The Licensed Practical Nurse works under the direction of an RN. The LPN is responsible for observing, recording, and reporting changes in the patient's condition, as well as assisting in the development and revision of the patient-driven holistic life care plan. The LPN requires a general review of their work by an RN for appropriateness and conformity with license, policies, and procedures. The Primary Care LPN is a member of the Core Team, working as a Clinical Associate (CA).
Duties
Makes observations and thoroughly documents care and the patient's response. Appropriately seeks assistance and guidance by informing RN or medical provider of changes in the patient's condition requiring higher-level intervention.
The LPN performs, reports, and records assigned nursing care in the Primary Care Clinics, including, but not limited to:
Oxygen therapy
Medication administration to include PO, IM, SQ.
VS Data Collection including height and weight.
Prepare the patient for the PCP appointment
Clinical reminders with appropriate consults and alerts as indicated
Check expiration dates of meds and supplies
Delegated procedures and treatments
Documentation
Assists with minor procedures, including obtaining and arranging necessary equipment, positioning patient, ensuring comfort and privacy. Monitors patient during the procedure.
Lab specimen collection (including blood draws)
Functions listed in the PACT LPN SOP
Performs EKGs
The LPN performs, reports and records assigned nursing care in the PACT Clinic with minimal supervision and assistance from the RN, including, but not limited to:
Performs initial screening for patients to include VS data collection including height and weight
Clinical reminders with appropriate consults and alerts as indicated
Prepare the patient for the provider appointment
Performs and assists with virtual appointments.
Lab specimen Collection (including blood draws, iStats, Coaguchek, and Accuchecks).
Performs finger sticks, peripheral blood draws
Maintains competencies in point of care testing (The same is expected if/when covering in the Therapeutic Monitoring Clinic (TMC)
Adheres to standards of practice and safe technique in preparation and administration of medication.
Demonstrates understanding of age- and population-specific characteristics that affect the health care needs of the Veteran.
Promptly recognizes urgent or emergent patient care situations, seeks assistance, and effectively intervenes as directed when appropriate.
Based on the patient's goals, provides patient and family health education, to include self-management, prevention, and wellness.
Function as a Health Coach.
Interacts and communicates appropriately with patients and family members.
Demonstrates professional behavior (i.e., respect others, collegiality, ability to accept constructive criticism), effective communication and interpersonal skills.
Adheres to principles of infection control, safety, and universal precautions.
Possesses knowledge of proper handling and pre-cleaning of reusable medical equipment.

Demonstrates initiative in assisting the core and expanded teams.
Assists in the orientation and competency validation of students, peers, and clerical staff.
Maintains confidentiality of electronic, written, and/or verbal patient information.

Coordinate communications during the inpatient to outpatient discharge interface, communicating with VA and non-VA medical offices/facilities to coordinate care.
Notify patients of normal and abnormal lab or other test results as instructed by the provider.
Possess knowledge of quality assurance/performance improvement principles and conduct individual quality assurance/process improvement checks/studies.
Promote veteran registration for and utilization of My Healthe Vet (MHV), online personal health record for veterans, including performing IPA (in-person authentication) for veterans requesting initial access to individually identifiable health information with/in the My Healthe Vet web-based application.
The LPN will be expected to work as relief for the RN in his/her absence for those duties that fall within the LPN Scope of Practice and Licensure. This includes but is not limited to:
Coordinate communications during the inpatient to outpatient discharge interface, communicating with VA and non-VA medical offices/facilities to coordinate care.
Post discharge and/or post ER visit, telephone or secure message follow up to see how the patient is doing, reinforcing the plan of care, and encouraging self-help solutions.
Other duties as assigned.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Education - Graduate of a school of practical or vocational nursing approved by the appropriate State agency and/or [accredited by] the National League for Nursing [Accrediting Commission (NLNAC)] at the time the program was completed by the applicant. Verification can be obtained from the State approving agency [or] the National League for Nursing [Accrediting Commission], 61 Broadway, 33rd Floor, New York, NY 10006. [Additional information may also be obtained from the NLNAC Website. Exceptions include:
(1) Health care education in the military service or training in the military service which is accepted by the licensing body in the jurisdiction in which the individual is licensed as qualifying for full LPN/LVN licensure will be accepted as meeting the education requirements for VHA employment.
(2) VHA employees who were converted to appointments under 38 U.S.C. 7401(3) in implementation of Public Law 98-160, enacted November 21, 1983, and for VHA employees converted to appointments under 38 U.S.C. 7405 in implementation of Public Law 99-576, enacted October 28, 1986; and for their future personnel actions under this authority. The waiver will not apply to persons appointed under 38 U.S.C. 7401(3) or 7405 after these conversions.
(3) Individuals granted a license by a jurisdiction that does not require graduation from an approved school. See paragraph 4, section C.
Licensure - Active, full, current and unrestricted licensure as a Graduate Licensed Practical or Vocational Nurse in a State, Territory, or Commonwealth (i.e., Puerto Rico), of the United States or the District of Columbia.
Grade Determinations: In addition to the basic requirements stated above, the following qualification criteria must be met for each grade. The candidate's qualifications must clearly demonstrate the level of competence required for the grade.

Proficiency in initiating, performing, and completing assigned duties in providing care to variable patient populations.
Knowledge and ability to appropriately carry out assigned patient care based on the patients' conditions, using judgment in selecting the appropriate order and sequence of procedures and treatments, and accurately recognizing, reporting, and recording relevant patient information.
Ability to observe, identify, and response to a patient's needs for care including medication, equipment-assisted care, and patient/family education.
Prepares and administers prescribed medications (oral, topical, subcutaneous, intramuscular and/or intravenous) and performs treatments according to established policies/procedures. Observes for, documents, and reports physical and/or emotional changes in the patient's condition from prescribed medications/treatments.
Knowledge and ability to recognize urgent or emergent patient care situations and initiating appropriate emergency interventions as directed.
Knowledge and understanding of human behavior, patient motivations and reactions to situations and the ability to appropriately utilize this knowledge in working effectively with patients, family members and other staff.
Knowledge and skill in performing support duties for complex diagnostic tests and/or specialized practices or procedures, which includes preparing the patient and assisting in the diagnostic examination, preparing, and handling specialized instruments or other specialized equipment, and monitoring the patient's condition before, during, and following the procedure.

Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, stooping, sitting, bending, pulling, pushing and reaching over head. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don protective clothing in isolation situations or operative/invasive procedures. The incumbent may occasionally be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations, able to shift priorities based on patient needs. Must complete annual Employee Health requirements; such as annual TB screening or testing, as a condition of employment. Incumbent may be exposed, on a short term basis to fumes, dust and other debris. See VA Directive and Handbook 5019.
