The Home-Based Primary Care Program is a Special Population Patient Aligned Care Team (PACT) and an integral component of the Rehabilitation and Extended Care Services provided by the Eastern Colorado Health Care System. The HBPC Program provides care to Veterans that is interdisciplinary, accessible, comprehensive, coordinated, longitudinal, accountable, and patient-centered.

Duties

The HBPC Program Manager PT is a Physical Therapist accountable for the management, supervision, and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC. They execute position responsibilities that demonstrate leadership, experience, and creative approaches to management and coordination of complex client service provision. Leadership is characterized as substantial and continuous responsibility for population groups or integrated programs that cross service lines and/or discipline lines and influence organizational mission and health care. He/she is responsible for the management and oversight of the HBPC program and other VA staffed in-home programs, contributing to Medical Center policy development, planning, decision making, resource management and evaluation that affect the delivery of health care services to patients. The HBPC Program Manager PT supervises assigned administrative and professional clinical staff within the HBPC program and other VA staffed in home programs and provides mentorship throughout the Rehab and Extended Care GEC service. Office space is in the Rocky Mountain Regional Medical Center (RMRMC) and the Pueblo and Colorado Springs Community Based Outpatient Clinics (CBOCs). Veterans are seen in their homes within the HBPC Southern and Northern Program boundaries. Duties and skills needed to be successful in this position include, but are not limited to: Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Rehab and Extended Care Service and all healthcare disciplines included on the HBPC teams. Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program managers. Veteran Integrated Service Network (VISN) leadership, and VA Central Office staff on a regular and ongoing basis. Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility; communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary. Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety; evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained. Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency; assesses the continuing education needs of program staff and assures appropriate resources to meet those needs; coordinates student education programs in HBPC. Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA, and VA owned Vehicles; processes, assigns and tracks referrals for HBPC and ensures timely access to HBPC. Responsible for managing the Electronic Waiting List (EWL) for HBPC referrals; Collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data. The HBPC Program Manager PT is responsible to provide assistance, collaboration and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment and contracting services to comply with regulatory standards. Responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and or accrediting bodies. As a leadership member of the Rehab and Extended Care Service, participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring. The incumbent is regarded as the authority for home care program activities and within the incumbent's own initiative, provides definite interpretation of regulations, standards or codes pertaining to the program. The incumbent informs management of potentially controversial issues or problems with widespread impact but resolves conflicts as they arise both regarding administrative and clinical. Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education and experience. The individual must meet at least one of the requirements below: (1) Bachelor's degree in Physical Therapy AND five (5) years of progressively independent experience as a physical therapist. (2) Master's degree in Physical Therapy AND two (2) years of progressively independent experience as a physical therapist. (3) Doctorate degree in physical therapy. Licensure. Must have a full, current, and unrestricted license to practice physical therapy in a State, Territory or Commonwealth of the United States, or in the District of Columbia. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). All licensed PTs employed in VHA on the effective date of this qualification standard are considered to have met all qualification requirements for the title, series and grade held, including positive education and licensure that are part of the basic requirements of the occupation. For employees who do not meet all the basic requirements required in this standard, but who met the qualifications applicable to the position at the time they were appointed to it, the following provisions apply: (1) Employees grandfathered into the GS-0633 occupational series as PTs may be reassigned, promoted up to and including the full performance Gourney) level, or changed to lower grade within the occupation. (2) PTs who are appointed on a temporary basis prior to the effective date of this qualification standard may not have their temporary appointment extended or be reappointed on a temporary or permanent basis until they fully meet the basic requirements of this standard. (3) PTs initially grandfathered into this occupation, who subsequently obtain additional education and/or licensure/certification/registration, meet all the basic requirements of this qualification standard and must maintain the required credentials as a condition of employment in the occupation. (4) If PTs who are grandfathered under this provision leave the occupation, the employee loses protected status and must meet the full VA qualification standard requirements in effect at the time of reentry to the occupation. Grade Determinations: Experience. In addition to meeting the basic requirements, one year of progressively complex experience equivalent to the GS-12 grade level and one of the following: 1- Evidence of a minimum of 2,000 hours of clinical practice directly in the specialty area. This time must be between the past two and five years; or 2. Board Certification in the specialty area recognized by the ABPTS; or 3.-Completion of a fellowship program, or 4- An additional advanced degree in a related field. In addition, to meeting the required specialized experience, the candidate must demonstrate the all following KSAs. 1- Advanced knowledge and expert skill in a range of specialized interventions and treatment modalities used in a specialized treatment area of physical therapy. 2- Ability to serve as a consultant and subject matter expert for health care providers regarding the delivery of care within a specialized content area of physical therapy (see paragraph 2i). 3. Ability to teach and mentor medical center staff in a specialized content area of physical therapy. 4- Ability to guide the work of a multi-disciplinary team in a specialized content area of physical therapy. 5.. Advanced knowledge and expert skill to perform and interpret specialized procedures and tests in evaluating the treatment outcomes in the assigned specialty content area (see Definitions, paragraph 2. i.). Preferred Skills - 2 years current management or leadership experience; 1 year Home Care or equivalent; Experience with interdisciplinary team(s). References: VA HANDBOOK 5005/99 PART II APPENDIX G12 Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
