The Senior Social Worker is directly responsible for the provision of clinical services to Veterans assigned to the IDDT program and other duties as assigned. This position opening is assigned to the Addiction Recovery Services in the Mental Health Patient Service Line of the Minneapolis, Minnesota Veterans Affairs class 1A (High Complexity) Medical Center. Throughout this functional statement, the incumbent will be referred to as Senior Social Worker.

Major duties include, but are not limited to:
The Senior Social Worker works independently to provide services to Veterans with serious mental illnesses and active substance use disorders. The Senior Social Worker is part of the larger Addiction Recovery Services interdisciplinary team and is responsible for overall management of the IDDT caseload. The Senior Social Worker provides a full range of intensive case management services to Veterans in the IDDT programs, including assessments, consultations and interventions. The Senior Social Worker assists Veterans in achieving their self-determined recovery goals by promoting independence, community inclusion, and hope as well as addressing both their issues with substance use and severe mental illness. The Senior Social Worker performs a wide range of tasks to assist Veterans in regaining independence within the community, developing meaningful life roles, and gaining mastery over their own recovery process. The Senior Social Worker develops extensive knowledge of IDDT model to ensure appropriate programmatic delivery and to achieve high fidelity to the model.
The Senior Social Worker provides direct service to patients/families on an outpatient basis, applying knowledge of integrated treatment, casework, group work and family treatment methods in order to help patients maintain community living and obviate the need for hospitalization and other acute crisis services. This requires active engagement in the community and comfort with outreach case management. The Senior Social Worker coordinates community-based services, including information and referral for additional services from VA, other government and community agency programs.
The Senior Social Worker documents case activity in accordance with Joint Commission, CARF, VHA and facility policies. The Senior Social Worker documents in patients' medical records in accordance with Social Work Service requirements in order to substantiate activity with the patient. The Senior Social Worker conforms to legal and ethical codes of VA, Joint Commission and all appropriate professional Social Work sanctioning organizations. The Senior Social Worker protects electronic data from unauthorized release or from loss or alteration. Has working knowledge and skill in the use of word-processing, data management and other computer systems.
The Senior Social Worker completes substance use assessments, psychosocial intakes, and treatment plans in a timely manner. The Senior Social Worker completes intake assessments in order to obtain data base and formulate an initial assessment of problem areas, identifying causes contributing to the problem, and determines what can be changed to improve the circumstances.
The Senior Social Worker maintains cooperative and collaborative interdisciplinary working relationships in order to assist with the development and execution of the treatment plan. The Senior Social Worker maintains cooperative and collaborative working relationships with staff of community agencies in order to obtain needed resources for patients/families.
The Senior Social Worker identifies professional training needs and participates in continuing education in order to enhance skills and knowledge base.
The Senior Social Worker maintains a high level of work productivity in order to provide the most effective Social Work delivery service possible.
The Senior Social Worker establishes and maintains contact with a network of resources throughout Hennepin County, Ramsey County, and surrounding areas. Compiles information about application criteria, accessibility and eligibility criteria for such resources.
The Senior Social Worker participates in program evaluation pertinent to the clinical effectiveness of the program including tracking patient symptoms, hospitalizations, and utilization of acute crisis services. The Senior Social Worker monitors performance measures and metrics, and establishes action plans to improve performance when indicated. The Senior Social Worker analyses these data to share with leadership and community stakeholders. The Senior Social Worker plans and delivers facility and community education regarding Veterans with serious mental illness and co-occurring diagnoses issues and programming.
The Senior Social Worker has working knowledge and experience in use of medical and mental health diagnoses, disabilities and treatment procedures. This includes acute, chronic and traumatic illness/injuries, common medications and their effects and medical terminology.
Work Schedule: Monday through Friday; 08:00 to 16:30

Qualifications

Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
Education: Must possess a Master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE).
Licensure: Must be licensed or certified by a state to independently practice social work at the master's degree level.
English Language Proficiency: Candidates must be proficient in written and spoken English to be appointed.
Grade Determinations: Senior Social Worker GS-0185-12
Experience/Education:
The candidate must have at least two years of experience post advanced practice clinical licensure and should be in a specialized area of social work practice of which, one year must be equivalent to the GS-11 grade level. Senior social workers have experience that demonstrates possession of advanced practice skills and judgment. Senior social workers are experts in their specialized area of practice. Senior social workers may have certification or other post-masters training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship or equivalent supervised professional experience in a specialty.
Licensure/Certification:
Applicants must meet the requirements for providing supervision in the state where they are licensed at the advanced practice level and should list these qualifications within the assessment narrative and their resume. Documentation is required if part of your state's requirements for providing supervision for licensure is beyond your licensure only (example: certification of continuing education credits).
Demonstrated Knowledge, Skills, and Abilities:
In addition to the experience above, candidates must demonstrate all of the following KSAs

(a) Skill in a range of specialized interventions and treatment modalities used in specialty treatment programs or with special patient populations. This includes individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management.
(b) Ability to incorporate complex multiple causation in differential diagnosis and treatment within approved clinical privileges or scope of practice.
(c) Knowledge in developing and implementing methods for measuring effectiveness of social work practice and services in the specialty area, utilizing outcome evaluations to improve treatment services and to design system changes.
(d) Ability to provide specialized consultation to colleagues and students on the psychosocial treatment of patients in the service delivery area, as well as role modeling effective social work practice skills.
(e) Ability to expand clinical knowledge in the social work profession, and to write policies, procedures, and/or practice guidelines pertaining to the service delivery area.
