The Pl Coordinator is aligned under the Quality Management service line. They function under the direction of the Deputy Chief of Quality Management. In this capacity, the Pl Coordinator is accountable for demonstrating leadership, experience and expertise in the development, implementation, and evaluation of performance improvement in one or more Service Lines, committees, and councils.
Duties
The Pl Coordinator must demonstrate leadership in delivering and improving holistic care through collaborative strategies with others. She or he Is responsible and accountable for the facilitation of quality improvement and assurance processes throughout the facility including community-based outpatient clinics.
The Pl Coordinator is assigned to provide oversight reviews of specific services and functions to determine if appropriate care and services are rendered across the patient health care continuum. This includes but is not limited to performance Improvement duties assigned by the supervisor along with the following:
Evaluating assigned program or service lines areas to evaluate if appropriate practices, processes are being utilized based on the standards, guidelines, and regulations of respective governing bodies.
Conducting clinical reviews on a continuing basis to interpret and evaluate complex data directly related to diagnosis, condition treatment and discharge plans.
Serving as an active member of assigned health care committees and/or Pl sub-councils responsible for evaluating outcomes, actions, accuracy and reports of assigned groups daily.
Analyzing the work or assigned groups, councils or committees and prepares a monthly summary of findings, results of analytical and trends. The monthly report will be provided lo the Chief of Pl as designated.
All Pl Coordinators are expected to provide cross-coverage in the absence of others. He or she will also serve as the Acting Pl Chief as needed.
Submitting graphical and narrative reports or outcomes, trends and relevant issues on a monthly basis or sooner if deemed necessary.
Profiling the outcomes of specified clinical and administrative performance measures of assigned providers as specified using an approved format. Information gathered will be submitted to the Performance Measurement Coordinator for consolidation on a weekly basis.
Engaging in the performance improvement process and assists assigned services in deploying performance improvement strategies, measuring related outcomes, and achieving organizational goals. The active engagement may include serving as a facilitator of teams, team leader or liaison between teams, groups, councils, or committees.
Serving as a consultant on both internal and external regulations, standards, and requirements such as TJC, CARF, CAP, SOARS, OSHA, OIG, etc. The Pl Coordinator ls responsible for understanding, deploying, and implementing the standards and guidelines of these internal and external bodies.
Reviewing, addressing, and reserving Issues identified from Occurrence Screening and other data tracking modules.
Using Microsoft Word (MS), QI Macros, Microsoft Excel and or other data programs as recommended, to discuss collect, track, trend, graphical display and represent and store data of assigned services and committees.
Other duties as assigned.

Qualifications
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements: The physical requirements of this position include lifting, prolonged standing and sitting, frequent bending, walking (distance), climbing, reaching (overhead, extensive/repetitive), pulling, pushing, repetitive motion and stooping. The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing legibly, keyboarding, reading and logical thinking, depth perception, hearing (aide permitted); near/far vision correctable to meet Driver's License requirements. The environment's pace can vary from steady to a sometimes-fast pace requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes-noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials.
