinical Practice Guidelines, the Joint Commission , the America Diabetes Association, the Academy of Nutrition and Dietetics, EPRP, and other relevant diabetes management and education initiatives.
Patient Care, Patient and Staff Education:
Implements and coordinates a continuing staff development program for all personnel involved in the DSMES Program.
Ensures that all relevant members of DSME Program meet mandatory and continuing educational requirements to assure maintenance of registration and certification status.
Mentors DSMES Program staff in presenting and/or publishing articles, abstracts, or results of research studies in professional journals or at professional meetings.
Assists with case management functions for patient with complex needs the impact diabetes care or related issues.
Business/Supervisory Functions:
Manages resources in a cost effective manner for the DSMES Program. Works with other DSMES Program staff to maintain a continuous and systematic process of curriculum development and assessment of patient educational material, clinical forms, and, protocols.
Works with Medical Support Assistants in coordinating scheduling of appointments, completion of consults and tracking of program participating data.
Performs other duties as assigned.
Quality Improvement and Monitoring:
Develops local facility indicators to provide objective evidence of the effectiveness of quality procedures and outcomes.
Participates in providing outcomes reports to local facility management and to the VISN upon request.
Monitors quality program indicators and tracks DSMES Performance. Measures for effective work practices and to verify adherence to quality plans and program requirements.
Communicates DSMES Program elements, updates, and challenges within the medical center, CBOCS, and at the VISN and national levels.
DSME Telehealth Programs: Home Telehealth Modalities
Coordinates the implementation and use of Clinical Video Conferencing Telehealth (CVT), Veteran's Video Connect (VVC), and Telephone patient car encounters, and Secure Messaging (My HealtheVet) for the DSMES Program.
Monitors completion and accuracy of documentation and workload capture that is basic for the completion of the visit.
Reports update or concern to local Facility Telehealth Council and Facility Telehealth Coordinator.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Registered Dietitian Nutritionist (RDN) or Registered Dietitian (RD) - Must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA).
Current VHA Dietitians that qualify based on being covered by the Grandfathering Provision may not be promoted to a higher grade if they do not meet the current RDN/RD requirements.
Grade Determinations: Dietician (Advanced Level Practice), GS-12
Experience: One year of expeirnce equivalent to the next lower grade level.
Certification. Must hold an advanced practice certification from a nationally recognized certifying body such as the Commission on Dietetic Registration, the Association of Diabetes Care and Education Specialists or the National Board of Nutrition Certification. The certification must be directly related to the assignment being filled. For example, where the assignment requires diabetes management, a Board Certified-Advanced Diabetes Management or Certified Diabetes Care and Education Specialists certification is qualifying. Commonly held certifications include Board Certified Specialist in Renal Nutrition; Board Certified Specialist in Gerontological Nutrition; Registered Dietitian Nutritionist-Advanced Practitioner; Certified Nutrition Support Clinician.
Knowledge, Skills and Abilities. In addition to the experience and certification above, the candidate must demonstrate the following KSAs:
(a) Skill in leading a team in the implementation of nutrition-related activities.
(b) Skill in utilizing evidence- based practice guidelines to establish or modify goals and improve patient outcomes.
(c) Ability to communicate in writing to develop policies and standard operating procedures.
(d) Ability to serve as a nutrition expert, providing technical guidance, direction and staff development.
(e) Ability to lead research or performance improvement studies.
(f) Ability to present complex nutrition information to a large group of health care providers to persuade and influence provision of care.
Preferred Experience: Certified Diabetes Care and Education Specialist (CDCES) certification and experience with diabetes technology are required.
