The primary purpose of the position is to serve as a Community Care Coordinator. It is a foundational role within the continuum of patient care, working collaboratively with the patient care teams as well as a variety of specialty health care providers. This includes registered nurse care managers, primary care and/or specialty care providers, pharmacists, social workers, dieticians, behavioral health specialists, etc. to provide a robust interdisciplinary approach to care.
Duties
Major duties:
Provides administrative support to clinicians and other administrative and professional staff throughout the medical staff office for purposes of coordinating Non-VA medical care for veterans when inpatient and outpatient, and their VA provider has deemed such care necessary.
Responsible for generating authorizations for pre-authorized medical care, while making eligibility determinations for the various programs in Community Care.
Works closely with utilization review to ensure proper authorizations are issued and appropriate correspondence is sent to vendors.
Ensures all necessary components and documentation are distributed to appropriate services at the private institution where service will be provided.
Responsible for the coordination of all administrative information between the professional staff and the patient.
Responsible for maintaining a current directory of contact information, to include name, address, phone, and fax numbers of institution, as well as the contact person.
Provides advisory and technical assistance to veterans and staff in answering questions.
Actively involved in the continuous quality improvement of patient care.
Explains the VA's mandate to collect insurance information to veterans, their families, and other eligible patients.
Reviews and determines veteran's eligibility for specialty programs in the Community Care program (CC) and appropriately processes their authorizations.
Work Schedule: Full time, Monday - Friday, 8:00am - 4:30pm

Qualifications
To qualify for this position, applicants must meet all requirements by the closing date of this announcement, 11/14/2023.
You may qualify based on your experience as described below:
Specialized Experience: You must have one year of specialized experience equivalent to at least the next lower grade GS 5 in the normal line of progression for the occupation in the organization. Examples of specialized experience would typically include, but are not limited to: Outpatient Scheduling Processes and Procedures, Priority Scheduling for Outpatient Medical Services and Inpatient Hospital Care; general knowledge of other fee functions such as fee basis notifications, authorizations, appeals and other fee regulations; basic medical terminology; Working knowledge of personal computers and use of software: Windows, Microsoft Word, Access, Excel, Power Point, VISTA Computer System, EHR, and VATAS; Knowledge of spelling, arrangement, grammar and required formats for correspondence, reports, and other office documents; Advanced skill in compiling readily available data from prescribed sources; knowledge of the organization to search records, files, or electronic data banks to locate information needed, and to verify information given.; thorough working knowledge of clinic processes and services to function in and interact with various clinics throughout the hospital.
Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religions; spiritual; community; student; social). Volunteer work helps build critical competencies, knowledge, and skills and can provide valuable training and experience that translates directly to paid employment. You will receive credit for all qualifying experience, including volunteer experience.
Note: A full year of work is considered to be 35-40 hours of work per week. Part-time experience will be credited on the basis of time actually spent in appropriate activities. Applicants wishing to receive credit for such experience must indicate clearly the nature of their duties and responsibilities in each position and the number of hours a week spent in such employment.
Physical Requirements: The work is primarily sedentary at a desk and workstation area. However, there may be some walking, standing, bending, and carrying of light items like papers, reports, files, or possibly moving computer equipment. May stand to greet visitors and escort people to meeting areas. No special physical demands are required to perform the work.
