The PACT Modernization Administrative Provider is responsible for the overall management of patients enrolled for healthcare services provided by the Tennessee Valley Health Care System, Patient Aligned Care Team (PACT). Responsible for clinic oversight, data review, chart audits, and educational tasks associated with clinic operations, and provides leadership and direction to support staff assigned to this clinic.

Duties

Is knowledgeable of and ensures compliance with a) VHA Handbooks and Directives, b) VISN policies, c) Medical Center Memorandum and d) Service Policies and Procedures pertinent to provider operations. 2. Is knowledgeable and capable for responding to Congressional inquiries. 3. Is knowledgeable and in compliance with accreditation standards, including IG, Joint Commission, and other applicable survey/oversight entities. 4. Is knowledgeable and in compliance with Safety and Environment of Care (EOC) standards and programs including the following: a. SDS Requirements b. The Joint Commission EOC Standards c. Disposal of Hazardous Wastes d. GEMS Program e. Facility Emergency Preparedness Plan f. Facility Safety Plan g. Infection Control Guidelines h. Management of RME 5. Has a working understanding and knowledge of available reports and data sources available through VistA, VSSC, DSS and other VHA data sources to facilitate management of all clinical areas within the facility. a. Has a working knowledge of Compass and Almanac reports. b. Has a working knowledge of PACT Composite Measures. c. Has a working knowledge of DSS reports. d. Has a working knowledge of applicable VSSC reports including Ambulatory Sensitive Conditions and the SAIL report. 6. Has knowledge of and is in compliance with conflict of interest and ethics policies related to business practices/associations, including contracting and procurement regulations. 7. Uses MS Word or comparable word processing software to execute several office automation functions, such as storing and retrieving electronic documents and files, activating printers, inserting and deleting text, formatting letters, reports and memoranda, and transmitting and receiving e-mail. Uses MS Excel or other comparable spreadsheet software to develop and change spreadsheets to graphically display data. Uses MS PowerPoint or other comparable graphics software to develop presentation materials. Uses the Veterans Health Information & Technology Architecture (VistA) and Computerized Patient Record System (CPRS) to access information in the Medical Center Computer System. 8. Protects printed and electronic files containing sensitive data in accordance with the provisions of the Privacy Act of 1974 and other applicable laws Federal Regulations, VA statures and policy and VHA policy. Protects data from unauthorized release, or from loss, alteration, or unauthorized deletion and follows applicable regulations and instructions regarding access to computerized files and release of access codes, etc. 9. Has knowledge of and complies with the Medical Center Bylaws and the Rules and Regulations of the Medical Staff. SCOPE OF CLINICAL DUTIES. The PACT Modernization Administrative Provider shall be proficient in all aspects of general practice. Specifically, the provider should possess the expertise and competency to perform the following: 1. Fully implements the processes and systems relative to the VHA Primary Care Medical Home(PCMH) healthcare system design 2. Coordinates care for a panel of outpatients, as assigned by TVHS. 3. Evaluates and treats acute and chronic health problems for scheduled and unscheduled patients in the clinic setting. 4. Completes and documents history and physical exams. 5. Provides appropriate documentation to minimally include history, physical exam :findings, diagnosis, definitive care plan and disposition of the patient. 6. Ensures that patients are appropriately closed-out, including disposition that clearly delineates the next steps of care. 7. Ensures that each patient is provided a list of current medications and an appointment list at close-out. 8. Performs only procedures contained within their approved privileges. 9. Prescribes medications as appropriate. 10. Orders routine laboratory, radiological and diagnostic tests and provides initial interpretation of results. * Many other duties as assigned/outlined by Functional Statement Work Schedule: .5 FTE - 20 hours per week / Monday - Friday 8a - 4:30p

Qualifications

To qualify for this position, you must meet the basic requirements as well as any additional requirements (if applicable) listed in the job announcement. Applicants pending the completion of training or license requirements may be referred and tentatively selected but may not be hired until all requirements are met. Currently employed physician(s) in VA who met the requirements for appointment under the previous qualification standard at the time of their initial appointment are deemed to have met the basic requirements of the occupation. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Residency Training: Physicians must have completed residency training, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. (NOTE: VA physicians involved in academic training programs may be required to be board certified for faculty status.) Approved residencies are: (1) Those approved by the Accreditation Council for Graduate Medical Education (ACGME), b) OR [(2) Those approved by the American Osteopathic Association (AOA),OR (3) Other residencies (non-US residency training programs followed by a minimum of five years of verified practice in the United States), which the local Medical Staff Executive Committee deems to have provided the applicant with appropriate professional training and believes has exposed the physician to an appropriate range of patient care experiences. Residents currently enrolled in ACGME/AOA accredited residency training programs and who would otherwise meet the basic requirements for appointment are eligible to be appointed as "Physician Resident Providers" (PRPs). PRPs must be fully licensed physicians (i.e., not a training license) and may only be appointed on an intermittent or fee-basis. PRPs are not considered independent practitioners and will not be privileged; rather, they are to have a "scope of practice" that allows them to perform certain restricted duties under supervision. Additionally, surgery residents in gap years may also be appointed as PRPs. Proficiency in spoken and written English. Preferred Experience: Two years experience in Primary/Ambulatory Care. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-2 Physician Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: The work is largely office based and in a sedentary setting. No manual labor is required above ordinary situations. Incumbent must be capable of using a computer given that all clinical charting is electronic.

Education

Degree of Doctor of Medicine or an equivalent degree resulting from a course of education in allopathic medicine or osteopathic medicine. The degree must have been obtained from an institution whose accreditation was in place for the year in which the course of study was completed. Approved schools are:
  1. Schools of medicine accredited by the Liaison Committee on Medical Education (LCME) for the year in which the degree was granted, or
  2. Schools of osteopathic medicine approved by the Commission on Osteopathic College Accreditation of the American Osteopathic Association for the year in which the degree was granted.
  3. For foreign medical graduates not covered in (1) or (2) above, facility officials must verify with the Educational Commission for Foreign Medical Graduates (ECFMG) that the applicant has met requirements for certification, and must obtain a copy of the ECFMG certificate, if claimed by the applicant. [If the applicant does not claim an ECFMG certificate, facility officials must still confirm that the medical school meets (or met) ECFMG eligibility requirements for the year the candidate graduated.]
NOTE: The Under Secretary of Health or designee in the VHA Central Office may approve the appointment under authority of 38 U.S.C. 7405 of a physician graduate of a school of medicine not covered above if the candidate is to be assigned to a research, academic, or administrative position with no patient care responsibilities. The appointment will be made only in exceptional circumstances where the candidate's credentials clearly demonstrate high professional attainment or expertise in the specialty area.
