The Social Worker in this position will be assigned to the Middleton-Deming Way location and coordinates home and community based services for the Homemaker/Home Health Aide (HHA), Community Adult Day Care (CADHC), Purchased Skilled Home Care (PSHC), Community Nursing Home (CNH), Hospice and Respite programs. The position also includes support of the Home Based Primary Care (HBPC) program and other duties as assigned.

Duties

Major duties include, but are not limited to: Provides appropriate assessment, treatment plan formulation, and psychosocial intervention (within the established Scope of Practice for a Social Worker) to meet the needs of Veterans and families. These actions are documented in the electronic medical record. Establishes a social diagnosis/assessment to determine the most effective and culturally appropriate treatment method, and works collaboratively with the healthcare team to establish appropriate treatment goals based on identified clinical social problems. Maintains community activity, including coordination with other VA agencies and non-VA community agencies for proper service to Veterans and families.Possesses the knowledge and ability to independently implement treatment modalities, and provide counseling for individuals, families and groups (consistent with the assigned Scope of Practice); evaluates the outcome of interventions. Establishes and maintains effective working relationships with Veterans, family members, co-workers, hospital staff, and community agency/facility providers, as assessed per observance, report, and electronic medical record review. Has general knowledge of veterans' benefits and services relating to special programs, service connected compensation, non-service connected pension. Facilitates referrals based upon veteran needs and eligibility. Completes timely and appropriate clinical documentation in the electronic medical record, utilizing appropriate note titles, diagnostic codes, and event capture processes. Able to communicate effectively, both orally and in writing, with people from varied backgrounds. Documentation in the electronic medical record provides evidence (when appropriate) of interdisciplinary collaboration and social work continuity of care to ensure a comprehensive plan for Veteran care.Regularly interacts and collaborates with both outpatient primary care social work staff and inpatient acute care discharge planning staff on purchased community care treatment planning. The incumbent regularly interacts with non-VA community agencies and facilities, and provides information consistent with patient privacy regulations. Completes routine administrative processes and special projects in a timely and quality manner, as assessed per observation of the supervisor.Maintains consistent workload performance expectations and efficiency levels as defined by the supervisor and service chief. This includes completion of workload productivity reports. Completes referral/consult processes within established requirements for the work area, and consistent with hospital and national requirements. This includes completion of Non-Institutional Care (NIC) consults within 7 days, as well as hospital requirements for inpatient and outpatient consults and timely documentation consistent with work area/hospital requirements. Participates in an annual Professional Practice Peer Review process, including serving as a Peer Reviewer when assigned by the supervisor or service chief. Provides courteous, tactful, and prompt response to telephone calls and correspondence from Veterans or families, generally within two business days.Participates in inspection/survey processes, including administrative meetings and on-site visits to contract facilities. Participates in contract compliance reviews, and completes in a timely manner necessary assessments and reports. Participates in the education and training of other social work and interdisciplinary staff, as assessed per observation and review by the supervisor/service chief. Qualifications

BASIC REQUIREMENTS: U.S. Citizenship: Must be a Citizen of the United States. Language Proficiency: Must be proficient in both spoken and written English. Education: Must have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Social Worker, GS-11 (Full Performance Level)Experience and Licensure Appointment to the GS-11 grade level requires completion of a minimum of one year of post-MSW experience equivalent to the GS-9 grade level in the field of health care or other social work-related settings, (VA or non VA experience) and licensure or certification in a state at the independent practice level. NOTE: For appointment licensure or certification at this level please refer to paragraph 3c. ~OR~ Education In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required one year of professional social work experience in a clinical setting. Demonstrated Knowledge, Skills, and Abilities In addition to the experience above, candidates must demonstrate all of the following KSAs:(a) Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services.(b) Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds.(c) Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan.(d) Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals.(e) Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. Full Performance Level Assignments This is the full performance level. Social workers at this level are licensed or certified to independently practice social work. Incumbents are assigned to all program areas, including but not limited to: inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Physical Requirements Must be in overall good health, able to sit at a desk working at a computer, engage in light to moderate physical activity including sitting, walking, bending, and carrying supplies. Applicants must be able to perform primarily light and sedentary duties with occasionally moderate physical demands, exercise patience, and control emotions, with reasonable accommodation if necessary, without endangering the health and safety of the applicant or others. Position requires walking throughout the clinic and the Medical Center, standing for extended periods of time, and involves sitting at a computer and in counsel and consultation with Veterans and interdisciplinary teams. This position also requires close visual acuity to perform an activity such as: preparing and analyzing data and figures; transcribing; viewing a computer terminal; extensive reading; and repetitive motions of the wrists, hands, and/or fingers.
