Serves as a clinical and outreach pharmacist providing direct pharmaceutical care for patients within the Whiteriver Service Unit area. This position is a Full Time Position, with 50% of time located in the Pharmacy Department and 50% of time located in the Preventative Medicine Department of the Whiteriver Hospital, Whiteriver, AZ.

Duties

Performs home visits and conducts field work related to medication dispensing, education, medication reconciliation. Conducts hospital authorized outpatient pharmacy clinics for examination, diagnosis and treatment of specialized patients. Takes the appropriate health history, follows and adjusts the developed treatment plan and monitors the patient's outcome to the prescribed treatment plan. Document all clinical activities, update problem lists, and update medication allergy information in the medical record. Interview patients to ascertain significant past medical history and medication history. -Evaluate medication regimens for appropriateness, drug interactions, therapeutic duplications, non-compliance and adverse drug events. Serve as a therapeutic Consultant to the medical staff. Accurate filling and dispensing of prescription and over-the-counter drugs on orders of physician, optometrist, physician assistant, family nurse practitioner, and dentist. Dispensing over-the-counter drugs without the patient seeing the doctor (Pharmacy Primary Care) to treat minor ailments. Provide formal and informal education and in-services to medical staff, pharmacy, and nursing staff as requested. Provide training for pharmacy residents, pharmacy students and medical students. Provide education to patients and their families about medications, common disease states and information on health promotion. Immunize and educate out in the field at various off site locations both on and off the reservation to help improve vaccination rates and raise awareness.

Qualifications

To qualify for this position, your resume must state sufficient experience and/or education, to perform the duties of the specific position for which you are applying. Experience refers to paid and unpaid experience, including volunteer work done through National Service programs (e.g., Peace Corps, AmeriCorps) and other organizations (e.g., professional; philanthropic; religious; spiritual; community; social). You will receive credit for all qualifying experience, including volunteer and part time experience. You must clearly identify the duties and responsibilities in each position held and the total number of hours per week. BASIC REQUIREMENT - Education Graduated from a Pharmacy educational program, accredited by the Accreditation Council for Pharmacy Education (ACPE) or its predecessors, at a college, university, or educational institution that is accredited by an accrediting body or organization recognized by the Department of Education at the time the degree was obtained. In addition, I have successfully completed the North American Pharmacist Licensure Examination (NAPLEX), and applicable state law exam. Note: Applicants must maintain status as a certified Pharmacist, including completion of all requirements for Continuing Medical Education (CME) and re-certification. Licensure: All applicants must have passed the North American Pharmacist Licensure Examination (NAPLEX) and the Multistate Pharmacy Jurisprudence Examination (MPJE). Applicants also must be licensed to practice pharmacy in a State, the District of Columbia, the Commonwealth of Puerto Rico, or a territory of the United States. Medical Requirements: Distinguish basic colors In addition to the Basic Requirements, you must also meet the Minimum Qualifications stated below. MINIMUM QUALIFICATIONS: Your resume must demonstrate at least one (1) year of specialized experience equivalent to at least the next lower grade level in the Federal service obtained in either the private or public sector performing the following type of work and/or tasks :Experience includes: reviewing and approving prescription medications, providing guidance on appropriate pharmaceutical treatment plans, reviewing formulary medications and requests for non-formulary medications. Dispensing and administering medications including immunizations. Providing education on various medications and immunizations. Management of chronic diseases under a collaborative practice agreement Time In Grade Federal employees in the competitive service are also subject to the Time-In-Grade Requirements: Merit Promotion (status) candidates must have completed one year of service at the next lower grade level. Time-In-Grade provisions do not apply under the Excepted Service Examining Plan (ESEP).
