The Clinical Pharmacy Practitioner (CPP) serves in the Home-Based Primary Care (HBPC) section of Geriatric and Extended Care Service (GEC). The HBPC CPP is responsible for providing and managing all clinical pharmacy services of the HBPC program at this Medical Center, including but not limited to policy development, quality resources and coordination of of educating staff to optimize patient outcomes and cost effectiveness.

Operates independently under a defined scope of practice to effectively assure comprehensive, accurate drug/chart reviews are complete. Review prescriptions and/or orders for accuracy, eligibility, availability, cost effectiveness and therapeutic appropriateness. Reviews patient medication profiles; contact HBPC providers for clarification as indicated. Incumbent functions independently in day-to-day clinical and administrative duties associated with the position. Visit patients in their homes to provide care for diabetes, HTN, etc., under a scope of practice. Analyzes appropriate laboratory tests and other diagnostic studies necessary to monitor and support the patient's drug therapy. Applies knowledge of normal laboratory values in the evaluation of patient care. Recognizes significant abnormalities, and makes dose adjustment recommendations based on clinical and laboratory findings. Reviews patient medication regimens for clinical effectiveness, drug selection, dosing, contraindications, side effects, potential drug interactions, and therapeutic outcomes as required. Communicates findings with prescribers and provides appropriate alternatives to current treatment plans as needed. Functions to design and monitor therapeutic drug plans to achieve definite outcomes through direct interactions with patients and providers. Interprets the physical measurements necessary to monitor, support and modify the patient's medication therapy. Monitors for adverse drug reactions, allergies, and patient compliance issues. Promotes and monitors compliance for established drug therapy policies. Works with team providers to ensure compliance with national, VISN, and local initiatives. Demonstrates recognized expertise in the area of geriatrics and the HBPC Pharmacy program. Interacts with providers and other health care professionals as demonstrated by regular attendance at HBPC multidisciplinary team meetings, daily medication review rounds, and other meetings for the purpose of providing consultation in all Pharmacy-related matters. Works closely with providers and clinical staff to review patient response to medications, including efficacy, safety, and outcome, with special attention to polypharmacy problems in the elderly. Assumes a consultative role related to a significant impact of interventions on pharmaceutical care, costs, patient compliance, reduction in morbidity/mortality, selection of formulary products, reduction in numbers of medications, etc. Performs initial and periodic assessment of patients' medication therapy, monitors as needed, and makes recommendations or changes (within scope of clinical privileges) as necessary. Reviews for appropriateness of pharmacologic therapy with consideration for patient complexity, patients goals of care, geriatric concerns unnecessary or duplicate therapy, psychotropic medications, BEERS criteria, and medications with potential to increase fall risk. Works with the HBPC interdisciplinary team to design, implement, monitor, and document pharmaceutical care plans to achieve positive, patient-centric outcomes. Completes documentation via CPRS Progress Notes of all medication reviews and consultation interactions. Provides verbal and written medication information to providers on request. Assists the HBPC section to meet all of the Joint Commission medication management standards. Participates in developing criteria for drug utilization evaluations, data collection, trending reports, and other quality assurance activities Reviews and evaluates requests for non-formulary and restricted drugs for appropriateness and compliance with established criteria where applicable. Assesses data reflective of the patient's status and interpret the appropriate information needed to identify each patient's requirements relative to his or her age-specific needs, end of life symptoms, and to provide the care needed. Provides clinical support for patient care: fall risk reduction, curbside consults, continuous pertinent education, drug information requests from providers Participates in home and telehealth visits, for comprehensive medication or disease state management or medication safety concerns. Education: Graduate of an Accreditation Council for Pharmacy Education (ACPE) accredited College or School of Pharmacy with a baccalaureate degree in pharmacy (BS Pharmacy) and/or a Doctor of Pharmacy (Pharm.D.) degree. Licensure: Full, current and unrestricted license to practice pharmacy in a State, Territory, Commonwealth of the United States (i.e., Puerto Rico), or the District of Columbia. The pharmacist must maintain current registration if this is a requirement for maintaining full, current, and unrestricted licensure. Grade Determinations: GS-13 Experience: Must have 1 year of experience equivalent to the GS-12 grade level which includes 1 year of experience equivalent to the GS-11 grade level, OR completion of an ACPE-accredited Pharm.D. program. Knowledge, Skills, and Abilities (KSA's): Pharmacists assigned to this position must demonstrate the following KSA's: Ability to communicate orally and in writing to persuade and influence clinical and management decisions. This may include participation on various committees pertaining to HBPC, reviewing charts and drug therapy for patients, performing clinical assessments (both face to face and through chart reviews), interpreting lab data, providing drug information services and educational materials for staff and patients. Preferred Experience: PGY1 or PGY2 ASHP accredited Pharmacy Residency training. Board Certification in Geriatrics. At least one year of Clinical ambulatory care experience in Geriatrics and or HBPC. Physical Requirements: The work is primarily office based, including all wards and clinics of the VA Medical Center. All areas are adequately lighted and climate controlled. The work involves everyday risks or discomforts and requires normal safety precautions typical of meeting and training rooms and office settings. Some standing, bending and carrying light items such as books, medical records and paper are required. Due to the nature of the work, the incumbent may come in contact with Veteran patients who are confused, intoxicated or have a contagious condition. The work does require walking, at least one-half mile per day based on the location of the office.
