The Medical Foster Home Program Coordinator at Eastern Kansas Healthcare System, will work at either the Dwight D. Eisenhower VA Medical Center, Leavenworth Kansas, or the Colmery O'Neil VA Medical Center in Topeka Kansas, but may travel to the opposite facility and all associated CBOC's. The incumbent oversees program development, recruitment, and development of the foster homes, recruitment and admission of veteran residents, case management, development of policies and care processes.

VA Careers - Social Work: https://youtube.com/embed/enRhz_ua_UU The Medical Foster Home (MFH) Program Coordinator for Eastern Kansas Healthcare System (EKHCS) is responsible for the professional and administrative management of the Medical Foster Home Program. The MFH Coordinator carries out all administrative and oversight functions of the program which requires complex knowledge of various program designs, operations, guidelines, standards, performance, and outcome measures. The coordinator manages the daily operation of the program and provides direct Veteran-centric services in the program area. Responsibilities include recruitment and approval of medial foster homes, managing the daily operation of the program, developing policies and procedures for program operation, and preparing reports and statistics for facility, VISN, and national use. The incumbent is also responsible for direct patient care including assessing appropriateness for program, psychosocial assessment, treatment planning and working with the interdisciplinary team to assure and monitor quality of care and services provided within the program. The incumbent is responsible for maximizing psychosocial quality of life issues to assist veterans to remain in their home or in a less restrictive level of care other than a nursing home and avoid hospitalization as often as possible. The incumbent serves veterans who tend to have serious, frequent, and severe crises, lack family or an adequate community support network, have poor self-monitoring and advocacy skills; inability to understand and follow instructions and treatments; have significant deficits in coping skills and require continuing psychosocial and medical support. The incumbent should have knowledge or practice experience related to dementias, chronic illnesses, traumatic brain injuries and care for the older adult and caregiver. The Incumbent will be required to travel for recruitment and to inspect homes that are interested in being certified as foster homes. Travel between facilities and other CBOCS may be required. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Experience and Education: One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification: Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determination: Social Worker (Program Coordinator), GS-12 Experience and Education: Applicants must have at least one (1) year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Demonstrated Knowledge, Skills, and Abilities: In addition to the experience above, the candidate must demonstrate all of the following KSAs: (a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. (b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. (c) Ability to supervise multidisciplinary staff assigned to the program. (d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). (e) Ability to provide training, orientation, and guidance within clinical practice. Assignments. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12 grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff. They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts.] References: VA Handbook 5005/120, Part II Appendix G39 Social Worker Qualification Standard GS-0185 Veterans Health Administration. The full performance level of this vacancy is GS-12. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations, operative/invasive procedures, or home settings. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements, such as annual TB screening or testing, as a condition of employment. The incumbent who provides care in home settings or other off-site locations may be required to drive and/or ride in GSA-vehicles.
