This Social Worker (Program Coordinator) position is in Mental Health & Behavioral Science Services at the VA Providence Healthcare System, located at the Providence location. This position is a full-time temporary at 40 hours per week.

Duties

The COMPACT Act Coordinator position is located within the Mental Health & Behavioral Science Service, in the Acute Care Section. The Veterans Comprehensive Prevention, Access to Care, and Treatment Act of 2020 (COMPACT) Act, Section 201 expands the Department of Veterans Affairs (VA)'s ability to furnish and pay for treatment of eligible individuals in acute suicidal crisis. The Social Worker Program Coordinator serves as the COMPACT Act Coordinator and has responsibility as the primary navigator for Veterans receiving care under COMPACT Act. Major duties include but are not limited to: Coordinates transitions in care for Veterans receiving acute suicide care under COMPACT Act in collaboration with assigned care coordinators at each step of the Veteran journey. Develops and monitors administrative and clinical plans for Veterans receiving COMPACT Act services. Oversees daily operations of the program and provides reports and metrics to facility, VISN and VACO as requested. Captures and analyses local data trends to inform program development and improvement. Provide education and training on COMPACT Act to internal and external stakeholders as the facility subject matter expert. Reviews and develops policies, standard operating procedures, and practice guidelines for the program. Collaborates with interprofessional teams within the VA healthcare system and community to navigate complex care coordination. Coordinates with local Patient Advocate Tracking System - Replacement (PATS-R) COMPACT clinical review teams and local facility Office of Integrated Veteran Care (IVC) Care Coordinators. Provides clinical social work assessment, intervention, treatment and follow-up, using a social determinant of health framework. Assists with navigation between varying levels of care and conducts comprehensive transition planning. This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Social Worker - Program Coordinator In addition to the basic requirements, applicants must meet the below: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Familiarity with VA system of care as well as local healthcare systems outside of the VA system References: VA Handbook 5005/120 Part II Appendix G39 The full performance level of this vacancy is GS-12 Physical Requirements: This position requires potentially long periods of walking, standing, and sitting. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don and remove protective clothing in isolation situations. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and able to shift priorities based on patient needs.
