This Social Worker (Program Coordinator) position is in Mental Health & Behavioral Science Services at the VA Providence Healthcare System, located at the Providence location. This position is a full-time at 40 hours per week.

Duties

The HUD-VASH Social Work Program Coordinator position, located within Mental Health & Behavioral Science Service, in the Housing and Urban Development - VA Supportive Housing (HUD-VASH) unit of the Veterans Housing Support Program (previously Healthcare for Homeless Veterans), serves on the interdisciplinary geriatric team. This Social Worker is the recognized expert in delivery of community-based geriatric social work services to the most complex medical, surgical, and mental health patients. Veterans in this program are either acutely experiencing an episode of homelessness, have previously experienced homelessness, or are housed but at acute risk for loss of housing. Major duties include but are not limited to: Coordinates clinical and psychosocial geriatric mental health services. Manages, plans, implements, monitors, and evaluates the delivery of services within the HUD-VASH geriatric team. Monitors the efficiency, effectiveness, timeliness, and quality of services offered. Performs psychosocial assessments and makes diagnosis, provides case management services, and makes referrals to internal and community resources. Assesses for elder Abuse, neglect, and exploitation and makes appropriate referrals. Collaborates with the Geriatric PACT and sub-specialty SW to provide seamless care. Assists less experienced and less knowledgeable staff with technical expertise and guidance for patients with unusual or complicated medical and mental health conditions, risk factors, and ethical or legal circumstances. Performs program analysis, development, review, research, and design. Transports Veterans in government vehicles as clinically indicated. This position is eligible for the Education Debt Reduction Program (EDRP), a student loan payment reimbursement program. You must meet specific individual eligibility requirements in accordance with VHA policy and submit your EDRP application within four months of appointment. Approval, award amount (up to $200,000) and eligibility period (one to five years) are determined by the VHA Education Loan Repayment Services program office after complete review of the EDRP application. Basic Requirements: United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. English Language Proficiency: Licensed practical or vocational nurses appointed to direct patient care positions must be proficient in [both] spoken and written English as required by 38 U.S.C. 7402(d), and 7407(d). Education: Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work. Verification of the degree can be made by going to http://www.cswe.org/Accreditation to verify that the social work degree meets the accreditation standards for a Master of Social Work. Licensure: Persons hired or reassigned to social worker positions in the GS-0185 series in VHA must be licensed or certified by a state to independently practice social work at the master's degree level. May qualify based on being covered by the Grandfathering Provision as described in the VA Qualification Standard for this occupation (only applicable to current VHA employees who are in this occupation and meet the criteria). Grade Determinations: GS-12 Social Worker - Program Coordinator In addition to the basic requirements, applicants must meet the below: Experience and Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level and must be able to provide supervision for licensure. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs: Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring. Knowledge and ability to write policies, procedures, and/or practice guidelines for the program. Ability to supervise multidisciplinary staff assigned to the program. Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s). Ability to provide training, orientation, and guidance within clinical practice. Preferred Experience: Experience with older adults and experience working in community based settings. References: VA Handbook 5005/120 Part II Appendix G39 The full performance level of this vacancy is a GS-12. Physical Requirements: This position requires visual acuity, keen hearing, clear distinctive speech, and manual dexterity. This position requires potentially long periods of continued walking, standing, kneeling, sitting, bending, pulling, and pushing. Transferring patients and objects may be required. The incumbent may be exposed to infected patients and contaminated materials and may be required to don and remove protective clothing in isolation situations or invasive procedures. The incumbent may be exposed to patients who are combative secondary to delirium, dementia, or psychiatric disorders. The incumbent must be a mature, flexible, sensible individual capable of working effectively in stressful situations and able to shift priorities based on patient needs.
