The incumbent is responsible for the management, coordination and provision of social work services to veterans in the Healthcare for Homeless Veterans Grant & Per Diem program of the VA St. Louis Health Care System. The duty station is at the Hope Recovery Center but may shift to other locations based on veteran care needs.

Duties

The social worker provides consultation and guidance to colleagues and staff. They role model effective social work practice skills, teach or provide orientation to less experienced social workers, develop innovations in practice interventions, and provide clinical supervision for social work licensure or certification. Incumbent is an expert in psychosocial and psychiatric assessment, within approved clinical privileges or scope of practice. The incumbent utilizes various treatment modalities, including evidence-based practices for individual, family and group work. The incumbent acts as an advocate for homeless veterans through outreach activities in an effort to alleviate or remove problems that interfere with homeless veterans' ability to achieve independence. For direct practice and program development, the incumbent is guided by Recovery Model principles, as defined by SAMHSA, which include the following: recovery emerges from hope and is driven by self-determination; is supported by addressing trauma; is based on respect; involves individual, family, and community strengths and responsibilities; has many different paths; is holistic; supported by peers and other relationships; and is culturally-based and influenced. The incumbent has advanced and expert skill in a range of specialized interventions and treatment modalities used with special patient populations. This includes the use of evidence-based practice for individual, group, and/or family counseling or psychotherapy and advanced level psychosocial and/or case management interventions used in the treatment of veterans with complex conditions (e.g. mental health and substance use disorders, traumatic brain injuries, post-traumatic stress disorder, homelessness, compromised and/or complex health conditions, etc.). The incumbent provides services and oversight of the VA Grant & Per Diem (GPD) funded programs as defined in the GPD rules, regulations, and directives. This may include verification of veteran status and eligibility for participation in the GPD Program, maintaining admission and discharge dates of participants for billing purposes, reviewing and approving/disapproving readmissions to and extensions of care collecting and submitting participant information as outlined by NEPEC evaluations procedures, and tracking scheduling for and attendance as required VA Mental Health/Substance Abuse, and/or medical appointments. Incumbent is fiscally responsible for budgetary oversight including reviewing all bed days of care for each program and is responsible for accurately tracking bed days according to designated per diem rate for each GPD provider. Responsible to verify all provider payment vouchers and submit them for payment to GPD Office in Tampa, Florida and Austin, Texas. The incumbent coordinate and participants in required program site inspections utilizing the GPD Program rules, regulations, and directives as guidance and forwards completed inspection packages to the appropriate offices. When necessary, the incumbent identify inspection deficiencies and establishes approved methods to monitor corrective actions. This process may include development of timelines for consequences as a result of inaction or insufficient action related to deficiencies. Incumbent coordinates with the Medical Center Director, if needed, and appropriate VA supervisors to create the multidisciplinary Inspection Team for annual inspections, new program inspections, and spot inspections for assigned community agencies. Develops and implements methods for measuring effectiveness of social work practice and services in specialty area, utilizing outcome evaluations to improve treatment services. Qualifications

Experience/Education. One year of experience equivalent to the GS-11 grade level. Experience must demonstrate possession of advanced practice skills and judgment, demonstrating progressively more professional competency. Candidate may have certification or other post-master's degree training from a nationally recognized professional organization or university that includes a defined curriculum/course of study and internship, or equivalent supervised professional experience. Licensure/Certification. Individuals assigned as social worker program coordinator must be licensed or certified at the advanced practice level, and must be able to provide supervision for licensure. Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met. Demonstrated Knowledge, Skills, and Abilities. In addition to the experience above, the candidate must demonstrate all of the following KSAs.(a) Knowledge of program coordination and administration which includes consultation, negotiation, and monitoring.(b) Knowledge and ability to write policies, procedures, and/or practice guidelines for the program.(c) Ability to supervise multidisciplinary staff assigned to the program.(d) Skill in organizing work, setting priorities, meeting multiple deadlines, and evaluating assigned program area(s).(e) Ability to provide training, orientation, and guidance within clinical practice. Assignment. For all assignments above the full performance level, the higher-level duties must consist of significant scope, complexity (difficulty), and variety and be performed by the incumbent at least 25% of the time. Program coordinators at the GS 12grade level are administratively responsible for a clinical program providing treatment to Veterans in a major specialty area, such as but not limited to: Spinal Cord Injury, Homeless Continuum Veteran Program, Hospice and Palliative Care Program, Suicide Prevention Program, Veterans Justice Outreach, Caregiver Support Program and Community Nursing Home Program. The program coordinator may be the sole practitioner in this specialty at the facility and typically provide direct patient care services in the program area. The program coordinator oversees the daily operation of the program, develop policies and procedures for program operation, and prepare reports and statistics for facility, VISN, and national use. They may be responsible for the program's budget, developing and monitoring staff compliance with practice, standards and guidelines on documentation, workload, data entry, ethical practice and service delivery. The program coordinator provides analysis and evaluation of clinical program data and computerized programs to identify system-wide trends and needs to enhance the quality of service. They may be responsible for, or contribute to, the program's resource and fiscal management, monitoring control points developing the annual budget, operating within that budget, and accounting for appropriated funds. The program coordinator is administratively responsible for the clinical programming and prepares reports and statistics for facility, VISN, and national use. They provide leadership, direction, orientation, coaching, in-service training, staff development, and continuing education programs for assigned staff . They initiate and conduct a variety of program or service audits and complete designated clinical practice audits and reports, including productivity assessments. They oversee program operations and evaluations, identifying areas for improvement, gathering relevant data, assessing the data, developing and implementing ideas for improvement and evaluating efficacy of improvement efforts. If the incumbent is LCSW or equivalent, must be ale to provide clinical supervision for Social Workers seeking the licensure. Must be verified by the licensing board of the state where the license is awarded.

Education

Education. Have a master's degree in social work from a school of social work fully accredited by the Council on Social Work Education (CSWE). Graduates of schools of social work that are in candidacy status do not meet this requirement until the School of Social Work is fully accredited. A doctoral degree in social work may not be substituted for the master's degree in social work.
