The Physician Assistant provides comprehensive and quality care to patients in the FHCC by remaining fully accountable for adhering to established policies, standards of care and standards of practice, and demonstrating leadership and accomplishments when providing patient care and developing and implementing programs which will improve the delivery of care relating to Geriatrics and Extended Care.

Duties

Major Duties and Responsibilities: The Physician Assistant develops monitors and alters a comprehensive health care treatment program to include: 1 Gather a comprehensive database and perform initial and ongoing assessment of assigned patients' medical, physical, and psychosocial status: a. Conduct and document histories and physicals. b. Order and evaluate diagnostic tests, i.e., lab works, x-rays, EKGs, consults, and similar studies. c. Develop treatment plans in collaboration with the attending physician and health care team. Contribute to the Care Plan for the Minimum Data Set (MDS) d. Perform Abnormal Involuntary Movement Scale (AIMS) evaluations. e. Conduct rounds in the Community Living Center f. Record progress notes, write discharge notes and summaries. g. Generate appropriate referrals to other health care professionals and community agencies according to established procedures. h. Attend Interdisciplinary Treatment Team meetings. 2 Discriminate between normal/abnormal findings and formulating a treatment plan accordingly. Communicate with attending physicians regarding abnormal findings. 3. Initiate and adjust selected medications with formulary approval for inpatients. 4 Document in the medical record all procedures performed for which the Physician Assistant is responsible. Institute and adjust plans of care, utilizing Physician Assistant skills. 5. Provide appropriate response to life-threatening emergency situation when no physicians are immediately available. 6 Document clinical findings and progress notes in the medical record. 7. Develop and provide patient treatment and education plan. 8. Conduct preventive screening procedures based on age and history. 9. Provide wellness care, health promotion, health maintenance, and disease prevention care. 10 Initiate and monitor health maintenance in conjunction with the patient, family/significant other and members of the multi-disciplinary team. 11 Provide and promote teamwork support and collaboration with multi-disciplinary team members. 12 Participate in interdisciplinary team conferences to assure the quality patient care. 13.Participating in the quality assurance program 14. Act as consultant to nursing personnel, physicians, and other health care consultants in their area of clinical experience. 15. Determine, in collaboration with Attending physician, appropriate admissions, discharges referrals and transfers to different levels of care and coordinates management of patient with Geriatric Medicine attending' s and the Interdisciplinary team. Work Schedule: Monday - Friday, 8:00am to 4:30pm Telework: Ad-hoc Financial Disclosure Report: Not required

Qualifications

Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Physician Assistant Basic Requirements Master's degree in any discipline or a bachelor's degree in any discipline with one year of experience as a Physician Assistant ~AND~ Successfully passed the PA National Certifying Exam (PANCE) (or are you in the process to taking the exam) ~AND~ Currently hold, or will hold, a full and unrestricted license to practice in a State, Territory, or Commonwealth of the United States or in the District of Columbia. Grade Determinations: will be based on education and experience References: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-8 Physician Assistant Qualification Standard. This can be found in the local The full performance level of this vacancy is 03. The actual grade at which an applicant may be selected for this vacancy is in the range of 00 to 03. Physical Requirements: To perform in a fully successful manner, the individual must have the physical ability to perform job-related duties which may require light carrying (under 15 pounds), reaching above shoulder, use of fingers, both hands required, walking (up to 1 hour), standing (up to 1 hour), and both legs required. In addition, there may be exposure to the following environmental factors: working inside and outside, working closely with others, and protracted or irregular hours of work. A health examination must be successfully completed prior to this assignment and periodically thereafter as required by the Occupational Health Clinic policy. There may be intermittent exposure to blood and body fluids.
