The Social Work Program Coordinator (Care Coordination and Integrated Case Management) provides independent and expert clinical psychosocial and case management services in a specialized area of practice to Veterans who tend to have serious and severe crises, may lack any familial and community support, may be poor self monitors, may frequently fail to comply with instructions and treatment, or may have major deficits in coping skills and require continuing professional psychological support. The Social Work Program Coordinator will adhere to the NASW Code of Ethics, all local Social Work policies and directives, and maintain an unrestricted, unsuspended license to practice Social Work at the advanced independent licensure level.
The Social Work Program Coordinator (Care Coordination and Integrated Case Management) prepares related reports for submission to VA Central Office (VACO); manages and controls the use of data for daily program operations, evaluations and improvements. In collaboration with the Social Work, Primary Care, Medicine and other Services, the Social Work Program Coordinator (Care Coordination and Integrated Case Management) plans, organizes and directs administrative, operational, Service Member/Veteran and personnel activities for the service.
This Social Worker (Program Coordinator) position is located in Connected Care at the VA Bedford Medical Center, MA. This is a full-time position at 40 hours per week.
Duties may include, but are not limited to:
Develops, implements and oversees all components of VA Bedford HCS Integrated Case Management program and its implementation across the healthcare system.
Administers information and analytical systems to identify Veterans that might benefit from Integrated Case Management interventions.
Coordinates care and serves as the integration liaison between Executive Leadership and Case Management Staff.
Serves as a consultant and subject matter expert on Case Management practice, including keeping abreast of VA policies, laws, and regulation that affect CM delivery to Veterans, their families, and caregivers.
Coordinates and collaborates with the VA medical facility level Sponsor, care coordination, care management, and Case Management service area leads.
Provides oversight of implementation of local Care Coordination Review Team (CCRT).
Collaborates with internal stakeholders to assess Case Management composition, structure, and services to identify promising practices and gaps in Case Management services and identifying opportunities to enhance communication, collaboration, and coordination across the care continuum.
Serves as a VA medical facility consultant regarding quality assurance pertinent to monitoring Case Management performance measure and meeting Case Management performance metrics.
Researches community resources (i.e. local, State, and national) available to provide continuity of care and to enhance the quality of life.
Develops policies and Standard Operating Procedures (SOPs) to ensure compliance with the Joint Commission, Commission on Accreditation of Rehabilitation Facilities (CARF), and other accrediting bodies and regulatory standards.
Establishes collaborative relationships with physicians, psychologists, nurses, and other disciplines to foster their involvement in the Integrated Case Management Program
Establishes performance and outcome standards for the facility Integrated Case Management Program that promote quality and efficiency of services in alignment with the facility, VISN and VA Central Office mission and strategic plan.
Manages the daily operation of the Integrated Case Management Program and Social Work Programs under his/her purview and provides accountability for program effectiveness and modification of service patterns to enhance patient-centered care and customer satisfaction.

Basic Requirements:
United States Citizenship: Be a citizen of the United States. (Non-citizens may be appointed when it is not possible to recruit qualified citizens in accordance with chapter 3, section A, paragraph 3g this part).
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. In addition to meeting the basic requirements as stated above, you must have the following experience to qualify for this GS-12 Social Worker (Program Coordinator) position:
Grade Determinations:
Experience GS-12 Grade Level: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience.
Licensure/Certification. Individuals assigned as Social Worker (Program Coordinator) must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure.
Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs:
(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.
(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.
(c) Ability to supervise multidisciplinary staff assigned to the program.
(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).
(e) Ability to provide training, orientation, and guidance within clinical practice.
