The Kinesiotherapist Program Coordinator will be aligned with the Home-Based Primary Care (HBPC) Program which is a Special Population Patient Aligned Care Team (PACT) and an integral component of the Rehabilitation and Extended Care Services provided by the Eastern Colorado Health Care System (ECHCS). The HBPC Program provides care to Veterans that is interdisciplinary, accessible, comprehensive, coordinated, longitudinal, accountable, and patient-centered.
Duties
The HBPC Kinesiotherapist Program Manager is accountable for the management, supervision, and evaluation of care for patients enrolled in the HBPC and other VA staffed in-home programs that are organizationally aligned under HBPC. They execute position responsibilities that demonstrate leadership, experience, and creative approaches to management and coordination of complex client service provision. Leadership is characterized as substantial and continuous responsibility for population groups or integrated programs that cross service lines and/or discipline lines and influence organizational mission and health care. He/she is responsible for the management and oversight of the HBPC program and other VA staffed in-home programs, contributing to Medical Center policy development, planning, decision making, resource management and evaluation that affect the delivery of health care services to patients. The HBPC Kinesiotherapist Program Manager supervises assigned administrative and professional clinical staff within the HBPC program and other VA staffed in-home programs and provides mentorship throughout the Rehab and Extended Care GEC service.
Office space is in the Rocky Mountain Regional Medical Center (RMRMC) and the Pueblo and Colorado Springs Community Based Outpatient Clinics. Veterans are seen in their homes within the HBPC Southern and Northern Program boundaries.
Duties and skills needed to be successful in this position include, but are not limited to:
Facilitates an atmosphere of interactive management and development of collegial relationships among interdisciplinary team members, serving as a link between Rehab and Extended Care Service and all healthcare disciplines included on the HBPC teams.
Collaborates with the HBPC Medical Director for daily program management and collaborates with other HBPC program managers, Rocky Mountain Network Human Resources, Veteran Integrated Service Network (VISN) leadership, and VA Central Office staff on a regular and ongoing basis.
Responsible for the interpretation and communication of VA policies to the HBPC team and the VHA facility; communicating and interpreting non-VA national, state, and local home care policies and facilitates coordination and communication with non-VA care providers when necessary.
Responsible for the HBPC team development and implementation of local HBPC policies and procedures to include performance improvement, patient safety, utilization review, emergency preparedness, and staff safety; evaluation of the effectiveness of the program and assuring effective/efficient resources are maintained.
Collaborates with the HBPC Medical Director to manage human resources for the HBPC Program including selecting qualified staff, orients and mentors the staff, promotes staff development and professional growth, and assures a process for ongoing staff clinical competency; assesses the continuing education needs of program staff and assures appropriate resources to meet those needs; coordinates student education programs in HBPC.
Responsible for managing and tracking program resources to include but not limited to equipment inventory, GSA, and VA owned Vehicles; processes, assigns and tracks referrals for HBPC and ensures timely access to HBPC.
Responsible for managing the Electronic Waiting List (EWL) for HBPC referrals; Collaborates with the business office and fiscal service to ensure accurate reporting of HBPC clinical, administrative and workload data.
The HBPC Program Manager SW for HBPC is responsible to provide assistance, collaboration and subject matter expertise for a complex group of services HBPC often works with, such as Home Oxygen Programs, Home Medical Equipment and contracting services to comply with regulatory standards.
Responsible for evaluating program activities and providing follow up actions required by internal and external review organizations and or accrediting bodies. As a leadership member of the Rehab and Extended Care Service, participates in the formulation of overall service/program policies and procedures, goals, objectives, strategic planning, cost effectiveness, and quality monitoring.The incumbent is regarded as the authority for home care program activities and within the incumbent's own initiative, provides definite interpretation of regulations, standards or codes pertaining to the program. The incumbent informs management of potentially controversial issues or problems with widespread impact but resolves conflicts as they arise both regarding administrative and clinical.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Be a citizen of the United States. (Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.)
English Language Proficiency. Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. 7403(f).
Education - A bachelor's degree [or higher from a program accredited by the Commission on Accreditation of Allied Health Education Program, with a concentration/emphasis in kinesiotherapy. OR (2) A bachelor's degree or higher with a concentration/emphasis in kinesiotherapy or exercise science related field and registered as a kinesiotherapist in good standing with the COPSKT. OR (3) A master's degree or higher with a degree in exercise science, kinesiology, human performance or a related field, which has been approved by COPSKT and a certificate verifying completion of a Center of Excellence (COE) Kinesiotherapy clinical training program. The completion of the certificate ensures that COPSKT has approved the degree program as part of the application process for the COE internship.
Foreign Graduates. Graduates of a foreign program must meet the requirements in subparagraph 3b if they have a full unrestricted and current registration to practice kinesiotherapy in a State, Territory, or Commonwealth (i.e., Puerto Rico) of the United States or in the District of Columbia. They must have at a minimum, proof of a degree from an accredited college or university (or foreign equivalent, as verified through an independent credentialing evaluation company). d.
Registration. All applicants must be registered in kinesiotherapy in good standing with the COPSKT.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
GS-13
Experience and Education. One year of experience equivalent to the next lower grade level.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Ability to supervise, motivate, and effectively manage staff by organizing work, setting priorities, and delegating tasks and setting responsibilities.
(b) Ability to disseminate appropriate information through various media as a consultant or mentor.
(c) Skill in interpersonal relationships in dealing with employees, other team leaders, and managers.
(d) Knowledge of specialized programs and the ability to perform the administrative duties of special programs and/or components of a kinesiotherapist section.
(e) Knowledge of decision-making principles necessary to adjust programs on a day to day basis, to develop short term and long range goals, and/or to plan for future utilization of human resources, and the reassignment of staff to enhance/develop the value of existing or new programs.
Assignment: For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and range of variety, and be performed by the incumbent at least 25% of the time. Supervisors are found in Complexity Level 1 (High Complexity) facilities, or in facilities of lesser complexity levels where expanded services are performed. Monitor and modify clinical program development and resources (e.g., personnel, space, equipment, and supplies), in order to ensure that the objectives of treatment and delivery of care have been achieved. Ensure that best practices and procedures are implemented and utilized. The supervisory kinesiotherapists monitor evaluation, treatment, encounter data, collect and report workload and other required statistical data, and patient documentation practices using current research and/or studies to assure delivery of quality care. Supervisors are responsible for providing administrative supervision for planning, directing, organizing, coordinating, implementing, controlling and evaluating the overall administrative operations and clinical procedures, guidelines and protocols of the department. Supervisory kinesiotherapists identify and recommend continuing education and training needs for the kinesiotherapy staff. Monitor and approve or disapprove training and leave requests. Evaluate the work performance of subordinates, provide feedback to employees, and annual written performance appraisal evaluation. The supervisors develop and update performance evaluation standards and functional statements, making recommendations for appointments, awards, advancement, and when appropriate, disciplinary actions. Recruit and select candidates for hiring, recommending promotions, reassignments, or other personnel changes to higher level management. Resolve grievances and appeals that have been elevated through the chain of command to the supervisory level.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. Mechanical inpatient lifts are provided. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to
patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
