The Nephrology midlevel provider will be an integral member in establishing and managing a panel of Veterans assessed as needing nephrology care and follow-up visits, managing Veteran's needs for follow up (including imaging, tests, and clinic visits), managing consults, managing referrals for nephrology treatment, follow up with Veterans who have received their nephrology care in the community by assuring these Veterans are managed appropriately and not lost to follow up.
Duties

The employee will be primary clinical surrogate for other licensed independent provider(s) at their assigned duty station.
Triage consults, handle phone calls with Veterans, have clinical follow-up appointments, learn to perform telehealth visits and VVC visits as well.
Review all outpatient nephrology consults ordering any missing pre-referral work up not completed, then triaging for appropriate care management either through eConsult process, Tele-Health, or sending out to a community provider.
Work with Montana VA Staff, eConsult and Tele-Health Providers following their care plan for patients with recommendation of further diagnostic evaluations including laboratory and pathology evaluations to facilitate care.
Communicate with PCP related to the care plan.
Manage a panel of Veterans needing follow-up care through, phone clinics, VA Video Connect and clinical face to face visits. May have to travel to Montana CBOC's to provide face to face care when is appropriate.
Order and prescribe treatments and medications aimed at preventing health problems or maintaining health.
Make informed decisions of therapeutic intervention based on patient information and preferences, up-to-date scientific evidence, and clinical judgement and consult with the Nephrologist when needed.
Aid administration when requested. These requests may include, but are not limited to, assisting with data assimilation for VA reports, giving advice on programs that are directly or indirectly related to the nephrology process, or other tasks as determined by the Associate Chief of Staff.
Make decisions which serve to enhance the distinctive and contributory role of NPs within the local institution and the total healthcare system.
Participate in long range program planning and model development for effective delivery of health services.
Assure implementation, maintenance, and compliance with standards of professional practice of all health care personnel.
Practice as a Nurse Practitioner characterized by leadership and participation in developing and implementing programs to improve the delivery of nephrology patient care and by accomplishments in all the following areas: Systematically evaluate current clinical practice, initiate changes in clinical practice based on current concepts and findings from research and/or studies; demonstrate such expertise in patient care that advice is sought by others.
Access, plan, implement, coordinate, and evaluate educational program activities; initiate change in education programs based on current concepts and findings from research and/or studies; teach others effectively and creatively.
Demonstrate the knowledge and skills necessary to provide care appropriate to the age of the patients served by Montana VA. S/he must demonstrate the knowledge of the changes associated with aging and possess the ability to provide care based upon physical, psycho/social, cultural, safety and other age-related factors as noted in the competencies described in the departmental and/or unit specific policies and procedures.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Master's or Doctoral Degree from a program accredited by the Accreditation Commission for Education in Nursing or the Commission (ACEN-Formerly NCLAC) or the Collegiate Nursing Education (CCNE).
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Nurse Practitioners must be licensed or otherwise recognized as a nurse practitioner in a State and maintain full and current certification as a nurse practitioner from the American Nurses Association or another nationally recognized certifying body. The certification must be in the specialty to which the individual is being appointed or selected.
Preferred Experience and Certifications -Nephrology experience.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level III - Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - Master's degree in nursing or related field with a BSN and approximately 1-2 year's of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 year's of nursing practice/experience; OR a Doctoral degree and approximately 2-3 year's of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: ???????The incumbent must be able to perform all activities of the position without restriction. This position involves considerable standing, walking, stooping, bending, reaching, lifting and turning. It requires repositioning patients and assisting with transferring patients to wheelchairs/stretchers. Mechanical devices when available are required to be used for heavy or difficult to lift patients. The work may involve pushing or lifting weight in excess of 50 pounds and going up and down flights of stairs. The position requires visual and auditory acuity at a level to provide safe and effective nursing care. The positions requires the nurse to work for periods of time with arms above shoulder level or with the neck in a fully flexed or extended position and the ability to physically control or protect him/herself from physically abusive patients.
