This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application.

Duties

The incumbent is a social worker whose duties and responsibilities are assigned to the care management of veterans in the Contract Nursing Home/Adult Day Care/State Veterans Home (CNH/ADC/SVH) Programs. Responsibilities include the management, coordination (admission, treatment planning, discharge planning, etc) and provision of social work services to veteran and their families in community settings. The incumbent will participate in regular multidisciplinary inspections of CNH/ADC facilities to ensure that veterans placed in these settings are receiving appropriate psychosocial services. Clinical Functions: Must have a high level of skill and expertise to establish and maintain effective therapeutic relationships with veterans in the CNH/ADC/SVH Program, as well as, with the facility staff and veteran's families. The incumbent is able to independently work with veterans and their families who are experiencing a wide range of complicated medical, psychiatric, emotional, behavioral, and psychosocial problems. Must independently complete thorough psychosocial assessments to determine the psychosocial functioning and needs of Veterans and/or their families. The incumbent must be able to utilize this assessment in facilitating the Veteran's maximum use of treatment for attainment of the highest level of independence that is possible and practicable and in developing appropriate discharge plans from the nursing home to the community. Must facilitate discharge planning through collaboration with veterans and their families, as well as, facility staff and interdisciplinary treatment team members when veterans in the CNH/ADC Program are identified as needing alternative placement or when veterans request return to independent community living. Must possess knowledge and ability to independently implement treatment modalities, provide counseling and/or supportive groups for Veterans and families. Must be available to provide consultation to other treatment team and staff members as well as residential care sponsors regarding psychosocial needs of Veterans and/or their families and the impact of the identified psychosocial problems on the Veteran's health care planning, compliance with treatment, discharge planning, and ongoing adjustment to the residential care setting. Must possess skills and knowledge to provide case management services to Veterans and their families throughout the continuum of care. The incumbent will be able to appropriately utilize principles of human growth and development over the life span and will be able to assist Veterans in coping with the loss and grief experiences from disability and terminal illness. Administrative Functions: The Incumbent... Is responsible for supporting the mission, policies, and procedures of the Department of Veterans Affairs, the Veterans Health Administration (VHA), the appropriate Veterans Integrated Service Network (VISN), and the facility. Is willing to serve on committees, work groups, and task forces at the facility as deemed appropriate by the Supervisor and Chief of Social Work Service Line. Is mindful of keeping their supervisor apprised of problems and recommended solutions to problems encountered in the incumbent's area of responsibility. Is responsible for furthering one's own professional growth through continuing education and for ensuring continuing education appropriate to the area of assignment. Is able to provide assistance with the orientation and learning of less experienced social workers and, if interested, to serve as a preceptor for social work students. Is able to demonstrate knowledge and skill in the use of software applications for drafting documents and data management as well as other computer systems in use by the VHA. Is able to maintain a level of productivity and quality consistent with the complexity of the assignment and consistent with Social Work standards, Joint Commission standards, and facility standards. Qualifications

Basic Requirements: Citizenship. Be a citizen of the United States. Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Education. Have a Master's degree in social work (MSW) from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the school of social work is fully accredited. Please note: A doctoral degree in social work may not be substituted for the master's degree in social work. Licensure. Licensed or certified by a state to independently practice social work at the master's degree level. English Language Proficiency. Social workers must be proficient in spoken and written English in accordance with VA Regulations. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Must pass a background check. Grade Requirements: GS-11 Social Worker Assignments. This is the full performance level for social workers. GS-11 social workers are licensed or certified to independently practice social work. They are assigned to all program areas, including inpatient or outpatient medicine, surgery, mental health, neurology, rehabilitation medicine, and geriatrics. Employees provide professional, independent social work services in the assigned area. Experience, Education and Licensure. Promotion to the GS-11 full performance level requires completion of a minimum of 1 year of post-MSW degree experience in the field of health care social work (VA or non-VA experience) and licensure or certification in a state at the independent practice level. OR, In addition to meeting basic requirements, a doctoral degree in social work from a school of social work may be substituted for the required 1 year of professional social work experience in a clinical setting. In addition, the candidate must demonstrate the KSAs below. Demonstrated Knowledge, Skills, and Abilities 1. Knowledge of community resources, how to make appropriate referrals to community and other governmental agencies for services, and ability to coordinate services. 2. Skill in independently conducting psychosocial assessments and treatment interventions to a wide variety of individuals from various socio-economic, cultural, ethnic, educational and other diversified backgrounds. 3. Knowledge of medical and mental health diagnoses, disabilities and treatment procedures (i.e. acute, chronic and traumatic illnesses/injuries, common medications and their effects/side effects, and medical terminology) to formulate a treatment plan. 4. Skill in independently implementing different treatment modalities in working with individuals, families, and groups who are experiencing a variety of psychiatric, medical, and social problems to achieve treatment goals. 5. Ability to provide consultation services to new social workers, social work graduate students, and other staff about the psychosocial needs of patients and the impact of psychosocial problems on health care and compliance with treatment. The actual grade at which an applicant may be selected for this vacancy is a GS-11. Physical Requirements: Physical demands of the work are generally minor. The work is primarily sedentary.
