The Dietitian Program Coordinator will be aligned with the Home-Based Primary Care which is a Special Population Patient Aligned Care Team and an integral component of the Rehabilitation and Extended Care Services provided by the Eastern Colorado Health Care System. The HBPC program targets Veterans with complex chronic diseases and provides cost effective, comprehensive, and interdisciplinary primary care services in the home.
Duties
The HBPC Program Manager Dietitian (RD) accountable for the management, supervision, and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC. They execute position responsibilities that demonstrate leadership, experience, and creative approaches to management and coordination of complex client service provision. Leadership is characterized as substantial and continuous responsibility for population groups or integrated programs that cross service lines and/or discipline lines and influence organizational mission and health care. He/she is responsible for the management and oversight of the HBPC program and other VA staffed in-home programs, contributing to Medical Center policy development, planning, decision making, resource management and evaluation that affect the delivery of health care services to patients. The HBPC Program Manager RD supervises assigned administrative and professional clinical staff within the HBPC program and other VA staffed in-home programs and provides mentorship throughout the Rehab and Extended Care GEC service.
Office space is in the Rocky Mountain Regional Medical Center (RMRMC) and the Pueblo and Colorado Springs Community Based Outpatient Clinics (CBOCs). Veterans are seen in their homes within the HBPC Southern and Northern Program boundaries.
Duties and skills needed to be successful in this position include, but are not limited to:
Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Rehab and Extended Care Service and all healthcare disciplines included on the HBPC teams.
Responsible for directing the clinical and administrative services offered by the program to ensure that the program is in compliance with local and VHA standards and policies as well as accreditation standards for home care organizations.
Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program managers, Veteran Integrated Service Network (VISN) leadership, and VA Central Office staff on a regular and ongoing basis.
Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility; communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary.
Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety; evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained; assures the program is able to optimize cost effectiveness (budget) and patient outcomes.
Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency; assesses the continuing education needs of program staff and assures appropriate resources to meet those needs; coordinates student education programs in HBPC.
Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA, and VA owned Vehicles; processes, assigns and tracks referrals for HBPC and ensures timely access to HBPC.
Responsible for managing the Electronic Waiting List (EWL) for HBPC referrals; Collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data.
The HBPC Program Manager RD for HBPC is responsible to provide assistance, collaboration and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment and contracting services to comply with regulatory standards.
Responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and or accrediting bodies.
The incumbent is regarded as the authority for home care program activities and within the incumbent's own initiative, provides definite interpretation of regulations, standards or codes pertaining to the program. The incumbent informs management of potentially controversial issues or problems with widespread impact but resolves conflicts as they arise both regarding administrative and clinical.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. Dietitians must be proficient in spoken and written English as required by 38 U.S.C. 7402(d).
EDUCATION:
(1) The individual must have earned a bachelor's degree from a U.S. regionally accredited college or university or foreign equivalent and fulfilled each of the following criteria:
(a) Completed a didactic program in dietetics accredited by the Accreditation Council for Education in Nutrition and Dietetics (ACEND), formerly known as the Commission on Accreditation for Dietetic Education (CADE).
(b) Completed an ACEND accredited or approved supervised practice program. Supervised practice programs are post-baccalaureate degree programs that provide supervised practice experiences which meet the eligibility requirements and accreditation standards of ACEND, formally known as CADE. Completion of a coordinated program in dietetics fulfills the requirements of a supervised practice program. Time spent in a dietetic internship or supervised practice program does not qualify as creditable experience.
(2) Individuals who have earned college and/or university degrees outside the United States and its territories also must have their transcripts evaluated by a foreign transcript evaluation service to determine if their degree is equivalent to a degree granted by a regionally accredited college or university in the United States. Validation of a foreign degree does not eliminate the need for verification of completion of didactic program, dietetic internship, or coordinated program requirements. The completion of ACEND didactic program requirements and at least a bachelor's degree from a U.S. regionally accredited college/university or foreign equivalent are necessary for eligibility to enter an ACEND-accredited supervised practice program.
REGISTRATION/CERTIFICATION:
All applicants must be registered with the Commission on Dietetic Registration (CDR) the credentialing branch of the Academy of Nutrition and Dietetics formerly known as the American Dietetic Association (ADA). Advanced Level Practice Dietitians must possess a minimum of one advanced practice credential relevant to the area of practice, (e.g. Certified Nutrition Support Clinician (CNSC), Board Certified Specialist in Renal Nutrition (CSR), Board Certified Specialist in Gerontological Nutrition (CSG), Certified Diabetes Educator (CDE), or Board Certified Specialist in Oncology Nutrition (CSO)).
Grade Determinations GS-13 Dietitian Program Coordinator
Experience: In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level, the candidate must demonstrate all the following KSAs and demonstrate the potential to acquire the assignment-specific KSA designated by an asterisk (*):

(a)Knowledge of program administration to establish and modify program goals and objectives, coordinate activities, evaluate accomplishments and solve problems.

(d) Ability to supervise subordinate staff (i.e., plan, organize, delegate, direct, control and review activities and performance).
(e) Ability to develop policies and procedures.
(f) Ability to provide program-specific coordination to include leadership, technical advice and staff development.
Assignment: For all supervisory assignments above the full performance level, the higher-level duties must consist of significant scope, complexity, difficulty, variety and be performed by the incumbent as a major duty at least 25% of the time. The incumbent must spend 25% or more of their time providing administrative and technical supervision over staff at the GS-11 or above. Supervisory Dietitians in this assignment serve as a Supervisory Dietitian Program Coordinator at a facility and report to a GS 13 or above. Supervisory Dietitian Program Coordinators lead a multi disciplinary team. Supervisory Dietitian Program Coordinators provide program-specific leadership and exercise a full range of supervisory and personnel management authorities and responsibilities in planning, directing and assessing the work of subordinate staff. They establish performance standards and evaluate the overall performance of employees; define competency requirements, identify developmental and training needs and take necessary actions to ensure subordinate personnel maintains and enhance technical expertise. Supervisory Dietitian Program Coordinators formulate budget and staffing requirements in managing organizational changes and ensuring multi-year and long-range national mission directives and program goals/objectives are met. They are administratively responsible for a specialty program. Supervisory Dietitian Program Coordinators may provide direct patient care as they develop and implement comprehensive nutrition therapy, practicing within their scope/clinical privileges. They provide program-specific guidance and direction and technical advice and staff development. Supervisory Dietitian Program Coordinators have oversight responsibilities for program and policy and procedure development, set goals and monitor and redefine priorities and may have an active role in fiscal management for the program. They direct or participate in designing clinical research activities or quality improvement studies, including data collection and analysis. Supervisory Dietitian Program Coordinators oversee and are responsible for developing and maintaining training and reference materials. They maintain program information through approved social media outlets.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position
requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
