The Louis A Johnson VA Medical Center is seeking a Diabetes Education Nurse to assist in teaching self-management techniques to patients diagnosed with diabetes. Provides leadership in supporting the coordination and execution of best practice in diabetes care across services. Concentrates on helping clients adjust behaviors relating to eating well, staying physically active, monitoring their blood glucose levels, taking medication, coping with the stress of diabetes and reducing risks.
Duties
Responsibilities of the RN include but are not limited to the following:
Collaborates with the Clinical team and assists the Certified Diabetes Educator with clinic coordination.
Advising patients as to long-term complications.
Assist patients in monitoring blood glucose for the purpose of medication control and adjustment for both insulin and noninsulin dependent diabetics. Coordinate the development and implementation of patient diabetes care concept. Assess, monitor and implement treatment plans for patients at various stages of the disease and focus on prevention of complications and management of stable diabetes.
Use individual and aggregate needs assessments, standards of professional practice, and strategic plans in the process of program development. Lead, facilitate, and evaluate system changes that support meeting performance measure goals and thus, effective patient outcomes.
Serve as a role model and clinical leader to nursing staff for clinical excellence, research, and best practices.
Collaborate and consult with multidisciplinary teams in planning, directing, and implementing a patient-centered environment.
Act as a consultant in patient diabetes care principles and performance measures.
Effectively use group process, interpersonal, and communication skills to facilitate win-win outcomes in a positive and productive working environment; Use knowledge, expertise, principles or models, and current research to plan, implement, and evaluate organizational progress towards accomplishing patient diabetes care initiatives. Use data trends and patterns to indicate need for change.
Evaluate structure, process, outcomes and resource aspects to meet patient diabetes care goals.
The Diabetic Care Nurse will communicate needs for resources, as necessary; Utilizes best practices and benchmarking to establish realistic goals for patient diabetes care initiatives. Establishes and measures progress toward goals.
He/she is also responsible for the continuous evaluation and improvement of diabetic education and for employee education and competency validation
The Louis A Johnson VA Medical Center is seeking a Registered Nurse Diabetes Education to assist us in teaching self-management techniques to patients diagnosed with diabetes.
Patient population will include both inpatients and outpatients.
The Diabetes RN is a clinician, educator and patient advocate in the specialty of diabetic nursing practice.
He/she provides in/direct nursing care to patients who are seen in the Diabetics Clinic at the LAJVAMC, patients admitted to the inpatient wards, and may include occasional travel to the Outpatient CBOC Clinics.
He/she collaborates with Case Managers, nursing staff, and Medical teams to provide education, teaching of classes, meter conversions, chart reviews, telephone calls, medication instruction etc and ensure appropriate testing and follow-up.
He/she plays an important role in Performance Measures and collects/reports this data to leadership.

Qualifications
Applicants pending the completion of educational or certification/licensure requirements may be referred and tentatively selected but may not be hired until all requirements are met.
Basic Requirements:
United States Citizenship: Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy.
English Language Proficiency. In accordance with 38 U.S.C. 7402(d), no person shall serve in direct patient care positions unless they are proficient in basic written and spoken English.
Graduate of a school of professional nursing approved by one of the following accrediting bodies at the time the program was completed by the applicant: The Accreditation Commission for Education in Nursing (ACEN) or The Commission on Collegiate Nursing Education (CCNE). The approving official may authorize a waiver of the requirement for ACEN or CCNE accreditation of any degree in nursing provided the college or university has regional accreditation from an accrediting body recognized by the Department of Education at the time of the candidate's graduation and the composite qualifications of the applicant warrant such consideration. In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. (Reference VA Handbook 5005, Appendix G6) OR In cases of graduates of foreign schools of professional nursing, possession of a current, full, active and unrestricted registration will meet the requirement for graduation from an approved school of professional nursing.
Current, full, active, and unrestricted registration as a graduate professional nurse in a State, Territory or Commonwealth (i.e., Puerto Rico) of the United States, or the District of Columbia.
Preferred Experience:
Minimum of 3 years of successful nursing experience, with additional experience in Diabetes education/training.
Certified Diabetes Care and Education Specialist Certification preferred at the time of appointment or must be obtained within two years of appointment.
Grade Determinations: The following criteria must be met in determining the grade assignment of candidates, and if appropriate, the level within a grade:
Nurse I Level I - An Associate Degree (ADN) or Diploma in Nursing, with no additional nursing practice/experience required.
Nurse I Level II - An ADN or Diploma in Nursing and approximately 1 year of nursing practice/experience; OR an ADN or Diploma in Nursing and a bachelor's degree in a related field with no additional nursing practice/experience; OR a Bachelor's of Science in Nursing (BSN) with no additional nursing practice/experience.
Nurse I Level III - An ADN or Diploma in Nursing and approximately 2-3 years of nursing practice/experience; OR an ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 1-2 years of nursing practice/experience; OR a BSN with approximately 1-2 years of nursing practice/experience; OR a Master's degree in nursing (MSN) or related field with a BSN and no additional nursing practice/experience.
Nurse II - A BSN with approximately 2-3 years of nursing practice/experience; OR ADN or Diploma in Nursing and a Bachelor's degree in a related field and approximately 2-3 years of nursing practice/experience; OR a Master's degree in nursing or related field with a BSN and approximately 1-2 years of nursing practice/experience; OR a Doctoral degree in nursing or meets basic requirements for appointment and has doctoral degree in a related field with no additional nursing practice/experience required.
Nurse III - Master's degree in nursing or related field with BSN and approximately 2-3 years of nursing practice/experience; OR a Doctoral degree and approximately 2-3 years of nursing practice/experience.
Note regarding MSN degrees: If your MSN was obtained via a Bridge Program that qualifies for professional nursing registration, a BSN is not required.

Physical Requirements:
The work requires regular and recurring bending, lifting, stooping, stretching, lifting and repositioning patients or similar activities. The incumbent will be required to lift up to 50 pounds occasionally and up to 35 pounds frequently. The incumbent will be required to handle equipment and must be knowledgeable of an able to use safe lifting techniques. The use of correct body mechanics required. The mental/sensory requirements include recall, reasoning, problem-solving, hearing (aid permitted), speaking clearly, writing legibly, keyboarding, reading, and logical thinking. Also, ability for rapid mental and muscular coordination simultaneously, near vision correctable at 13" to 16". The environment's pace can vary from steady to a sometimes fast pace, requiring handling of multiple priorities, frequent, sometimes intense customer interactions, and the ability to adapt to frequent changes in a sometimes noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials (1-2 hours at a time).
