The Permanent Charge Nurse (PCN) Coordinator Registered Nurse reports to the inpatient Nurse Manager or designee of the 3B/CLC-5 Inpatient Unit at the Bath VAMC. This unit serves Veterans who may need acute care or extended care, but for a limited time. The primary role of this position is serving as a Care Coordinator and day to day clinical leader on the unit.

Duties

Duties may include however are not limited to the following: The PCN Coordinator Registered Nurse functions as a day-to-day clinical leader, directing and guiding the nursing staff to assure safe high-quality care is rendered. This includes the management, supervision and evaluation of care delivered by nursing and other allied health care staff. As a registered nurse the permanent charge nurse is also responsible for contributing to the establishment and implementation of evidence-based nursing standards of practice. Creates a positive work environment which enhances care delivery and professional development of clinically competent staff. Promotes, creates, demonstrates, and ensures a culturally diverse workforce in which the values and needs of all individuals are respected. The PCN coordinator interprets the philosophy and goals of nursing service to others in the setting; Functions within established guidelines and demonstrates the ability to work effectively and develop sound relationships with patients, their caregivers, and other professional and supportive personnel who provide patient care. The PCN coordinator works in close coordination and collaboration with the nurse manager. General Description Assigned duties include: 1. Assist in the implementation of new programs, policies, and equipment within the unit, as well as competency development and verification. 2. Guide the utilization of information resources, such as CLC and medical center policies and procedures, nursing shared drive, nursing SharePoint, and nursing procedures. 3. Directs RN team leaders and other team members in the provision of patient care. 4. Makes out staff daily assignments for all three shifts at least 24 hours in advance, ensuring consistency with staffing. 5. Assist in assignments of patient care, decision making, role modeling, mentoring, and educating staff throughout the workday. 6. Attend CLC screening meetings Monday through Friday in lieu of nurse manager, bringing forth the nursing plan of care and pertinent patient care issues to the screening team. 7. Participates as a CLC representative on committees, projects, or improvement teams as requested by the nurse manager. Assist with patient care as needed and take a patient assignment as the need arises. 8. Assists with nurse manager on projects and assist in quality metrics for CLC compare, nurse sensitive indicators (including, but not limited to, Falls Prevention, Oral Care, Infections Prevention, Medication error prevention, Safe patient Handling, Skin/Safety), HCAPS, and other outcome measures relative to 3B/CLC-5. 9. Attends IDTs, daily rounding on the acute inpatient unit, Leads huddles, Wound Rounds, Tracks MDS and implements and distributes a list to the care team for MDS due, and other patient care needs that arise. 10. Completes daily nurse staffing sheets ensuring accurate numbers of staff and staff assignments to code team and rooms are completed per policy. 11. Reviews new nursing orders ensures that are implemented and followed. 12. Assigns new admissions to rooms and make sure rooms are prepped prior to admissions based on screening need for infection prevention cohorting of patients. 13. Acts as point of contact for medical providers needing tasks completed in an urgent and timely manner. 14. Ensures patients attend scheduled appointments and any necessary follow up appointments are scheduled. 15. Directs staff in emergency situations as needed. Qualifications

Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English. Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing. Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia. Preferred Experience: Incumbent should have experience as lead or charge nurse in acute care inpatient unit and long term care, short-stay rehabilitation unit, understand the need to mentor and enhance professional development of their team, and work closely with a multidisciplinary team. Has skills necessary to support the various team roles and understand how and when to delegate duties and tasks according to available resources. Knowledgeable and willing to lead the team in quality performance measures for the inpatient and extended care settings to use evidence based practices to achieve optimal patient outcomes and experience. Promotes a positive work and care environment. Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade: Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required. Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience. Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience. Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required. Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience. Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required. Reference: VA Regulations, specifically VA Handbook 5005, Part II, Appendix G-6 Nurse Qualification Standard. This can be found in the local Human Resources Office. Physical Requirements: A pre employment physical evaluation will be required if selected.
