This position works for the James A. Haley Veterans' Hospital and Clinics (JAHVH) in Tampa, Florida, as a direct report to Social Work Service, and is assigned to the Physical Medicine and Rehabilitation Program. This position provides care coordination, discharge planning, psychosocial & family support services to Veterans, Service Members & their Family Members/Caregivers for The Polytrauma Network Site (PNS).
Duties
The Polytrauma Network Site (PNS) represents a vital link in the continuum of PSC rehabilitation services between the regional referral center at the Polytrauma Rehabilitation Center (PRC) and the local resources at the Polytrauma Support Clinic Team (PSCT) and the Polytrauma Point of Contact (PPOC). The PNS Program Coordinator is the primary outpatient point of contact in the Polytrauma Network Site and coordinates services, resources, and communications among the patient, family, military and VA.
Major highlights include, but are not limited to:
Rehabilitation care at the PNSs focuses on outpatient services, but inpatient bed units are also available to address post-acute and chronic complications.
PNSs maintain a full complement of rehabilitation professionals on staff to address complex TBI and polytrauma-related symptoms and functional deficits. PNS staff provide clinical oversight of the PSC programs within their VISN including care coordination, managing referrals and consultations, and advising on the collection and reporting of data.
PNS provides leadership from clinical, educational, and administrative services related to polytrauma and TBI rehabilitation within the VISN including: coordination of polytrauma and TBI services, specialty consultations for patients with polytrauma and TBI, developing and maintaining an inventory of VA and non-VA sources for polytrauma and TBI services available across the VISN, and making them available to patients, families, and VA facilities with a PSCT and PPOC, developing and conducting VISN level education programs for providers patients and families, and collaborating in tracking VISN level outcome date and performance measures.
The PNS Coordinator role is responsible for PNS Program Development.
The PNS Coordinator is responsible for case managing patients with Severe TBI and other related disorders.

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency: Candidates must be proficient in spoken and written English to be appointed as authorized by 38 U.S.C. § 7403(f).
Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may NOT be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a masters of social work.
Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. Current state requirements may be found by going to http://vaww.va.gov/OHRM/T38Hybrid/.
May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria).
Grade Determinations:
In addition to meeting the basic requirements stated above, the following qualifications criteria must be met in determining the appropriate grade assignment of candidates.
GS-12 Social Worker Program Coordinator candidates must have the following:
Experience/Education: Applicants applying at the GS-12 grade level must have 1 year of experience, equivalent to the GS-11 grade level. Program Coordinator social workers have experience that demonstrates possession of advanced practice skills and judgment. Social workers are experts in their specialized area of practice. They may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty. Training or additional certs must be submitted with application.
AND
Licensure/Certification: Senior social workers must be licensed or certified by a state at the advanced independent practice level which included an advanced generalist or clinical examination, unless they are grandfathered by the state in which they are licensed to practice at the advanced practice level (except for licenses issued in California, which administers its own clinical examination for advanced practice) and they must be able to provide supervision for licensure. Advanced License must be submitted with application.

Preferred Experience:
Case Management experience is preferred, working with Veterans and Active Duty Service Members who have suffered a traumatic brain injury
Medical and mental health clinical experience needed for completion of bio-psychosocial assessments, implementation of treatment interventions, and determination of appropriate level of care.
Knowledge of community resources is required for disposition and discharge planning purposes.
Crisis intervention skills and the ability to remain calm in high pressured situations is mandatory to effectively problem solve and implement effective clinical interventions.

Physical Requirements: The physical demands of the work requires use of fingers; moderate walking and standing; near vision correctable at 13" to 16" to Jaeger 1 to 4; far vision correctable in on eye to 20/20 and to 20/40 in the other; hearing aid permitted; clear speech; emotional stability; moderate duty.
